Release form and composition

Dosage forms of Imunofan:

• Solution for intramuscular (i / m) and subcutaneous (s / c) administration: clear, colorless liquid (5 or 10 ampoules in a blister strip, in a cardboard box 1 or 2 packs);
• Suppositories for rectal use: torpedo-shaped with a homogeneous structure from white to white with a yellowish tinge, may have a slight specific odor (5 or 10 pieces in a blister strip, in a cardboard box 1 or 2 packs);
• Dosed spray for nasal use: colorless, transparent or slightly opalescent liquid, a slight specific odor is allowed (8.5 ml each in a plastic bottle with a dosing device, 1 bottle in a cardboard box).

Pharmacological properties

Pharmacodynamics

When using Imunofan, three main effects are observed: weakening of multidrug resistance caused by proteins of the transmembrane transport pump of the cell, restoring the balance of the oxidative-antioxidant reaction of the body and regenerating the immune system.

Pharmacokinetics

The effect of the drug appears within 2-3 hours after administration (fast phase) and lasts for 4 months (medium and slow phases).

The fast phase begins 2–3 hours after the application of Imunofan and lasts up to 2–3 days. First of all, it is characterized by a detoxification effect, which is expressed in the inhibition of the breakdown of phospholipids of the cell membrane and the production of arachidonic acid with a further decrease in blood cholesterol levels and the synthesis of inflammatory mediators, normalization of lipid peroxidation processes and an increase in antioxidant protection by activating the synthesis of lactoferrin and ceruloplasmin and stimulating activity catalase. In case of infectious and toxic lesions of the liver, Imunofan suppresses the activity of transaminases and reduces the content of bilirubin in the blood serum, and also prevents cytolysis.

The middle phase after administration of the drug begins in 2-3 days and lasts up to 7-10 days. During this period, phagocytosis reactions intensify, and intracellular bacteria and viruses die.

The slow phase begins on the 7-10th day and lasts about 4 months. At this time, the immunoregulatory effect of Imunofan is noted, which contributes to the restoration of the disturbed parameters of humoral and cellular immunity. An activation of the production of specific antibodies is also observed, and the immunoregulatory index is normalized. The effect of Imunofan on the synthesis of antibacterial and antiviral antibodies of a specific nature is identical to the effect of certain therapeutic vaccines. Unlike the latter, the drug does not significantly affect the production of reaginic antibodies of the IgE class (immunoglobulin E) and does not enhance the immediate type of hypersensitivity reaction. The drug increases the production of IgA (immunoglobulin A) with its congenital deficiency.

Imunofan effectively inhibits multidrug resistance of tumor cells and increases their sensitivity to the action of cytostatic drugs.

Indications for use

According to the instructions, Imunofan is indicated for the prevention and treatment of chronic inflammatory diseases of various etiologies, immunodeficiency and toxic conditions.

In addition, the drug is prescribed:

• Solution for injection and rectal suppositories - for vaccination against viral and bacterial infections as an adjuvant (only for adults);
• Nasal spray - for the treatment of acute forms of infectious and inflammatory pathologies.

Contraindications

• Age up to 2 years;
• Hypersensitivity to the components of Imunofan.

Additionally, solution for injections and rectal suppositories are contraindicated for use in pregnancy complicated by Rh-conflict.

It is recommended to use a nasal spray with caution during pregnancy and breastfeeding.

Instructions for the use of Imunofan: method and dosage

The solution is intended for s / c or i / m injections, the drug is prescribed in courses, the daily (single) dose is 50 mcg.

Suppositories are used rectally in a daily (single) dose of 100 μg.

The spray is used intranasally, for this it is necessary (with the vertical position of the bottle and the patient's head) to insert the sprayer into the nasal passage and press all the way on the wide rim of the device. The use of a new bottle should be started by filling the dosing pump by pressing 3-4 times on the wide rim of the spray device. One spray dose contains 50 mcg of Imunofan. The maximum daily dose is 200 mcg.

The frequency of application of suppositories and injections of Imunofan - 1 time per day, spray - 1-2 times. The course of treatment depends on the clinical indications and the patient's age:

• Oncological pathologies in the scheme of combined radical (surgery, chemoradiation therapy) treatment: 8-10 injections or suppositories (1 dose of spray in each nasal passage for 8-10 days) before surgery and chemoradiation therapy, then at intervals of 15-20 days between courses throughout the entire period of therapy;
• A widespread tumor process (III-IV stage of oncological disease) of various localization (complex or symptomatic therapy): 8-10 injections or suppositories (1 dose in each nasal passage for 8-10 days), this course should be repeated after a break of 15 -20 days throughout the entire period of therapy;
• Malignant diseases of the lymphoid and hematopoietic systems in children: 10-20 injections or suppositories throughout the entire period of chemoradiation therapy and after completion to prevent the development of toxicosis;
• Papillomatosis of the larynx and oropharynx in children (as part of complex therapy): 10 injections or suppositories;
• Cytomegalovirus and herpes infection, cryptosporidiosis, toxoplasmosis, pneumocystosis, chlamydia: 15-20 injections or suppositories, or 1 dose of spray in each nasal passage 2 times a day for 10-15 days, repetition of courses is possible after a break of 2-4 weeks;
• HIV infection (as part of complex therapy): 15-20 injections or suppositories (1 dose in each nasal passage for 10-15 days), repeated courses are possible after a break of 2-4 weeks;
• Chronic forms of viral hepatitis and brucellosis: 15-20 injections or suppositories (1 dose in each nasal passage for 10-15 days), to prevent relapses after 2-3 months, it is recommended to carry out repeated courses of treatment (for a nasal spray - after 4 6 months);
• Diphtheria: 8-10 injections or suppositories, carriers of diphtheria bacteria - 1 time in 3 days, treatment period - 3-5 injections or suppositories;
• Psoriasis: 15-20 injections or suppositories;
• Burns of III-IV degree with signs of toxemia and septicotoxemia, septic endocarditis, purulent-septic complications after surgery, long-term non-healing wounds of the extremities: 7-10 injections or suppositories, in severe cases it is recommended to continue treatment for up to 20 days;
• Vaccine prophylaxis: for adults only - 1 injection (1 suppository) on the day of vaccination.

In the treatment of broncho-obstructive syndrome, rheumatoid arthritis, cholecystopancreatitis, Imunofan is prescribed 1 time in 3 days, the treatment period consists of 8-10 suppositories or injections, if necessary, the course should be extended according to the same scheme to 20 procedures.

The use of the spray for acute and chronic infectious and inflammatory diseases with symptoms of intoxication and immunodeficiency is carried out as part of a complex therapy daily, 1 dose in each nasal passage 2 times a day, for 10-15 days.
