Heparidex is a combined direct-acting anticoagulant drug for topical use.

Release form and composition

Dosage form - gel for external use: almost transparent or transparent, colorless or with a slightly yellowish tinge, opalescence is possible, with a specific odor (30 or 50 g in aluminum tubes, in a cardboard box 1 tube and instructions for use of Heparidex).

Composition of 1 g of gel:

  • active substances: dimethyl sulfoxide - 150 mg, dexpanthenol - 25 mg, sodium heparin - 4.17 mg (in terms of dry matter);
  • auxiliary components: Cremophor RH 40 (polyoxyl 40 hydrogenated castor oil, macrogolglycerol hydroxystearate), carbomer 940 (carbopol 980), trometamol (tromethamine), isopropanol (isopropyl alcohol), lavender oil, purified water.

Pharmacological properties

Pharmacodynamics

Heparidex is a drug of combined composition, the action of which is due to the properties of its active substances:

  • dimethyl sulfoxide is an anti-inflammatory and anti-edema agent due to its ability to inactivate hydroxyl radicals and improve metabolic processes in the focus of inflammation. By reducing the rate of nociceptive (pain) impulses in peripheral neurons, it has a local analgesic effect. Has a high (more than 50%) ability to penetrate through biological membranes, including through the skin, which ensures a deeper penetration of other active substances of the drug into tissues;
  • dexpanthenol is an anti-inflammatory and dermatoprotective agent. Improves metabolic processes, which contributes to the regeneration of damaged tissues;
  • heparin is a direct-acting anticoagulant with an anti-inflammatory effect. By inhibiting the activity of hyaluronidase, it improves the regeneration of connective tissue. Activates the fibrinolytic properties of blood. Prevents blood clots. Improves local blood flow.

Pharmacokinetics

The physiological plasma concentration of dimethyl sulfoxide is normally 40 ng / ml. 6 hours after external application of Heparidex, the maximum plasma concentration reaches 120 ng / ml and lasts up to 12 hours. Approximately 12-25% of the absorbed drug is excreted during the first day, 37-48% is excreted over 7 days unchanged through the kidneys or as the main metabolite of dimethylsulfone. About 3.5–6% of the total amount of dimethyl sulfoxide is excreted as a metabolite through the lungs 6–12 hours after drug administration. The half-life (T ½) is 11-14 hours.

When applied externally, dexpanthenol is absorbed and biotransformed into pantothenic acid, which is part of coenzyme A, a coenzyme that plays an important role in oxidation and acetylation processes.

When applied externally, heparin is absorbed slightly.

Indications for use

  • bruises, closed injuries;
  • edema, hematoma and inflammation of soft tissues, muscles, tendons and tendon sheaths;
  • joint injuries with sprains of ligaments and tendons;
  • acute neuralgia;
  • periarthritis of the shoulder joint;
  • bursitis, tendinitis, tendovaginitis, shoulder epicondylitis.

Contraindications

Absolute:

  • open wounds at the site of application;
  • severe disorders of the cardiovascular system;
  • severe renal and / or hepatic impairment;
  • bronchial asthma;
  • children under 5 years old;
  • the period of pregnancy and breastfeeding;
  • hypersensitivity to any component of the Heparidex gel.

Heparidex, instructions for use: method and dosage

Gel Heparidex must be applied externally: apply a thin layer on the skin over the affected area and evenly distribute with light rubbing movements.

The frequency of procedures is 2-4 times a day. The duration of treatment is 7-10 days or until the swelling, swelling, inflammation and pain disappear.

Heparidex can be used for iontophoresis. Since treatment with electric shock can cause skin irritation, you should carefully follow the recommendations specified in the instructions for the device so that the drug does not increase local reactions.

Heparidex is allowed to be used as a contact gel for phonophoresis - it complements the therapeutic effect of ultrasonic waves.

Side effects

Heparidex is generally well tolerated. In some cases, drug therapy can cause the following undesirable effects:

  • local skin reactions (at the site of application of the gel): itching, redness, burning sensation;
  • allergic reactions, including urticaria, angioedema;
  • others: change in taste (disappears a few minutes after applying the gel), garlic odor from the mouth, headache, chills, diarrhea, nausea, difficulty breathing.

Overdose

Overdose cases are not known to date.

Before using the gel, you should thoroughly cleanse the skin area at the site of application of Heparidex from dirt and other drugs. If necessary, you can apply bandages to the affected area, but not earlier than after a few minutes (after most of the gel penetrates the skin and the alcohol contained in it evaporates).

Heparidex should not be applied to damaged skin and open wounds (for example, due to severe sunburn or radiation), to postoperative scars, to the mucous membranes of the mouth, nose and eyes.

The drug can increase the sensitivity of the skin, therefore during the treatment period it is recommended to limit exposure to intense sunlight and a visit to the solarium. If any skin reactions occur, therapy should be discontinued.

Unless otherwise prescribed by the doctor, the dosages and duration of treatment recommended in the instructions should not be exceeded. If there is no noticeable improvement or worsening of the condition, after 10 days of treatment, you must seek medical help and clarify the diagnosis.

Dimethyl sulfoxide has a high absorption, therefore, Heparidex cannot be used in combination with other external gels and ointments.

Influence on the ability to drive vehicles and complex mechanisms

The components of Heparidex do not have the ability to adversely affect the speed of reactions and the ability to concentrate.

Application during pregnancy and lactation

Due to the lack of experience with the use of dimethyl sulfoxide during pregnancy, Heparidex is contraindicated to appoint during this period.

Dimethyl sulfoxide is able to penetrate into breast milk, and therefore you should not use the drug during lactation, or you need to stop breastfeeding.

Pediatric use

Heparidex is not used to treat children under 5 years of age.

With impaired renal function

Severe renal failure is a contraindication to the appointment of Heparidex.

Drug interactions

Heparidex enhances the skin permeability of other drugs used simultaneously for external use.
