Zoxon - alpha 1 -adrenergic blocker, antihypertensive agent; the drug is also effective in benign prostatic hyperplasia as a means of reducing urinary disorders.

Release form and composition

Dosage form - tablets: round or oblong (round - flat with engraving on one of the sides "ZX 1", oblong - biconvex with a dividing line and engraving "ZX 2" or "ZX 4" on one side), almost white or white ("ZX 1": 15 pcs. in blisters, in a cardboard box 1 or 2 blisters; "ZX 2": 10 pcs. in blisters, in a cardboard box 1 or 3 blisters; "ZX 4": 10 pcs. In blisters, in a carton box 3, 9 or 10 blisters).

The active substance is doxazosin mesylate, in 1 engraved tablet:

  • "ZX 1" - 1.213 mg, which is equivalent to 1 mg of doxazosin;
  • "ZX 2" - 2,425 mg, which is equivalent to 2 mg of doxazosin;
  • "ZX 4" - 4.85 mg, which is equivalent to 4 mg of doxazosin.

Auxiliary components: microcrystalline granular cellulose, lactose, microcrystalline cellulose, anhydrous colloidal silicon dioxide, sodium carboxymethyl starch (type A), magnesium stearate, sodium lauryl sulfate.

Pharmacodynamics and pharmacokinetics

When Zoxon is prescribed to patients with benign prostatic hyperplasia, the indicators of urodynamics are significantly improved and the manifestations of the symptoms of the disease decrease. The positive effect of the drug is due to the blockade of alpha 1 -adrenergic receptors, which are located in the capsule and stroma of the prostate gland and in the region of the bladder neck.

As a result of a decrease in the total peripheral vascular resistance, blood pressure is significantly reduced. A clinically significant hypotensive effect is achieved with the appointment of tablets once a day and lasts for 24 hours. In this case, blood pressure decreases gradually, and the maximum decrease is observed 2–6 hours after taking the pills.

Doxazosin has a positive effect on the lipid profile of the blood, significantly reducing the concentration of cholesterol and triglycerides and thereby contributing to a decrease in the likelihood of developing coronary heart disease. Zoxon reduces left ventricular hypertrophy, inhibits platelet aggregation and reduces the tendency to thrombus formation.

The drug can be used in patients with diabetes mellitus, patients with gout, bronchial asthma and in the elderly, since it does not have a negative effect on the general metabolism.

Doxazosin is well absorbed in the digestive tract (by 80–90%), but when taken with meals, the absorption of the drug slows down by 1 hour. The maximum plasma concentration is reached 1-2 hours after the application of Zoxon. Up to 98% of the drug binds to plasma proteins. The biotransformation of doxazosin occurs in the liver, the metabolites are pharmacologically inactive.

Bioavailability of Zoxon is 69-70%. In case of impaired liver function and taking drugs that alter hepatic metabolism, it is possible that the process of biotransformation of doxazosin is disrupted. Its elimination from plasma is biphasic, the terminal half-life is 22 hours. Inactive metabolites are excreted in the feces, and less than 5% of the taken dose of the drug is excreted unchanged. In patients with kidney disease, as well as in the elderly, the pharmacokinetics of doxazosin does not change significantly.

Indications for use

  • Arterial hypertension - monotherapy and in combination with angiotensin-converting enzyme (ACE) inhibitors, beta-blockers, thiazide diuretics, slow calcium channel blockers (to achieve sufficient blood pressure (BP) control);
  • Treatment of urination disorders in benign prostatic hyperplasia in patients with normal blood pressure and arterial hypertension.

Contraindications

  • Age under 18;
  • Hypersensitivity to doxazosin, quinazoline derivatives and drug components.

With caution should be prescribed for orthostatic hypotension, mitral or aortic stenosis, liver dysfunction.

Instructions for use of Zoxon: method and dosage

The tablets are taken orally, regardless of the meal, swallowing whole (without chewing) and drinking a little water. The frequency of admission is 1 time per day, in the morning or in the evening.

The doctor prescribes the dose and duration of treatment based on clinical indications.

