Release form and composition
The dosage form of the drug is a gel for local use: transparent, viscous, almost colorless, homogeneous gel (6 or 11 ml in disposable syringes, 1 syringe in a blister, 10 blisters in a cardboard box).
Composition of 100 ml gel:
- active substances: lidocaine hydrochloride - 2000 mg, chlorhexidine gluconate - 50 mg;
- excipients: propylene glycol, propyl 4-hydroxybenzoate, hydroxyethyl cellulose, methyl 4-hydroxybenzoate, purified water qs

Pharmacodynamics
Pharmacodynamic properties of the active components of Instillagel:
- chlorhexidine is an antimicrobial agent that acts on gram-negative and gram-positive microorganisms, dermatophytes, yeast; effective against causative agents of infections that are sexually transmitted - ureaplasmosis, chlamydia, trichomoniasis, gardnerellosis, syphilis, gonorrhea, genital herpes. Retains activity (slightly reduced) in the presence of various secretions, pus, blood and organic matter;
- lidocaine, a local anesthetic; adequate anesthesia and antimicrobial action develops within 5-10 minutes after the drug is used. The clear, water-soluble gel allows clear optical viewing in a variety of endoscopic procedures.
Indications for use
- gynecology and obstetrics (prevention of infectious complications, for anesthesia and lubrication): during prenatal sanitation of the vagina and cervix, insertion and removal of intrauterine contraceptives (IUDs), diagnostic curettage of the walls of the cervical canal and uterine cavity, hysteroscopy, biopsy of the endometrium, uterine biopsy abortions, polypectomy of the cervix during surgical interventions with transvaginal access, combined therapy for nonspecific colpitis, bacterial cervicitis, chronic endometritis;
- urology (prevention of infectious complications, for anesthesia and lubrication): during surgical interventions on the urethra and bladder, introduction and replacement of an indwelling catheter, urethroscopy and cystoscopy, urethral catheterization, urethral bougienage, transurethral prostatectomy, therapy of cystalgia and dysuria, urodynamic research nephrostomy, local treatment of non-gonorrheal urethritis;
- proctology and abdominal surgery (prevention of infectious complications, for anesthesia and lubrication): for all types of endoscopic examinations, replacement of fistular catheters and drains, care for entorostomy, diagnostic and surgical interventions in proctology, to protect against damage to the colon and rectum during endoscopic manipulations.
Also used as a disinfectant, local anesthetic and lubricant for mechanical ventilation (ALV), intubation and in pediatric practice.
Contraindications
- severe bradycardia (possibly suppression of the cardiovascular system);
- hypersensitivity to any component of the drug.
Instillagel should be used with caution during pregnancy (I trimester).
Instructions for the use of Instillagel: method and dosage
It is necessary to remove the tip and the syringe with the gel (6 or 11 ml of the drug) from the package. Then remove the protective cap from the syringe cannula and insert the syringe cannula tightly into the mouth of the tip. Fill the tube of the installation tip completely along the entire length of the tube by applying even pressure on the plunger of the syringe.
In men with urological diseases, a syringe with 11 ml of gel is used: in the area of the external opening of the urethra (after having previously treated the head of the penis with a disinfectant solution), apply a few drops of gel by pressing the syringe plunger, then insert the syringe cannula into the opening of the urethra through the applied gel and slowly inject 11 ml of gel into the urethra. Temporarily pinching the head of the penis in the area of the coronary groove to prevent the backflow of the gel.
When carrying out cystoscopy, it is necessary to completely cover the urethra with the gel, including the external sphincter. To do this, it is necessary to inject 11 ml of the drug into the urethra (if necessary, another 6-11 ml of the drug). For women, use a syringe with 6 ml of gel. On the area that surrounds the external opening of the urethra, you need to apply (pre-treated with a disinfectant solution) a few drops of gel and insert the syringe cannula into the opening of the urethra through the applied gel. Next, the contents of the syringe in a volume of about 4 ml are slowly introduced into the urethra and the syringe is removed, while continuing to press on the plunger. In this case, the gel covers the entire length of the urethra. Before carrying out the manipulations, it is recommended to leave the drug in the urethra for 5–10 minutes to achieve a complete disinfecting and anesthetic effect.
Children use a syringe with 6 ml of gel. The introduction of the drug in children is performed in the same way as in adults - according to the sex of the child.
In gynecology, a syringe with 6 ml or 11 ml of gel is used (depending on the risk of infection). To facilitate insertion into the cavity and cervix, an installation tip is used. The tube of the installation tip is filled along its entire length by pressing the plunger of the syringe. After treatment with a disinfectant solution of the external genital organs, cervix and vagina, the cervix is exposed in the mirrors. The gel covers the area of the external os of the cervix. Through the cervical canal, the tip is advanced into the uterine cavity, pre-sending the gel, and 2-3 ml (if necessary, 5-6 ml) of the gel is slowly injected into the uterine cavity. Continuing to press on the syringe plunger, the installation tip is removed from the cervix and the cavity (while the gel is coated over the entire length of the cervical canal). The following manipulations are carried out 10 minutes after the introduction of the gel.
Side effects
Allergic reactions - skin rash, allergic dermatitis, Quincke's edema; burning at the site of application.
Overdose
With severe damage to the urethra, side effects associated with the systemic action of lidocaine (collapse, convulsions, bradycardia) may develop; with the wrong use of the drug, orthostatic hypertension of central genesis is possible.
Treatment: in case of collapse - intravenous dopamine or epinephrine, in case of seizures - short-acting muscle relaxants (succinylcholine) or small doses of short-acting barbiturates, with bradycardia - beta-adrenostimulants (Alupent).

Application during pregnancy and lactation
According to the instructions, Instillagel should be used with caution in the first trimester of pregnancy. During lactation, it is recommended to refrain from breastfeeding for 12 hours after using the drug.
Drug interactions
Combined use with antiarrhythmic drugs can lead to an increase in the severity of side reactions of lidocaine.
Avoid the combined use of the gel with iodine preparations.
The local anesthetic effect of lidocaine is enhanced by monoamine oxidase (MAO) inhibitors.
Instillagel is incompatible with detergents containing an anionic group (sodium lauryl sulfate, saponins, sodium carboxymethyl cellulose) and with soap.
Allowed to be combined with drugs containing a cationic group (benzalkonium chloride).
