Diacerein is an anti-inflammatory non-steroidal drug.
Release form and composition
The drug is produced in the form of capsules: gelatinous hard, size No. 1, blue, with black marking Dr. REDDY'S - on the lid, and Movagain white markings on the body. The capsule contains a yellow granular powder. The drug is packaged in polyethylene laminated aluminum strips of 10 capsules, in a cardboard box of 3 or 6 strips.
1 capsule contains:
- active substance: diacerein - 50 mg;
- excipients: lactose monohydrate - 222.5 mg, croscarmellose sodium - 18 mg, colloidal silicon dioxide - 3 mg, hypromellose 3 cps - 3 mg, magnesium stearate - 1.5 mg;
- capsule: propyl parahydroxybenzoate, methyl parahydroxybenzoate, sodium lauryl sulfate, titanium dioxide (E171), blue proprietary dye (E131), azorubin dye (E122), water, gelatin;
- ink: black - ethanol, shellac, isopropanol, butanol, iron oxide black dye (E172), macrogol, concentrated ammonia solution; white - ethanol, shellac, isopropanol, titanium dioxide, concentrated ammonia solution, polysorbate-80.

Pharmacodynamics
A diacetylated rhein derivative and an anthraquinoline derivative is metabolized to the active metabolite of rhein, thereby reducing the activity of interleukin-1, which plays a special role in the development of inflammation and cartilage degradation in osteoarthritis.
It inhibits the action of other cytokines (including TNF alpha, interleukin-6). Effective after 2-4 weeks.
Pharmacokinetics
Absorption from the gastrointestinal tract (gastrointestinal tract) is fast, bioavailability when taken together with food increases by 25%.
The time to reach the highest concentration (Tc max) is 144 minutes. With a single dose of 50 mg, the maximum concentration (C max) is 3.15 mg / l, with repeated administration, due to the cumulation of the drug, C max increases. Fully deacetylated to rhein. The connection of rhein with albumin (proteins) reaches 100%. The Rhine crosses the blood-brain and placental barriers.

Instructions for the use of Diacerein: method and dosage
According to the instructions, Diacerein is administered orally (inside) after meals, the capsule should be swallowed whole with a sufficient amount of water. Apply one capsule (50 mg) twice a day (morning and evening).
During the first two weeks, when taking the drug, there may be an acceleration of transit in the intestine, in this case, treatment should be started with one capsule per day. It must be taken in the evening, simultaneously with food, for four weeks, then the dose is increased to two capsules (100 mg) per day.
The course of treatment is at least four months.
Side effects
- digestive system: nausea, vomiting, abdominal pain, diarrhea, general malaise;
- allergic reactions: rash, itching.
Intensive coloration of urine (depending on pH) from brown to yellow is infrequently noted (there is no clinical significance, does not require dose reduction or drug withdrawal).
Overdose
Overdose may cause symptoms such as weakness and diarrhea.
With severe diarrhea, monitoring of the electrolyte composition of the plasma is necessary.

Before the onset of a therapeutic effect, it is possible to combine it with analgesic drugs (including non-steroidal anti-inflammatory drugs). Periodic monitoring of blood parameters, functional urine samples, liver is required. If renal function worsens, the dose of the drug must be reduced.

The effect of the drug on the speed of psychomotor reactions and concentration of attention when driving vehicles and other theoretically dangerous activities is unlikely.
Application during pregnancy and lactation
Studies on the use of the drug during pregnancy and lactation, confirming safety, have not been conducted. There is no information on the effect on fetal development.
With impaired renal function
For patients with chronic renal failure whose creatinine clearance is less than 30 ml / min, it is necessary to reduce the daily dose of diacerein to 50 mg, and subsequently adjust it in accordance with clinical efficacy and tolerability. In chronic renal failure, the drug must be used with caution.
Drug interactions
Antacids help to reduce the absorption of the drug. When taken simultaneously with medicines that affect the intestinal microflora (including antibiotics), as well as with substances that change the quality of the intestinal contents and the rate of emptying (fiber), the frequency of side effects from the intestines increases.
Medicines containing aluminum hydroxide and / or magnesium hydroxide reduce the bioavailability of diacerein.
The simultaneous use of laxatives is not recommended.