Recommended dosage:

  • Arterial hypertension: the first dose - 1 mg 1 time per day (the best time is before bedtime, since after taking the patient must be in the supine position for 6-8 hours due to the risk of developing the phenomenon of the first dose, especially during preliminary therapy diuretics). Then, to achieve the required therapeutic effect, the dose is recommended to be increased several times every 1-2 weeks. The optimal antihypertensive effect is usually achieved with a daily dose of 8 mg. After reaching a stable blood pressure, the patient is recommended to transfer to a maintenance average daily dose of 2-4 mg. Depending on the severity of the condition and the characteristics of the course of the disease, the prescription of the dose can range from 1 mg to 16 mg per day, but no more;
  • Violation of urination against the background of benign prostatic hyperplasia: in the absence of arterial hypertension - 2-4 mg once a day. The maximum daily dose is 8 mg.

Usually Zoxon is taken over a long period.

Side effects

  • Cardiovascular system: often, especially at the beginning of therapy - orthostatic hypotension (in rare cases, it can cause fainting), edema; a causal relationship has not been established in clinical studies - chest pain, tachycardia, angina pectoris, bradycardia, palpitations, myocardial infarction, cerebrovascular accident, arrhythmia;
  • Respiratory system: rhinitis;
  • Nervous system: drowsiness, dizziness, headache;
  • General reactions: malaise, fatigue, asthenia;
  • Digestive system: nausea.

In addition, in patients with benign prostatic hyperplasia, side effects may develop:

  • Nervous system: hyperexcitability, insomnia, dry mouth, paresthesia, hypesthesia, tremor, impotence, priapism;
  • Digestive system: lack of appetite, flatulence, dyspepsia, abdominal pain, constipation or diarrhea, increased activity of liver enzymes, cholestasis, hepatitis, jaundice;
  • Respiratory system: shortness of breath, nosebleeds, bronchospasm, cough;
  • Urinary system: urinary incontinence, dysuria, hematuria, polyuria, nocturia;
  • Musculoskeletal system: muscle weakness, myalgia, arthralgia, muscle cramps;
  • Endocrine system: gynecomastia;
  • Sense organs: tinnitus, blurred vision;
  • Dermatological reactions: alopecia;
  • Hematopoietic organs: thrombocytopenia, leukopenia, purpura;

Overdose

With an overdose of Zoxon, a pronounced decrease in blood pressure is observed, sometimes with fainting.

In case of symptoms of an overdose, the drug should be discontinued, the patient should be laid on his back with his head down and his legs raised. It is also recommended to flush the stomach, prescribe activated charcoal and carry out symptomatic treatment. Dialysis in case of overdose of doxazosin is ineffective.

Patients with impaired liver function should be prescribed doxazosin with caution, especially while taking drugs that have a negative effect on the liver. If the functional indicators of the liver condition deteriorate, taking the tablets should be canceled immediately.

The effect of the first dose is especially pronounced with a sodium restricted diet and previous diuretic therapy.

Orthostatic reactions can be prevented if the patient adheres to smooth transitions from a horizontal position to a standing position.

Drinking alcohol increases the risk of side effects.

Malignancy of the prostate gland must be excluded before starting treatment with the drug.

In patients with benign prostatic hyperplasia and arterial hypertension, the drug effectively affects both pathologies.

During the period of Zoxon's use, patients are advised to refrain from potentially hazardous activities, including driving vehicles and mechanisms.

Application during pregnancy and lactation

Experience with the use of doxazosin in patients with high blood pressure during pregnancy and breastfeeding is limited. Experimental studies have not revealed embryogenic and teratogenic effects, but despite this, the drug is prescribed to pregnant and lactating women only after the ratio of the expected benefits and possible risks.

Pediatric use

According to the instructions, Zoxon is contraindicated in children and adolescents under 18 years of age.

For violations of liver function

The drug is prescribed with caution to patients with impaired liver function.

Drug interactions

No signs of negative interaction were noted when Zoxon was combined with beta-blockers, thiazide diuretics, furosemide, ACE inhibitors, slow calcium channel blockers, antibiotics, uricosuric and oral hypoglycemic drugs, indirect anticoagulants.

With the simultaneous use of doxazosin:

  • Drugs for the treatment of arterial hypertension can cause mutual enhancement of the hypotensive effect;
  • Cimetidine increases the plasma concentration of doxazosin;
  • Indomethacin (especially) and other non-steroidal anti-inflammatory drugs, sympathomimetic drugs, estrogens (retain fluid) can help reduce the antihypertensive effect of the drug;
  • Inducers of microsomal oxidation in the liver can increase the effect of doxazosin, inhibitors can decrease it;
  • Epinephrine loses its alpha-adrenostimulating effects (there is a risk of arterial hypotension and tachycardia).
