Gliatilin is a nootropic drug, a centrally acting cholinomimetic.

Release form and composition

Dosage forms:

  • Solution for intravenous (i / v) and intramuscular (i / m) administration: colorless, transparent, odorless liquid (4 ml in colorless glass ampoules, 3 ampoules in a blister strip, 1 pack in a cardboard box);
  • Capsules: oval, soft, matte, yellow gelatinous structure, inside contain a colorless solution of a viscous consistency (14 pieces in blisters, in a cardboard box 1 blister).

The active ingredient is choline alfoscerate:

  • 1 ml of solution - 250 mg;
  • 1 capsule - 400 mg.

Excipients:

  • Solution: water for injection;
  • Capsules: purified water, glycerol.

The composition of the capsule shell: esitol, sodium ethyl parahydroxybenzoate, sorbitan, sodium propyl parahydroxybenzoate, iron (III) meta-hydroxide (E172), titanium dioxide (E171), gelatin.

Pharmacological properties

Pharmacodynamics

The active ingredient of Gliatilin is choline alfoscerate, a nootropic agent with neuroprotective properties.

The drug improves the transmission of nerve impulses in cholinergic neurons. Has a beneficial effect on the function of receptors and the plasticity of neuronal membranes. Strengthens metabolic processes in the brain, improves cerebral blood flow, activates the structures of the reticular formation of the brain, restores consciousness in case of traumatic brain damage.

Gliatilin has a corrective and prophylactic effect on some pathogenetic factors of the involutionary psychoorganic syndrome, such as a decrease in cholinergic activity and a change in the phospholipid composition of neuronal membranes.

In experimental studies, it was found that the drug stimulates the dose-dependent release of acetylcholine under physiological conditions of neurotransmission.

In the human body, alfoscerate is broken down by enzymes into choline and glycerophosphate.

As a choline donor, Gliatilin enhances the synthesis of acetylcholine and has a positive effect on neurotransmission. Glycerophosphate is involved in the synthesis of phosphatidylcholine (membrane phospholipid). Both of these phenomena together have a positive effect on membrane elasticity and receptor function, as a result of which synaptic transmission is improved.

Thus, according to pharmacodynamic studies, Gliatilin affects synaptic (including cholinergic) neurotransmission, neuronal membrane plasticity and receptor function.

Pharmacokinetics

When taken orally, the absorption of choline alfoscerate is about 88%.

The drug, after entering the human body, accumulates mainly in the brain (concentration reaches 45% of the level in the blood), liver and lungs. Penetrates well through the blood-brain barrier.

It is excreted mainly by the lungs in the form of carbon dioxide - 85%, the remaining 15% are excreted through the intestines and kidneys.

Choline alfoscerate is not mutagenic and teratogenic. Does not affect reproductive functions.

Indications for use

  • Cerebral circulation disorders of the hemorrhagic type during the recovery period, of the ischemic type - in the acute and recovery period;
  • Acute stage of traumatic brain injury, mainly with stem lesions, including in a coma or impaired consciousness;
  • Involutional and degenerative psychoorganic syndromes, consequences of cerebrovascular insufficiency - primary and secondary disorders of mnestic functions (memory impairment, disorientation, confusion, decreased ability to concentrate, motivation and initiative);
  • Multi-infarction dementia;
  • Changes in the behavioral and emotional sphere: decreased interest, increased irritability, emotional lability;
  • Senile pseudomelancholia.

Contraindications

Contraindications to the use of Gliatilin:

  • The period of pregnancy and breastfeeding;
  • Hypersensitivity to drug components.

Instructions for the use of Gliatilin: method and dosage

The solution is administered intravenously or intramuscularly. A solution for infusion is being prepared in the proportion: 4 ml (1 ampoule) of the drug to 50 ml of saline. The rate of administration is 60-80 drops per minute. Dosing in acute clinical conditions: for intravenous administration - 1-3 ampoules per day, intramuscularly - 1 ampoule per day. Treatment lasts until positive dynamics appear, usually up to 10 days, then the patient is transferred to taking capsules.

