Ixim Lupine is an antibiotic of the cephalosporin group.

Release form and composition

Dosage form - powder for suspension (50 ml) for oral administration: from almost white to pale yellow; ready-made suspension - from almost white to yellow, with a characteristic fruity odor (25 g in polyethylene bottles, in a cardboard box 1 bottle complete with a measuring cup or without it).

5 ml of the finished suspension contains:

  • active substance: cefixime F. USA (in the form of trihydrate) - 120.39 mg, which is equivalent to 100 mg of anhydrous cefixime;
  • additional components: colloidal silicon dioxide (Aerosil 200), sodium benzoate, sucrose, xanthan gum, strawberry flavor 052311 AR0551.

Pharmacological properties

Pharmacodynamics

Cefixime is a third generation semi-synthetic cephalosporin antibacterial agent. It has a bactericidal effect by disrupting the synthesis of the cell wall of pathogenic microorganisms. It is characterized by a broad spectrum of antimicrobial activity against various aerobic and anaerobic gram-positive and gram-negative bacteria, including Pseudomonas aeruginosa. Resistant to gram-positive and gram-negative beta-lactamases.

It is highly active against the following microorganisms: Haemophilus parainfluenzae, Serratia marcescens, Escherichia coli, Haemophilus influenzae, Neisseria gonorrhoeae (including penicillinase-producing strains), Moraxella catarrhalis (including lactamic acid bacteria) strains … (including Citrobacter diversus and Citrobacter amalonaticus), Pasteurella multocida, Proteus vulgaris, Proteus mirabilis, Providencia spp., Salmonella spp., Streptococcus pyogenes, Streptococcus pneumoniae, Streptococcus pneumoniae, Streptococcus pneumei. agalactiae, Shigella spp., The following microorganisms are resistant to cefixime: most strains of Staphylococcus (including methicillin-resistant strains), Pseudomonas spp., Bacteroides fragilis, Clostridium spp., Listeria monocytogenes, Enterococcus spp. (Streptococcus) serogroup D, Enterobacter spp.

Pharmacokinetics

When taken orally, the bioavailability of cefixime is 40-50%. Simultaneous food intake increases the time of maximum absorption by 0.8 hours.

After taking the suspension at a dose of 100, 200 and 400 mg C max (maximum concentration) in the blood serum is 1–1.3; 1-4.5 and 1.9-7.7 μg / ml, respectively. The time to reach C max for a suspension at a dose of 200 mg / 5 ml is 2–5 hours, at a dose of 400 mg / 5 ml - 2–6 hours.

After taking a suspension at a dose of 100, 200 and 400 mg, C max in urine is 73, 107 and 164 μg / ml, respectively.

The drug is stored for a long time in high concentrations in serum, urine and bile.

The connection with plasma proteins is 65–70%. The volume of distribution is 0.11 l / kg.

The half-life is 3-4 hours, with renal failure it increases to 6.4-11.5 hours.

50% is excreted unchanged by the kidneys, 10% - with bile.

Indications for use

The antibiotic Ixim Lupine is used to treat infectious and inflammatory diseases caused by microorganisms sensitive to cefixime, such as:

  • otitis media;
  • infections of the upper and lower respiratory tract: pharyngitis, sinusitis, tonsillitis, acute bronchitis and exacerbation of chronic;
  • uncomplicated urinary tract infections;
  • uncomplicated gonorrhea of the cervix and urethra.

Contraindications

  • children's age up to 6 months;
  • hypersensitivity to the drug, penicillamine or penicillins.

Elderly people, patients with colitis and renal failure, Ixim Lupine should be used with caution.

Instructions for use Ixim Lupine: method and dosage

A suspension for oral administration is prepared from the powder. To do this, turn the bottle over and shake the powder, open the lid and add about 17 ml of cooled boiled water (half the required volume), close the bottle with the lid and shake thoroughly until the powder dissolves, then add the remaining amount of water to the mark on the label (in the form of an arrow), close and shake well again until a homogeneous suspension forms. Let stand for 5 minutes. Shake the contents of the bottle well before each use.

