The dosage form of the release is a solution for intravenous and intramuscular administration: transparent, colorless or somewhat colored, has a slight smell of hydrogen sulfide (in 1 ml ampoules, in blisters of 5 ampoules, in a cardboard box 1 package; in 5 ml ampoules, in blister packs of 1 or 5 ampoules, in a cardboard box 1 package).
Composition of 1 ml solution:
- active substance: serotonin adipate - 10 mg;
- auxiliary components: unitiol - 1.5 mg, water for injection - up to 1 ml.
Pharmacological properties
Pharmacodynamics
Dinaton is a hemostatic and serotonergic drug. The hemostatic effect of the drug is due to its ability to increase the resistance of capillaries and platelet aggregation, as well as a decrease in bleeding time.
Serotonin has an antidiuretic effect, leads to vasoconstriction of the kidneys, normalizes the contractile activity of smooth muscles of internal organs (peristalsis, endogenous vasomotor).
The normalization of smooth muscle function is manifested by stabilization of hemodynamics, a decrease in local hypoxia, and restoration of the motor-evacuation function of the gastrointestinal tract.
Pharmacokinetics
In blood plasma, serotonin is rapidly deaminated and loses its activity. It has no cumulative effect.
Indications for use
- hemorrhagic syndrome: with malignant neoplasms, Werlhof's disease, radiation sickness (chronic, acute and subacute), against the background of treatment with cytostatics;
- functional intestinal obstruction;
- a- and hypoplastic anemia;
- hemorrhagic vasculitis;
- thrombocytopenia.
Contraindications
- glomerulonephritis in acute and chronic course;
- bronchial asthma;
- kidney disease;
- anuria and oliguria;
- acute thrombosis;
- arterial hypertension;
- diseases occurring with hypercoagulability;
- angioneurotic edema;
- individual intolerance to the components of the drug.
Instructions for use of Dinaton: method and dosage
With functional intestinal obstruction (for treatment and prevention), the drug is administered intravenously into the central vein; the rate of introduction is 5–10 mg / h. 10 mg of the drug must first be dissolved in a physiological solution with a volume of 100-200 ml. When injected into a peripheral vein, 10–20 mg should be dissolved in 200–400 ml of saline. The drug can be administered at 360 mg or more per day.
For intravenous administration, 20-50 mg must be diluted in 200-400 ml of 0.9% sodium chloride solution; injected drip (30 drops per minute) 2-3 times a day until the condition improves (manifests itself as the appearance / intensification of weakened peristalsis and clinical manifestations of restoration of the motor-evacuation function of the gastrointestinal tract).
The initial dose when using Dinaton for other indications is 5 mg. If there are no side reactions, the dose may be doubled. Do not use more than 15–20 mg per day. The average duration of the course is 10 days.
For intravenous administration, 5-10 mg of the drug is diluted in 100-150 ml of 0.9% sodium chloride solution, or in 5-10 ml of 0.9% sodium chloride solution with further dilution in 100-150 ml of 5% dextrose solution (preserved blood, plasma). It is administered by drip (30 drops per minute) no more than 2 times a day.
For intramuscular administration, 5-10 mg of serotonin adipate is diluted in 5 ml of 0.5% lidocaine solution. It is introduced 2 times a day with a break of at least 4 hours.
Side effects
- digestive system: nausea, abdominal pain, diarrhea;
- urinary system: decreased urine output;
- cardiovascular system: increased blood pressure, cardialgia;
- respiratory system: difficulty breathing;
- others: with a rapid intravenous administration of the solution - pain along the vein; with intramuscular injection - a feeling of heaviness in the occipital region, soreness at the injection site.
Overdose
The main symptoms: tachypnea, hyperemia of the skin.
No specific treatment is required. The introduction of Dinaton should be paused for 5-10 minutes, then it can be continued at a slower rate.
special instructions
In cases of manifestation of adverse reactions, administration is suspended for 2-3 minutes, after which it is resumed at a slower rate.
Application during pregnancy and lactation
The efficacy / safety of the drug Dinaton in pregnant women has not been studied, since it is rapidly deaminated in the bloodstream.
The lactation period is not a contraindication to the use of the drug.
With impaired renal function
According to the instructions, Dinaton is contraindicated in patients with kidney disease.
Drug interactions
With the combined use of certain drugs / substances, the following effects may develop:
