Maxikold is a drug for the symptomatic treatment of acute respiratory diseases.

Release form and composition

  • Film-coated tablets: oval, convex on both sides, pinkish-orange in color, with a line; on a transverse cut of pinkish-orange color with orange and white blotches (2, 10 or 12 pcs. in blister strip packs, in a cardboard box 1 or 2 packs);
  • Powder for preparation of oral solution: crystalline, light yellow or pink (possibly the presence of white crystals and easily disintegrating lumps), with a specific odor; ready-made solution - opalescent, light yellow in color with a characteristic smell of orange or lemon or pink with a characteristic smell of raspberries (5 g each in sachets made of a combined material, in a cardboard box 5, 10 or 50 sachets).

Active ingredients in 1 tablet:

  • Paracetamol - 500 mg;
  • Ascorbic acid - 30 mg;
  • Phenylephrine hydrochloride - 10 mg

Active ingredients in 1 powder sachet:

  • Paracetamol - 750 mg;
  • Ascorbic acid - 60 mg;
  • Phenylephrine hydrochloride - 10 mg

Additional inactive components of the tablets:

  • Core composition: magnesium silicate, hyprolose (hydroxypropyl cellulose), talc, calcium hydrogen phosphate, croscarmellose sodium, ethyl cellulose, sunset yellow dye;
  • Sheath composition: titanium dioxide, talc, hyprolose, hypromellose, sunset yellow dye.

Powder excipients: potato starch, citric acid, lactose, sodium saccharin, colloidal silicon dioxide, sodium citrate, flavoring, quinoline yellow dye (powder with orange and lemon flavors) or acid red dye (powder with raspberry flavor).

Indications for use

Maxicold is a drug for the symptomatic treatment of infectious and inflammatory diseases (including influenza and acute respiratory viral infections), accompanied by chills, fever, nasal congestion, pain in the sinuses, throat, muscles and bones, and headache.

Contraindications

Absolute:

  • Hyperthyroidism (including thyrotoxicosis);
  • Arterial hypertension;
  • Heart disease (tachyarrhythmias, acute myocardial infarction, severe aortic stenosis);
  • Closed-angle glaucoma;
  • Hyperplasia of the prostate;
  • Severe renal / hepatic impairment;
  • Chronic alcoholism;
  • Children under 9 years of age (or body weight less than 30 kg) - for tablets, up to 12 years - for powder;
  • Concomitant use of other drugs containing paracetamol;
  • Simultaneous administration of monoamine oxidase inhibitors (as well as 14 days after their cancellation), beta-blockers, tricyclic antidepressants;
  • Hypersensitivity to drug components.

Relative:

  • Benign hyperbilirubinemia;
  • Deficiency of glucose-6-phosphate dehydrogenase;
  • Pregnancy;
  • Lactation;
  • Elderly age.

Method of administration and dosage

Maxicold film-coated tablets should be taken orally, swallowing the tablets whole and drinking water, before meals or 1-2 hours after meals.

Recommended doses:

  • Adults and adolescents over 12 years old (weighing more than 40 kg): 1-2 tablets every 4-6 hours, but not more than 4 times a day;
  • Children 9-12 years old (weighing 30-40 kg): 1 tablet every 4-6 hours, but no more than 4 times a day.

Powder for preparation of solution for oral administration

The drug is taken orally. To do this, pour the contents of 1 packet into a cup, pour hot water and stir until the powder is completely dissolved. Take hot.

Recommended doses:

  • Adults: 1 packet every 4-6 hours, but no more than 4 times a day;
  • Teenagers over 12 years old: 1 packet every 6 hours, but no more than 3 times a day.

Regardless of the prescribed dosage form of Maxikold, the duration of treatment should not exceed 3 days if the drug is used as an antipyretic agent, and 5 days as an anesthetic. The need for longer treatment is determined individually by the doctor.

Side effects

  • Allergic reactions: rash, urticaria, skin flushing, angioedema;
  • Side effects caused by paracetamol: hematopoiesis disorders (methemoglobinemia, thrombocytopenia, anemia);
  • Side effects due to phenylephrine: shortness of breath, irritability, bradycardia, palpitations, angina pectoris, tachycardia, motor restlessness, increased or decreased blood pressure, ventricular arrhythmia (especially when taking high doses), headache, nausea or vomiting, allergic reactions;
  • Side effects due to ascorbic acid: irritation of the mucous membrane of the gastrointestinal tract, thrombocytosis, erythropenia, hypokalemia, neutrophilic leukocytosis, glucosuria, inhibition of the function of the insular apparatus of the pancreas; with prolonged use of high doses - diarrhea, ulceration of the mucous membrane of the gastrointestinal tract, nausea, vomiting, hyperacid gastritis, decreased capillary permeability (increased blood pressure, deterioration of tissue trophism, microangiopathy, hypercoagulation).