Adults and children over 12 years old with body weight> 50 kg are usually prescribed 200 mg 2 times a day or 400 mg 1 time a day.

With uncomplicated gonorrhea of the cervix and urethra, a single dose of 400 mg of the drug is sufficient.

Children under 12 years old are prescribed 4 mg / kg 2 times a day at intervals of 12 hours or 8 mg / kg 1 time per day. These doses usually correspond to the following suspension volume:

  • 5-11 years old - 6-10 ml;
  • 2-4 years - 5 ml;
  • 6-12 months - 2.5-4 ml.

The duration of treatment is 7-10 days. For infections caused by Streptococcus pyogenes, the duration of therapy is determined individually, but is at least 10 days.

For patients with impaired renal function, the dose is adjusted depending on the creatinine clearance (CC) in the blood serum:

  • CC 21-60 ml / min and patients on hemodialysis - reduce the daily dose by 25%;
  • CC ≤ 20 ml / min and patients on peritoneal dialysis - reduce the daily dose by 50%.

Side effects

  • from the digestive system: stomatitis, nausea, dysbiosis, abdominal pain, constipation, diarrhea, cholestasis, vomiting, pseudomembranous enterocolitis, cholestatic jaundice;
  • on the part of hematopoietic organs: bleeding, agranulocytosis, leukopenia, hemolytic anemia, thrombocytopenia, pancytopenia, aplastic anemia, neutropenia;
  • from the nervous system: dizziness, headache, tinnitus;
  • from the urinary system: interstitial nephritis, impaired renal function;
  • from the respiratory system: shortness of breath;
  • from the genitourinary system: itching in the genital area, vaginitis, candidiasis;
  • laboratory parameters: increased prothrombin time, increased urea nitrogen, eosinophilia, hypercreatininemia, hyperbilirubinemia, increased activity of hepatic transaminases and alkaline phosphatase;
  • allergic reactions: fever, pruritus, skin flushing, urticaria, toxic epidermal necrolysis, exudative erythema multiforme;
  • others: hypovitaminosis

Overdose

In case of an overdose, the risk of developing / increasing adverse reactions increases.

There is no specific antidote for cefixime. Peritoneal and hemodialysis are ineffective. When taking a dose exceeding the recommended one, the stomach is washed, symptomatic and supportive therapy is carried out, including glucocorticosteroids, infusion therapy, and, if necessary, artificial ventilation of the lungs.

special instructions

In patients with a history of an allergic reaction to penicillins, hypersensitivity to cephalosporin antibiotics is possible.

The drug may interfere with the results of some laboratory tests. During treatment, it is possible to obtain a false-positive direct Coombs' reaction, a false-positive urine reaction to glucose (when using Benedict's or Fehling's solutions) and to ketone bodies (when using tests with nitroprusside). If it is necessary to conduct such studies during the period of cefixime therapy, it is preferable to carry out tests based on an enzymatic reaction using glucose oxidase.

Influence on the ability to drive vehicles and complex mechanisms

According to the instructions, Ixim Lupine does not affect the ability to drive vehicles and engage in other potentially hazardous activities that require increased attention and speed of psychomotor reactions.

Application during pregnancy and lactation

During pregnancy, the antibiotic Ixim Lupine can be used only in cases where the expected benefit to the woman outweighs the potential risks to the fetus.

If it becomes necessary to prescribe the drug to women who are breastfeeding, it is recommended to stop breastfeeding.

Pediatric use

The drug can be used in pediatrics for the treatment of infections indicated in the indications in children aged 6 months.

With impaired renal function

Treatment of patients with renal insufficiency should be carried out with extreme caution, under close medical supervision.

In old age, cefixime should be used with caution.

Drug interactions

Antacids containing magnesium and aluminum hydroxide slow down the absorption of cefixime.

Cefixime increases the level of carbamazepine in the blood serum, enhances the effect of indirect anticoagulants, and reduces the prothrombin index.

With the simultaneous use of tubular secretion blockers (including diuretics and allopurinol), they delay the excretion of cefixime by the kidneys, as a result of which toxicity may increase.
