Enteric film-coated tablets, Penzital

Penzital is an enzyme remedy that compensates for the insufficiency of the exocrine function of the pancreas.

Release form and composition

Dosage form - enteric film coated tablets: round, convex on both sides, white (10 pcs. In strips, in a cardboard box from 1 to 12 strips).

The active ingredient of the drug is pancreatin, its content in 1 tablet is 212.5 mg, which corresponds to the enzymatic activity:

  • Lipase - 6000 U FIP;
  • Amylase - 4500 U FIP;
  • Proteases - 300 U FIP.

Additional components: sodium starch glycolate, microcrystalline cellulose, povidone, titanium dioxide, methacrylic acid copolymer, colloidal silicon dioxide, diethyl phthalate, talc, lactose.

Pharmacological properties

Pharmacodynamics and pharmacokinetics

Penzital is used to compensate for the insufficiency of the external secretory function in the pancreas. The drug has proteolytic, lipolytic and amylolytic effects on the body.

The enzymes included in pancreatin (alpha-amylase, lipase, trypsin, chymotrypsin) take part in the breakdown of fats to glycerol and fatty acids, proteins to amino acids, starch to dextrins and monosaccharides, and also contribute to the normalization of digestive processes and improve the functional state of the gastrointestinal tract. …

Trypsin inhibits the stimulated secretion of the pancreas, having an analgesic effect.

The release of pancreatic enzymes from the dosage form occurs in the alkaline medium of the small intestine, since the drug is protected from the influence of gastric juice by the membrane. The maximum enzymatic activity after oral administration is observed 30–45 minutes after the drug is administered. Pancreatin is not absorbed in the gastrointestinal tract and is excreted along with the feces.

Indications for use

Indications for Penzital:

  • Disturbances in the assimilation of food (including in conditions after resection of the stomach or small intestine);
  • Exocrine pancreatic insufficiency (diarrhea of non-infectious origin, flatulence, dyspepsia, cystic fibrosis, gastrocardial syndrome, chronic pancreatitis, conditions after irradiation and pancreatectomy) - as replacement therapy;
  • Prolonged immobilization, impaired chewing function, inaccuracies in nutrition (irregular eating, eating fatty foods, eating large amounts of food) in people with a normally functioning gastrointestinal tract - to improve food digestion;
  • Examination (ultrasound or X-ray) of the abdominal cavity - to prepare the organs of the gastrointestinal tract.

Contraindications

Absolute:

  • Exacerbation of chronic pancreatitis;
  • Acute pancreatitis;
  • Hypersensitivity to drug components.

Instructions for the use of Penzital: method and dosage

The tablets should be taken orally during meals or immediately after meals: swallow whole and drink plenty of non-alkaline liquid (water or fruit juice).

The doctor determines the dose for each patient individually, depending on the degree of digestive disorders.

Adults are usually prescribed 1-2 tablets 3 times a day.

The duration of treatment is also set on a private basis, it can vary from a few days (with inaccuracies in nutrition) to several months or even years (with constant substitution therapy).

Side effects

  • On the part of the digestive system: nausea, constipation / diarrhea, a feeling of discomfort in the stomach (however, the causal relationship of the development of these effects with the use of Penzital has not been proven, since these reactions are also symptoms of exocrine pancreatic insufficiency);
  • From the side of metabolism: with prolonged use of the drug in high doses - hyperuricosuria, hyperuricemia;
  • Others: allergic reactions.

In patients with cystic fibrosis, if the recommended doses are exceeded, fibrous colonopathy (strictures) may develop in the ileocecal part of the ascending colon.

In children, when taking high doses of pancreatin, irritation of the oral mucosa and the development of perianal irritation is possible.

Overdose

In case of an overdose, constipation (mainly in children), hyperuricemia, hyperuricosuria may occur. In difficult cases of overdose, the drug is canceled, the stomach and intestines are washed, and symptomatic treatment is prescribed.

special instructions

The prescribed dose of the drug should correspond to the amount of enzymes required to ensure fat absorption. In this case, the quality and quantity of food consumed must be taken into account.

When carrying out long-term therapy, it is recommended to take iron supplements at the same time.

Patients with cystic fibrosis are at risk of developing fibrous colonopathy, therefore they are not recommended to use Penzital in high doses.

Application during pregnancy and lactation

There are no reliable data indicating the safety of using pancreatin during pregnancy. Experimental studies have shown that pancreatin has no teratogenic effect.

Treatment of pregnant women with Penzital according to indications is possible only in cases where the benefits to the mother outweigh the possible harm to the child.

Pediatric use

When treating children, the drug is used according to the dosage regimen. The high activity of lipase contained in pancreatin increases the risk of constipation in children.

Drug interactions

Antacids containing magnesium hydroxide and / or calcium carbonate reduce the effectiveness of pancreatin.

According to the instructions, Penzital can reduce the absorption of iron preparations.