With prolonged use of Maxikold at doses significantly higher than recommended, the risk of increased excitability of the central nervous system, the appearance of headache and insomnia, the development of renal dysfunction (manifested by hyperoxaluria, moderate pollakiuria, damage to the glomerular apparatus of the kidneys, nephrolithiasis) increases.

special instructions

Before taking Maxicold, patients who are taking cholestyramine, domperidone or metoclopramide (since the rate of absorption of paracetamol may change), or anticoagulants (because their effect increases) should be consulted with a doctor.

It should be borne in mind that during treatment with the drug, the results of laboratory tests to determine the concentration of uric acid and glucose in blood plasma are distorted.

With long-term treatment (more than 5-7 days), it is necessary to monitor liver function and the state of peripheral blood.

While taking Maxicold, you should refrain from drinking alcohol, because the risk of toxic liver damage increases.

The drug does not have a negative effect on the speed of reactions and the ability to concentrate.

Drug interactions

Paracetamol:

  • In high doses, it enhances the effect of anticoagulants;
  • Reduces the effectiveness of uricosuric and diuretic drugs.

Ascorbic acid (AA):

  • Reduces the effectiveness of indirect anticoagulants, heparin and antipsychotic drugs (neuroleptics), which are phenothiazine derivatives;
  • Increases the total ethanol clearance, which decreases the concentration of AA;
  • Reduces tubular reabsorption of tricyclic antidepressants and amphetamine;
  • Improves the absorption of iron preparations in the intestine;
  • Reduces the concentration of oral contraceptives in the blood;
  • Slows down the excretion of acids by the kidneys;
  • Reduces the chronotropic effect of isoprenaline;
  • Enhances the excretion of drugs with an alkaline reaction (including alkaloids);
  • Increases the risk of developing crystalluria when treated with short-acting sulfonamides or salicylates;
  • Increases the concentration of tetracyclines and benzylpenicillin in the blood;
  • In high doses, it increases the excretion of mexiletine by the kidneys;
  • With prolonged use or taken in high doses, it can interfere with the interaction of disulfiram with ethanol.

Phenylephrine:

  • Reduces the antianginal effect of nitrates;
  • Reduces the hypotensive effect of diuretics and antihypertensive drugs (including mecamylamine, methyldopa, guanethidine, guanadrel).

Possible interaction reactions in the case of simultaneous use of Maxikold with other drugs:

  • Monoamine oxidase inhibitors, sedatives, ethanol: their effects are enhanced;
  • Ethanol, hepatotoxic drugs, inducers of enzymes of microsomal oxidation in the liver (for example, barbiturates, phenylbutazone, rifampicin, tricyclic antidepressants, phenytoin): the risk of developing the hepatotoxic effect of paracetamol increases;
  • Long-term barbiturates: the effectiveness of paracetamol decreases;
  • Metoclopramide, domperidone: the rate of absorption of paracetamol increases;
  • Cholestyramine: the rate of absorption of paracetamol decreases;
  • Inhibitors of microsomal oxidation enzymes (for example, cimetidine): the risk of hepatotoxic action of paracetamol decreases;
  • Ethanol: the likelihood of developing acute pancreatitis increases;
  • Salicylates (when taking high doses of paracetamol): increases the risk of developing cancer of the bladder or kidney;
  • Diflunisal: the plasma concentration of paracetamol and the risk of developing hepatotoxicity increase;
  • Myelotoxic drugs: manifestations of paracetamol hematotoxicity increase;
  • Monoamine oxidase inhibitors (including selegiline, procarbazine, furazolidone), tricyclic antidepressants, ergot alkaloids, adrenostimulants, methylphenidate, oxytocin: the vasoconstrictor effect and arrhythmogenicity of phenylephrine increase;
  • Reserpine: the risk of hypertension increases;
  • Thyroid hormones: increases the effect of phenylephrine and the associated risk of coronary insufficiency (especially in patients with coronary atherosclerosis);
  • Non-steroidal anti-inflammatory drugs: the risk of developing "analgesic" nephropathy, renal papillary necrosis and the onset of end-stage renal failure increases;
  • Inhalation anesthetics (including halothane, enflurane, methoxyflurane, isoflurane, chloroform): the likelihood of severe atrial and ventricular arrhythmias increases;
  • Acetylsalicylic acid (ASA): excretion of AA in urine increases, excretion of ASA decreases;
  • Oral contraceptives, ASA, alkaline drinks, fresh juices: absorption and assimilation of ascorbic acid decreases;
  • Primidone, barbiturates: increased excretion of ascorbic acid in the urine;
  • Glucocorticoids, salicylates, calcium chloride, quinoline series drugs: with prolonged use, the reserves of ascorbic acid are depleted;
  • Antipsychotic and antiparkinsonian drugs, antidepressants, phenothiazine derivatives: the risk of dry mouth, urinary retention, constipation increases;
  • Glucocorticosteroids: the risk of developing glaucoma increases;
  • Halothane: increased likelihood of ventricular arrhythmias;
  • Guanethidine: the alpha-adrenostimulating effect of phenylephrine is enhanced;
  • Tricyclic antidepressants: the sympathomimetic effect of phenylephrine is enhanced.
