Comfoderm is an anti-inflammatory and keratolytic drug for external use; glucocorticosteroid (GCS).
Release form and composition
Comfoderm is produced in the form of an ointment for external use: translucent, homogeneous, from white with a gray or yellow tint to white, with the possible presence of a specific odor (15 or 30 g in an aluminum tube, in a cardboard box 1 tube).
Composition of 1 g of ointment:
- active substance: methylprednisolone aceponate (in terms of 100% substance) - 1 mg;
- additional components: liquid paraffin, petroleum jelly, white beeswax, castor bean seed oil.
Pharmacological properties
Pharmacodynamics
Methylprednisolone aceponate, the active ingredient in Comfoderm, is a non-halogenated steroid. When used externally, the agent prevents the development of inflammatory and allergic skin reactions, as well as the effects caused by proliferation, which helps to reduce the intensity of pain, irritation, itching against the background of a decrease in the severity of objective symptoms of inflammation (erythema, rashes, edema, oozing, etc.).
When the substance is used externally in the recommended doses, the manifestation of systemic action in humans and animals is minimal. After repeated application of the agent on 40-60% of the body surface area or application under an occlusive dressing, functional disorders of the adrenal glands were not found: there was no decrease in cortisol concentration in daily urine, the circadian rhythm of this hormone and its plasma level remained within normal limits.
Methylprednisolone aceponate (mainly its main metabolite - 6α-methylprednisolone-17-propionate) combines with intracellular GCS receptors. The resulting steroid-receptor complex, by binding to certain immunostimulatory DNA regions, leads to a series of biological effects, including the induction of macrocortin production. In turn, macrocortin, by inhibiting the release of arachidonic acid, blocks the production of inflammatory mediators such as prostaglandins and leukotrienes.
Suppression of corticosteroids synthesis of vasodilating prostaglandins and an increase in the vasoconstrictor effect of adrenaline cause a vasoconstrictor effect.
Pharmacokinetics
The hydrolysis of methylprednisolone aceponate takes place in the dermis and epidermis. The most active and main metabolite of the drug is 6α-methylprednisolone-17-propionate, which demonstrates a significantly higher affinity for the skin GCS receptors, which confirms the presence of its bioactivation in the skin.
The degree of percutaneous absorption of this active substance depends on the method of application (with or without occlusive dressing), dosage form and skin condition. When it enters the systemic circulation, 6α-methylprednisolone-17-propionate is rapidly conjugated with glucuronic acid, and then the resulting 6α-methylprednisolone-17-propionate glucuronide is inactivated.
Methylprednisolone aceponate metabolites are eliminated mainly by the kidneys, the half-life is approximately 16 hours. In the body, methylprednisolone aceponate and its metabolites do not accumulate.
Indications for use
According to the instructions, Comfoderm is prescribed for the treatment of inflammatory skin diseases that are sensitive to therapy with topical GCS:
- microbial eczema;
- true eczema;
- atopic dermatitis, neurodermatitis, childhood eczema;
- dyshidrotic eczema;
- professional eczema;
- allergic (contact) dermatitis;
- simple contact dermatitis.
Contraindications
- viral lesions at the site of application of the drug (including chickenpox, shingles);
- syphilitic / tuberculous process in the area treated with ointment;
- perioral dermatitis, rosacea at the site of ointment application;
- areas of the skin with manifestations of a reaction to vaccination;
- children's age up to 4 months;
- hypersensitivity to any of the constituents of the drug.
Instructions for use Comfoderm: method and dosage
Comfoderm ointment is used externally.
Children over 4 months old and adults are recommended to apply a thin layer of ointment to the affected skin areas once a day. The course of continuous daily drug therapy, as a rule, should not be more than 12 weeks in adults and 4 weeks in children.
With very dry skin, long-term chronic inflammatory skin processes should be treated with an anhydrous dosage form of methylprednisolone aceponate.
The occlusive effect of Comfoderm provides a pronounced therapeutic effect even against the background of significant lichenification and infiltration.
Side effects
- skin and subcutaneous tissues: rarely - skin depigmentation, perioral dermatitis, allergic reactions to any component of the drug; with an unknown frequency - striae, telangiectasias, skin atrophy, acne-like skin changes (with a course longer than 4 weeks and / or applying an ointment on an area of 10% or more of the body surface area);
- general disorders and local reactions: rarely - hypertrichosis, folliculitis; extremely rare - erythema, burning, itching, vesicular rash; with an unknown frequency - systemic effects caused by the absorption of GCS (with a course of more than 4 weeks and / or when applied on an area of 10% or more of the body surface area).
In case of aggravation of the severity of the above undesirable effects or the appearance of any other disorders, you should immediately consult a doctor.
Overdose
In case of unintentional oral intake of methylprednisolone aceponate or its excessive single external application (over a large area in the presence of conditions favorable for absorption), no risk of acute intoxication was identified during the study of acute toxicity of the drug.
In the case of a very long and / or intensive local use of GCS, there is a possibility of developing skin atrophy - striae, telangiectasias, thinning of the skin. If these symptoms appear, the drug should be canceled.

In the presence of dermatomycosis and / or bacterial complications, it is recommended in addition to the use of Comfoderm to carry out specific antibacterial and / or antimycotic therapy.
After external use of corticosteroids, as well as with their systemic use, there is a risk of developing glaucoma, for example, with the application of large doses, prolonged use of occlusive dressings or use in the areas around the eyes.

No negative effect of Comfoderm on the ability to control complex mechanisms and road transport was found.
Application during pregnancy and lactation
During pregnancy and lactation, if it is necessary to use Comfoderm, it is necessary to carefully balance the possible risk to the health of the fetus / child and the expected benefit of therapy for the woman. During these periods, you should not apply the ointment for a long time on extensive surfaces of the skin.
Breastfeeding mothers are advised not to apply Comfoderm to their breasts.
Pediatric use
The use of the drug is contraindicated in children under 4 months of age. Continuous daily use of the ointment in children should not exceed 4 weeks.
