Diclo-F is a non-steroidal anti-inflammatory drug (NSAID) for topical use in ophthalmology.

Release form and composition

Dosage form - eye drops 0.1%: a clear solution from colorless to light yellow (5 ml each in dark glass bottles complete with a dropper or in dropper bottles, 1 bottle in a cardboard box).

Active ingredient: diclofenac sodium, 1 ml - 1 mg.

Additional substances: polyoxyl 35 castor oil, boric acid, tromethamine, disodium edetate, water for injection, benzalkonium chloride.

Pharmacodynamics and pharmacokinetics

Diclofenac has a local antipyretic, anti-inflammatory and analgesic effect. The effect of this substance is due to the indiscriminate inhibition of the activity of cyclooxygenase 1 and 2, which makes it possible to suppress the synthesis of prostaglandins in the focus of inflammation.

Diclo-F eye drops cause the following effects:

• reduction of eye inflammation resulting from surgery, trauma, or infection;
• reduction of miosis during surgical operations;
• decreased synthesis of prostaglandins in the moisture of the anterior chamber of the eye.

The drug is able to penetrate well into various tissues of the eye (except for the lens). The maximum concentration is reached within 30 minutes (with the highest concentration in the moisture of the anterior chamber). Despite the systemic absorption of the drug, the concentration of diclofenac in the blood does not reach the detection limit and has no clinical significance.

Indications for use

• Suppression of miosis during cataract surgery;
• Treatment of non-infectious conjunctivitis;
• Treatment and prevention of inflammatory processes after eye surgery;
• Treatment and prevention of post-traumatic inflammation in non-penetrating and penetrating wounds of the eyeball (in combination with local antibacterial drugs);
• Prevention of cystic macular edema after cataract surgery.

Contraindications

Absolute:

• Exacerbation of the erosive and ulcerative process in the gastrointestinal tract;
• Violation of hematopoiesis of unknown origin;
• Hypersensitivity to the components of the drug, acetylsalicylic acid or other NSAIDs.

Relative (diseases / conditions in which eye drops should be used with caution due to the likelihood of complications):

• Epithelial herpetic keratitis, including a history;
• Diseases that cause bleeding disorders (including bleeding tendency, prolonged bleeding time, hemophilia);
• Elderly age.

There is no data on the safety of using the drug in childhood, during pregnancy and lactation, so drops can be prescribed only in cases where the intended benefits outweigh the possible risks.

Instructions for the use of Diclo-F: method and dosage

According to the instructions, Diclo-F must be dripped into the conjunctival sac of the affected eye.

• Inhibition of intraoperative miosis: instill 4 times within 2 hours before surgery at intervals of 30 minutes;
• Prevention of cystic edema of the macula: 1 drop 3-4 times a day within 2 weeks after surgery;
• Other indications: 1 drop 3-4 times a day, depending on the severity of the disease. Duration of treatment is 1-2 weeks.

Side effects

• Organ of vision: rarely - blurred vision (immediately after instillation), burning, iritis, corneal opacity;
• Allergic reactions: flushing, itching in the eyes, chills, fever, photosensitivity, urticaria or other skin rash (including erythematous), exudative erythema multiforme, angioedema of the face;
• Gastrointestinal tract: nausea, vomiting.

Overdose

There are no data on overdose.

Patients using contact lenses should remove them 5 minutes before instilling the drug and install no earlier than 15 minutes later.

During the application of Diclo-F, care should be taken not to touch the eye with the tip of the pipette. Close the bottle tightly after each instillation.

During the treatment period, it is recommended to refrain from driving vehicles and performing potentially hazardous work requiring visual acuity, increased attention and speed of reactions.

Application during pregnancy and lactation

There are no data on the use of eye drops during pregnancy and breastfeeding. The use of non-steroidal anti-inflammatory drugs in the third trimester of pregnancy can lead to impaired blood circulation in the fetus.

Prescribing the drug during pregnancy and lactation is possible only by the attending physician after analyzing the ratio of the possible benefits to the mother and the potential risk to the child.

Drug interactions

• Methotrexate, diflunisal, sulfonylureas, other NSAIDs, including acetylsalicylic acid in high doses (3 g or more per day): there is a risk of bleeding from the gastrointestinal tract;
• Lithium preparations, indirect anticoagulants, digitoxin: their effect is enhanced.

If necessary, other medications for topical use in ophthalmology (including glucocorticosteroids) may be simultaneously prescribed, however, at least 5-minute intervals should be observed between instillations.
