Buscopan is an M-anticholinergic antispasmodic agent.

Release form and composition

  • sugar-coated tablets: round, biconvex, white, almost odorless (10 and 20 pcs. in blisters, 1 or 2 packs in a cardboard box);
  • rectal suppositories: oblong, with a smooth surface, white or ivory (5 pcs. in strips of aluminum foil, 2 strips in a cardboard box).

Composition of 1 tablet:

  • active substance: hyoscine butyl bromide - 10 mg;
  • auxiliary components: colloidal silicon dioxide, dried corn starch, anhydrous calcium hydrogen phosphate (calcium hydrogen phosphate), tartaric acid, soluble corn starch, stearic acid;
  • shell: gum arabic (acacia gum), sucrose, carnauba wax, white wax (white beeswax), talc, macrogol 6000 (polyethylene glycol), povidone (polyvinylpyrrolidone), titanium dioxide.

Composition of 1 suppository:

  • active substance: hyoscine butyl bromide - 10 mg;

Pharmacological properties

Pharmacodynamics

Hyoscine butyl bromide is an M-anticholinergic blocker that has a local spasmolytic effect on the smooth muscles of the internal organs: the gastrointestinal tract, urinary and biliary tract. It also helps to reduce the secretion of the digestive glands.

The local spasmolytic effect of the drug is explained by its antimuscarinic and ganglion-blocking activity. Hyoscine butyl bromide is a quaternary ammonium derivative, does not penetrate the blood-brain barrier, therefore it does not have an anticholinergic effect on the central nervous system.

Pharmacokinetics

After oral administration, hyoscine butyl bromide is slightly absorbed in the gastrointestinal tract - by about 8%. With rectal administration of the drug, absorption is about 3%.

Absolute bioavailability averages no more than 1%.

After a single oral administration of Buscopan in doses from 20 to 400 mg, the average maximum plasma concentrations are reached within about 2 hours and are 0.11-2.04 ng / ml.

Due to the high affinity for nicotinic and muscarinic receptors of hyoscine, butyl bromide is distributed mainly in the muscle cells of the pelvic organs and the abdominal cavity, as well as in the intramural ganglia of the abdominal organs.

It is characterized by a low bond with plasma proteins (albumin) - about 4.4%. It was found that in vitro at a concentration of 1 mmol, the drug interacts with the transport of choline (1.4 nmol) in the epithelial cells of the human placenta.

It is metabolized mainly by hydrolysis of the ether bond. When taken orally, excretion occurs with urine and feces, excretion through the intestine is 90%, renal elimination is 2–5%. Excretion by the kidneys of metabolites of the active substance Buscopan is <0.1% of the dose taken. In cases of taking the drug in the dose range from 100 to 400 mg, the average clearance values are 881-1420 l / min, the corresponding volumes of distribution are from 6.13 to 11.3 x 10 5 l, which can be explained by low systemic bioavailability.

After rectal administration of Buscopan, renal elimination is 0.7-1.6% and occurs mainly through the intestines. Excretion by the kidneys of metabolites of the active substance Buscopan is <0.1% of the dose received.

The metabolites that are excreted in the urine weakly bind to muscarinic receptors, therefore they are not active and do not have pharmacological properties.

The final half-life of the drug after a single oral dose in the range from 100 to 400 mg is 6.2-10.6 hours.

Indications for use

  • exacerbation of gastric ulcer and duodenal ulcer (as part of complex therapy);
  • intestinal, renal and biliary colic;
  • cholecystitis;
  • spastic dyskinesia of the gallbladder and biliary tract;
  • pylorospasm;
  • algodismenorrhea.

Contraindications

  • megacolon;
  • myasthenia gravis;
  • children under 6 years old;
  • period of pregnancy and lactation;
  • hypersensitivity to any component of the drug.

Buscopan tablets contain sucrose (41.2 mg per 1 pc.), Therefore in this dosage form it is contraindicated in malabsorption of glucose-galactose and sucrase-isomaltase deficiency, fructose intolerance.

According to the instructions, Buscopan should be used with caution in the following cases:

  • obstruction of the urinary tract (including benign prostatic hyperplasia);
  • suspicion of intestinal obstruction (including pyloric stenosis);
  • angle-closure glaucoma;
  • tachyarrhythmia, including atrial fibrillation.

Instructions for the use of Buscopan: method and dosage

Buscopan tablets should be taken orally with water. Suppositories are used rectally, injected with a pointed end into the rectum (after being removed from the package).

Unless otherwise prescribed by a doctor, the following dosage regimen is usually recommended for adults and children from 6 years of age: 1-2 tablets or 1-2 suppositories 3-5 times a day.

Without consulting a doctor, you can use Buscopan for no more than 3 days in a row.

Side effects

  • from the digestive system: dry mouth;
  • on the part of the cardiovascular system: tachycardia;
  • from the urinary system: urinary retention;
  • from the skin and subcutaneous tissues: dyshidrotic eczema;
  • from the immune system: skin reactions (for example, rash, itching, urticaria, erythema), shortness of breath, anaphylactic reactions, anaphylactic shock and other manifestations of hypersensitivity.

Many of the described side effects can be due to the anticholinergic properties of Buscopan, they are usually mild and go away on their own.

Overdose

Overdose cases have not been reported to date. It is assumed that the development of anticholinergic symptoms, such as dry mouth, urinary retention, transient visual impairment, skin redness, inhibition of gastrointestinal motility, tachycardia, is possible.

The action of the M-anticholinergic antagonist is stopped with the help of cholinomimetics. Glaucoma patients are prescribed pilocarpine in the form of eye drops. If necessary, use systemic cholinomimetics, for example, intravenous or intramuscular neostigmine at a dose of 0.5–2.5 mg. Cardiovascular complications are treated according to the usual therapeutic guidelines. With urinary retention, bladder catheterization is indicated. In the case of paralysis of the respiratory muscles, intubation and artificial ventilation are required. Supportive therapy is required.

Hyoscine butyl bromide can increase tachycardia caused by beta-adrenergic drugs.

With the simultaneous use of dopamine antagonists (for example, metoclopramide), there is a mutual weakening of the effect on the gastrointestinal tract.

Buscopan can enhance the anticholinergic effect of anticholinergic drugs (for example, atropine-like compounds, ipratropium bromide, tiotropium bromide), antipsychotic and antihistamines, tetracyclic and tricyclic antidepressants, amantadine, disopyramide and quinidine.