Gliatilin capsules are taken orally, before meals. Dosage for the treatment of multi-infarction dementia, cerebrovascular insufficiency, changes in the behavioral and emotional sphere - 400 mg 3 times a day. The treatment period is 3-6 months.

Gliatilin in children with various types of delayed motor, mental or speech development, as well as in the case of perinatal encephalopathy and in the aftermath of birth trauma, for a number of reasons, is recommended to be used in the form of intramuscular injections. This recommendation is justified by the fact that when using a solution it is easier to measure the dose the child needs, and also by the fact that the course of treatment with Gliatilin in the form of a solution is shorter than when taking capsules, and is about 2 weeks, while capsules are required to be used within 4– 12 weeks. In addition, the effectiveness of the use of Gliatilin in the form of a solution for injection is significantly higher than when taking capsules.

Dosing regimen of solution for i / v and i / m administration:

  • developmental delay, perinatal encephalopathy or the consequences of birth trauma (intramuscularly): at the age of 1 year - 250-500 mg (1-2 ml of solution), at the age of 1 to 3 years - 500-750 mg (2-3 ml), over the age of 3 years - 1000 mg (4 ml). Frequency rate of application - 1 time per day, duration of treatment - 10-12 days;
  • concussion or contusion of the brain, traumatic brain injury (intravenous or intramuscular): at a dose of 1000 mg (4 ml) once a day for 7 days (starting from the first day of injury). At the end of the weekly course of therapy, they switch to taking capsules.

Dosing regimen of Gliatilin capsules:

  • developmental delay, perinatal encephalopathy or the consequences of a birth injury: at the age of over 1 year - 1 capsule 3 times a day for 30–90 days;
  • concussion or contusion of the brain, traumatic brain injury: after a week of therapy with the drug in the form of a solution for intravenous and intramuscular administration, 1 capsule is prescribed 2 times a day for 60 days.

If it is difficult for young children to swallow capsules, pierce the capsule with a needle on a syringe, pull out its contents and, removing the needle, drip it into the child's mouth, and then give it to drink with plain water.

Side effects

Even long-term treatment is usually well tolerated by patients. But during the use of the drug, side effects in the form of allergic reactions, nausea are sometimes possible.

Overdose

An overdose of Gliatilin usually manifests itself with the following symptoms: nausea, vomiting, dyspeptic disorders (feeling of heaviness in the stomach, flatulence, bloating, constipation / diarrhea, belching, etc.). You should rinse the stomach and take a sorbent (for example, activated carbon). If the disorders are not very pronounced, it is enough to reduce the dose of the drug. If the symptoms are significantly pronounced, it is necessary to temporarily stop taking the drug. You can resume application after stabilization of the condition.

According to the instructions, Gliatilin does not affect the ability to drive vehicles and perform potentially hazardous activities that require a high speed of psychomotor reactions.

Application during pregnancy and lactation

In animal studies, mutagenic and teratogenic effects of choline alfoscerate were not detected, however, Gliatilin is not recommended for use in pregnant women and breastfeeding mothers.

If pregnancy occurs during treatment, the drug should be canceled. Screening tests are also recommended (including ultrasound at 12 and 20 weeks of gestation and an angiotensin converting enzyme concentration test).

If necessary, a course of therapy during lactation is recommended to stop breastfeeding.

Pediatric use

For young children, Gliatilin is often prescribed to eliminate the consequences of birth trauma, perinatal encephalopathy, as well as delayed mental, motor and speech development. In rare cases, the drug is used to treat bruises or concussions, traumatic brain injury.

Gliatilin in the form of a solution for injection is recommended for children under 7 years of age, as they require a lower dose than adults. When taking the drug in capsule form, the doses for children are usually the same as those for adults.
