Valdoxan (agomelatine) is an antidepressant used to effectively treat major depressive disorder in adults.
pharmachologic effect
Valdoxan (an analogue of agomelatine) counteracts the work of melatonin MT 1 and MT 2 receptors and serotonin 5-HT 2c receptors.
As a result of scientific research, the activity of Valdoxan has been proven in the treatment of:
- Eligible models of depression, such as learned helplessness, despair, moderate chronic stress;
- Modeled depression caused by a violation of cyclical fluctuations in the intensity of biological processes associated with the change of day and night (circadian rhythms);
- In situations of experimentally provoked anxiety and stress.
It has been proven that Valdoxan has no effect on the capture of monoamines and does not affect the work of α-, β-adrenergic receptors, histamine receptors, cholinergic receptors, dopamine and benzodiazepine receptors.
Valdoxan enhances the release of norepinephrine and dopamine, especially in the prefrontal cortex, and does not affect the concentration of serotonin outside the cells.
Research results have shown that by stimulating melatonin receptors, Valdoxan promotes the release of melatonin. It restores circadian rhythms, normalizes sleep patterns and lowers body temperature by slowing down physiological processes.
Even a short-term intake of Valdoxan (6-8 weeks) 25-50 mg per day is effective for major depressive episodes, more severe forms of depression, with an initially high level of anxiety and with a combination of depressive and anxiety disorders. Indications for the use of Valdoxan

Contraindications
Valdoxan, according to the instructions, is contraindicated:
- Children under 18;
- In severe liver failure, various liver diseases in the active phase;
- With lactose intolerance - galactosemia, lactase deficiency, glucose / galactose malabsorption syndrome;
- In case of hypersensitivity to agomelatine and other excipients;
- Concomitant use with strong inhibitors of the CYP1A2 isoenzyme - ciprofloxacin, fluvoxamine;
- Women during breastfeeding.
Valdoxan, according to the instructions, should be prescribed with caution and mandatory medical supervision in the following cases:
- Elderly patients over 65;
- Patients with severe or moderate renal impairment with major depressive episodes;
- When taken simultaneously with moderate inhibitors of the isoenzyme CYP1A2 - grepafloxacin, propranolol, enoxacin;
- Patients with hypomanic or manic attacks;

- During pregnancy;

Method of using Valdoxan
Valdoxan, according to the instructions, is taken orally, swallowing the tablet whole (without chewing). Taking pills does not depend on food intake.
The recommended daily dose of Valdoxan is one tablet (25 mg) before bedtime. If no improvement is observed within two weeks, the dose can be increased to two tablets (50 mg) at night.
It is recommended to monitor liver function in the first days of admission and periodically during the entire course of treatment - after 3, 6, 12 and 24 weeks. The course of treatment for depression with Valdoxan is at least 6 months or until complete recovery.
There is no need to gradually reduce the dose, since the drug does not cause addiction.
Drug interactions Valdoxan
Strong inhibitors of the isoenzyme CYP1A2 significantly slow down the metabolism of agomelatine, increasing its concentration in the body by an order of magnitude. Therefore, the use of drugs ciprofloxacin, fluvoxamine and their analogues with Valdoxan is prohibited at the same time.
Moderate inhibitors of the isoenzyme CYP1A2 - estrogens, increase the concentration of agomelatine several times. Prescribe propranol, enoxacin, grepafloxacin and their analogues with Valdoxan with caution.
You should not combine the reception of Valdoxan with ethanol.
Based on research and reviews, Valdoxan:
- Does not affect the concentration in the body of drugs, the metabolism of which is associated with cytochrome P450 isoenzymes;
- Does not affect the free concentration of drugs that bind to plasma proteins, and they do not affect the concentration of agomelatine;
- There is no pharmacodynamic and pharmacokinetic interaction of Valdoxan with drugs used in psychiatry - benzodiazepines, lithium preparations, paroxetine, fluconazole and theopheline.
There are no data on the use of electroconvulsive therapy simultaneously with Valdoxan in patients. Experiments on animals show no undesirable consequences of such joint treatment.
Side effects of Valdoxan
