Aspirin Express is a non-steroidal anti-inflammatory drug (NSAID) with analgesic, antipyretic and anti-inflammatory effects.
Release form and composition
Dosage form - effervescent tablets: white, flat, round, slightly beveled to the edge, branded engraving in the form of a Bayer cross on one side (in paper strips laminated with aluminum foil, 2 pcs., In cardboard boxes along with instructions for use Aspirin Express 6 or 12 strips).

Acetylsalicylic acid is an NSAID with analgesic, antipyretic and anti-inflammatory properties. The mechanism of action lies in the irreversible inhibition of the enzyme cyclooxygenase (COX), which regulates the synthesis of thromboxane A2, as well as prostaglandins E12 and E2.
Pharmacokinetics
Acetylsalicylic acid is highly absorbed; after oral administration, it is rapidly absorbed from the gastrointestinal tract (GIT) in full. The maximum concentration (C max) of the substance in the blood plasma when ingested Aspirin Express effervescent tablets is achieved after 18 minutes, which is 2 times faster than standard tablets, when taken, C max is fixed after 30–45 minutes. During and immediately after absorption, acetylsalicylic acid is converted into salicylic acid, the main metabolite with pharmacological activity. The C max of this metabolite in blood plasma for a dosage form in the form of effervescent tablets is reached after 48 minutes, while the same indicator for standard tablets is 1.5–2 hours.
The connection with plasma proteins of acetylsalicylic acid and its metabolite is approximately 80%, distribution in tissues is rapid. Salicylic acid is metabolized in the liver, resulting in the formation of the following metabolites: salicilurate, salicylophenolic glucuronide, salicylacyl glucuronide, gentisuric and gentisic acids.
Penetrates through the placenta, is found in breast milk.
The elimination of salicylic acid from the body depends on the doses of Aspirin Express taken. The period of excretion varies from 2–3 hours with low doses to about 15 hours with high doses of the drug. Salicylic acid and its metabolites are excreted mainly in the urine.
Indications for use
Aspirin Express is used as an antipyretic agent for colds and other infectious and inflammatory diseases, as well as an analgesic agent for relieving sore throat, toothache and headache, back pain, joints, muscles, menstrual pain, mild arthritis pain, etc. P.
Contraindications
Absolute:
- severe heart failure in the stage of decompensation;
- severe renal / hepatic impairment;
- hemorrhagic diathesis;

- I and III trimesters of pregnancy;
- lactation period;

In addition, the drug should not be prescribed to children under 15 years of age with acute respiratory diseases caused by viral infections, as this increases their risk of developing encephalopathy and acute fatty liver disease with acute development of liver failure (Reye's syndrome).

Aspirin Express, instructions for use: method and dosage
Aspirin Express effervescent tablets are taken orally after meals, after dissolving them in a glass of water.
For febrile conditions and pain syndrome of mild / moderate intensity, a single dose for adults and children over 15 years old - 2 effervescent tablets (1000 mg of acetylsalicylic acid).
The maximum single dose that should not be exceeded is 2 effervescent tablets (1000 mg of acetylsalicylic acid). A maximum of 6 effervescent tablets (3000 mg of acetylsalicylic acid) can be taken per day. At least 4-hour intervals must be observed between doses.
It is allowed to carry out therapy independently (without consulting a doctor) for no more than 7 days when taking Aspirin Express as an anesthetic and for no more than 3 days as an antipyretic agent.
Side effects
Possible adverse reactions when taking acetylsalicylic acid:
- from the digestive system: heartburn, nausea, vomiting, abdominal pain, stomach and duodenal ulcers (including perforated), bleeding from the gastrointestinal tract (including latent, which can lead to the development of iron deficiency anemia), increased activity of hepatic transaminases;
- on the part of the hematopoietic system: lengthening of the bleeding time with the development of bleeding of various localization (bleeding from the gums, epistaxis, hemorrhagic purpura, etc.);
- from the central nervous system and sensory organs: hearing impairment, tinnitus, dizziness, headache (as a rule, they are signs of an overdose);
- allergic reactions: urticaria and other skin manifestations of hypersensitivity, Quincke's edema, bronchial asthma, anaphylactic reactions.
Overdose
Overdose of Aspirin Express is especially dangerous for elderly patients and young children, in whom excessive intake of acetylsalicylic acid can be fatal.
Overdose symptoms:
- moderate degree: lethargy, dizziness, a feeling of hearing loss, tinnitus, headache (symptoms can be controlled by reducing the dose of the drug);
- severe: ketosis, hyperventilation, fever, metabolic acidosis, respiratory alkalosis, coma, respiratory failure, cardiovascular shock, severe hypoglycemia.
As measures of emergency care, it is necessary to urgently hospitalize the victim in a specialized department for emergency therapy: perform gastric lavage, give the patient activated charcoal, determine the acid-base balance, carry out alkaline diuresis until urine pH values are within 7.5-8 alkaline diuresis with a concentration of salicylates in the blood of more than 300 mg / l (2.2 mmol / l) in children and 500 mg / l (3.6 mmol / l) in adults, compensate for fluid loss, if necessary, prescribe symptomatic treatment.
When diagnosing severe poisoning, hemodialysis is possible.

