Release form and composition
The drug is produced in the form of two types of tablets (21 pcs. In a blister, 1 or 3 blisters in a cardboard box with a calendar scale):
- The tablets are biconvex, round, white, on one side - the code "D" (11 pcs. In a blister);
- Tablets are flat, round, blue, with a beveled edge (10 pcs. In a blister).
The composition of 1 white tablet contains the active substance: estradiol valerate - 2 mg.
The composition of 1 blue tablet contains active substances: estradiol valerate - 2 mg; medroxyprogesterone acetate - 10 mg.
Auxiliary components: Kollidon 25, lactose monohydrate, corn starch, magnesium stearate, gelatin, indigo carmine (E132), talc, purified water.
Pharmacodynamics and pharmacokinetics
Estradiol valerate is an ester of estradiol, a natural estrogen. Estrogens have the ability to bind to specific receptors, which contributes to the growth of the endometrium and the development of secondary sexual characteristics.
Medroxyprogesterone acetate is a derivative of 17-alpha-hydroxy-6-methylprogesterone, a natural progesterone. This substance has the ability to bind to progestin-specific receptors and transfer the endometrium from the proliferative to the secretory phase.
Estradiol not only stimulates the growth of the endometrium, but also increases the risk of developing endometrial hyperplasia and cancer. To counteract this effect, medroxyprogesterone acetate is added to the drug.
With oral administration, the absorption of estradiol valerate occurs in the gastrointestinal tract. With the help of esterases in the liver and the wall of the small intestine, this substance is hydrolyzed to estradiol. Circulating estradiol has the ability to bind to plasma proteins (mainly serum albumin and sex hormone binding globulin). The maximum concentration of estradiol in plasma is observed after 4-6 hours. Metabolites in the form of sulfate and glucuronide conjugates are excreted in the urine. A small part of the drug is excreted in feces.
After oral administration of one tablet, medroxyprogesterone acetate is extensively metabolized in the liver by conjugation and hydroxylation. This substance has the ability to bind to blood plasma proteins (mainly albumin) by more than 90%. The maximum concentration of medroxyprogesterone acetate in blood plasma is observed after 1–2 hours. The substance is excreted in bile and urine, the half-life is 24 to 48 hours.
Indications for use
Divina is prescribed to women in menopause to relieve symptoms arising from a deficiency of progesterone and estrogen.
Also, the drug is indicated for postmenopausal women for the prevention of osteoporosis.
Contraindications
- Chronic and acute liver diseases;
- Sickle cell anemia;
- Arterial hypertension, showing resistance to therapy;
- Pathology of cerebral vessels or thromboembolic disorders in the anamnesis (including in the family);
- Tumors of the pituitary gland;
- Endometriosis;
- Estrogen-dependent tumors (cancer of the uterus, breast);
- Pregnancy;
- Hypersensitivity to drug components.
Instructions for use of Divina: method and dosage
Divina should be taken orally, preferably in the evening.
The drug is taken for 21 days, 1 tablet per day. After that, take a break for 7 days. During this period, menstrual bleeding should occur.
For the first 11 days, you need to take white pills (containing estrogen), then over the next 10 days - blue pills containing 2 components (estrogen and progesterone).
In cases where the normal menstrual cycle has stopped or has become irregular, the drug can be started at any time. Also, therapy can be started on the fifth day of menstruation.
If you miss a dose, you must take it within the next 12 hours. In the future, the drug is taken according to the usual scheme.
Side effects
During therapy, in rare cases, it is possible to develop disorders from some body systems:
- Reproductive system: engorgement of the mammary glands, metrorrhagia (violation of the regularity of bleeding);
- Digestive system: nausea;
- Central nervous system: increased fatigue, headache;
- Others: feeling of heaviness in the lower limbs.
Overdose
Overdose symptoms are headache, nausea and uterine bleeding. There is no specific antidote, symptomatic treatment is recommended.

Due to the fact that in menopause the duration of the menstrual cycle can be different, bleeding in some women may begin even while taking the blue pills. In this case, you need to temporarily stop taking Divina, and after the bleeding ends, resume taking the white pills.
Drug interactions
According to the instructions, Divina can reduce the effectiveness of oral hypoglycemic drugs, oral anticoagulants and antihypertensive drugs.
Its effect is reduced by anticonvulsants, barbiturates, rifampicin and tetracyclines in case of simultaneous use.
