Hepatrombin G is an antihemorrhoidal drug for local use with antithrombotic, anti-inflammatory and venosclerosing effects.

Release form and composition

  • rectal suppositories: conical, homogeneous, white on the outside and on the cut (5 pcs. in polyvinyl chloride cell contour packs, 2 packs in a cardboard box);
  • ointment for external and rectal use: a translucent yellowish-white gel-like mass with a specific odor (20 g each in aluminum tubes with a hole protected by an aluminum membrane, the screw cap is equipped with a device for membrane perforation; the set includes a tip for intrarectal administration; in cardboard pack 1 set).

Composition for 1 rectal suppository:

  • active substances: sodium heparin - 120 IU; lauromacrogol 600 - 30 mg; prednisolone acetate - 1.675 mg;
  • auxiliary components: colloidal silicon dioxide, medium-chain triglycerides, glyceryl tristearate, Novata B (solid fat).

Composition for 1 g of ointment for external and rectal use:

  • active substances: sodium heparin - 65 ME; lauromacrogol 600 - 30 mg; prednisolone acetate - 2.233 mg;
  • auxiliary components: viscous paraffin, solid paraffin, colloidal silicon dioxide, anhydrous lanolin.

The anticoagulant activity of 1 IU of sodium heparin corresponds to the activity of 7.7 μg of the II International Standard for Heparin [according to the classification of the World Health Organization (WHO)].

The combination of the active components of Hepatrombin G provides its antithrombotic, anti-inflammatory and venosclerosing effect when applied topically in proctology.

Pharmacodynamic properties of the active ingredients of the drug:

  • heparin: a direct-acting anticoagulant that affects blood coagulation factors directly in the vascular bed; as a result of external use, it has a local anti-exudative, antithrombotic and moderate anti-inflammatory effect; improves the regeneration of connective tissue, prevents blood coagulation in hemorrhoids;
  • prednisolone: a glucocorticosteroid (GCS) with anti-inflammatory, anti-exudative and anti-allergic action, which reduces burning sensation, itching and pain in the anorectal region;
  • lauromacrogol 600: venosclerosing agent with local anesthetic effect, promotes adhesion and overgrowth of hemorrhoids.

Pharmacokinetics

  • heparin: when absorbed, it is biotransformed in the liver with the participation of the hepatic heparinase enzyme and partially in the reticuloendothelial system (RES) of macrophages; up to 95% of the substance binds to plasma proteins; the volume of distribution (V d) increases with increasing dose; heparin is eliminated by the kidneys, the half-life (T 1/2) is from an hour to one and a half hours;
  • prednisolone: being absorbed, it is biotransformed in the liver; about 1% is eliminated by the kidneys unchanged; the biological T 1/2 is from 18 to 36 hours, and the T 1/2 from plasma is from 115 to 212 minutes.

Indications for use

Hepatrombin G is recommended for the local treatment of external and internal hemorrhoids, thrombophlebitis of the anus veins, anal fissures, fistulas, eczema and itching in the anus.

Rectal suppositories are also used in the preparation of the patient for surgery in the anus and as part of the complex treatment of operated or thrombosed hemorrhoids.

Contraindications

  • skin lesions of bacterial, viral, fungal etiology;
  • reactions to vaccinations;
  • syphilis;
  • tuberculosis;
  • skin tumors;
  • bleeding tendency;
  • pregnancy (I trimester);
  • children under 12 years of age (for rectal suppositories);
  • hypersensitivity to the active and auxiliary components of the drug.

Instructions for the use of Hepatrombin G: method and dosage

  • external use: the ointment is applied 2–4 times a day to the affected skin with a thin layer; after the passage of painful sensations, the frequency of application is reduced to 1 time per day, therapy is continued for 1 week;
  • rectal administration (Hepatrombin G is injected into the anus after a bowel movement): suppositories - 1-2 times a day; the ointment is injected using a tip screwed onto the tube, and a small amount is squeezed out by lightly pressing on the tube.

Side effects

Hepatrombin G can cause local side effects: hypersensitivity reactions, skin hyperemia.

As a result of long-term treatment and / or application of the ointment to large skin surfaces, systemic side reactions are possible, for example, edema, slowing down of the regeneration of cracks.