In pediatrics, drugs containing acetylsalicylic acid are not prescribed, since when treating a viral infection in children, they increase their risk of developing Reye's syndrome, manifested by prolonged vomiting, acute encephalopathy, and enlarged liver.
Acetylsalicylic acid can cause bronchospasm and lead to an attack of bronchial asthma or other allergic reactions. Because Aspirin Express is able to inhibit platelet aggregation, it can cause increased bleeding during and after surgical procedures, including tooth extraction and other minor surgical procedures. When breastfeeding a child, the use of drugs containing acetylsalicylic acid is contraindicated, since salicylates and their metabolites penetrate into breast milk in small quantities.
Pediatric use

Drug interactions
- methotrexate: a combination with methotrexate at a dose of more than 15 mg per week is contraindicated, when using a dose of less than 15 mg per week, it is used with caution. This is due to an increase in the hematological toxicity of methotrexate when used in combination with acetylsalicylic acid, since NSAIDs generally decrease the renal clearance of methotrexate, and salicylates, in particular, displace it from its connection with plasma proteins;

- other NSAIDs with salicylates, taken in high doses (over 3000 mg per day): due to synergism, the risk of ulceration of the gastrointestinal mucosa and bleeding increases;
- probenecid, benzbromarone and other uricosuric drugs: due to the competitive tubular excretion of uric acid, their therapeutic effect is reduced;
- digoxin: due to a decrease in the excretion of digoxin, its concentration in plasma increases;
- sulfonylurea, insulin and other antidiabetic drugs: acetylsalicylic acid in high doses has a hypoglycemic effect and displaces sulfonylurea from its connection with plasma proteins, in connection with which the hypoglycemic effect is enhanced;
- ticlopidine and other antiplatelet drugs of other classes, thrombolytics: the risk of bleeding increases;
- diuretics: in combination with acetylsalicylic acid at a dose of 3000 mg per day, due to a decrease in the synthesis of prostaglandins in the kidneys, there is a decrease in glomerular filtration;
- systemic glucocorticosteroids (GCS), except for hydrocortisone (used in the treatment of Addison's disease): when used simultaneously with acetylsalicylic acid, the content of salicylates in the blood decreases, since GCS enhances their elimination;
- angiotensin-converting enzyme (ACE) inhibitors: with the combined use of acetylsalicylic acid at a dose of 3000 mg per day or more, and ACE inhibitors, a decrease in the hypotensive effect of the latter was noted due to a decrease in glomerular filtration;
- valproic acid: acetylsalicylic acid breaks the bond of valproic acid with plasma proteins, and therefore the toxicity of the latter increases;
- ethanol: the use of ethanol-containing agents while taking Aspirin Express lengthens the bleeding time and enhances the damaging effect on the gastrointestinal mucosa.
