Arkoksia is a drug of analgesic, antipyretic and anti-inflammatory action. Selective inhibitor of COX-2, NSAIDs (non-steroidal anti-inflammatory drug).

Release form and composition

The drug is available in the form of film-coated tablets:

  • Arcoxia 60 mg - biconvex, apple-shaped, green, on one side there is an embossing ARCOXIA 60, on the other - “200” (7 pieces in blisters, in a cardboard box 1 or 4 blisters);
  • Arcoxia 90 mg - biconvex, apple-shaped, white, on one side there is an embossing ARCOXIA 90, on the other - "202" (7 pieces in blisters, in a cardboard box 1 or 4 blisters);
  • Arcoxia 120 mg - biconvex, apple-shaped, light green, on one side there is an embossing ARCOXIA 120, on the other - "204" (7 pieces in blisters, 1 blister in a cardboard box).

Composition of 1 tablet:

  • active substance: etoricoxib - 60 mg, 90 mg or 120 mg;
  • auxiliary components: magnesium stearate, microcrystalline cellulose, calcium hydrogen phosphate, croscarmellose sodium;
  • film shell: hypromellose, triacetin, lactose monohydrate, titanium dioxide; additionally for 60 mg and 120 mg Arcoxia tablets - dye iron oxide yellow and aluminum varnish based on indigo carmine;
  • shell: carnauba wax, opadry II green 39K11520 (tablets 60 mg), opadry II white 39K18305 (tablets 90 mg) and opadry II green 39K11529 (tablets 120 mg).

Indications for use

The drug Arkoksia is used for the symptomatic treatment of diseases and conditions such as:

  • rheumatoid arthritis;
  • acute gouty arthritis;
  • ankylosing spondylitis;
  • osteoarthritis.

It is also prescribed after dental surgeries for the treatment of severe acute and moderate pain.

Contraindications

Absolute:

  • exacerbation of inflammatory bowel diseases (ulcerative colitis and Crohn's disease);
  • severe renal failure (creatinine clearance less than 30 ml / min), confirmed hyperkalemia, progressive renal disease;
  • active gastrointestinal, cerebrovascular, or other bleeding;
  • erosive and ulcerative lesions of the mucous membrane of the duodenum or stomach;
  • severe liver failure, liver disease in the active phase;
  • persistent high blood pressure with uncontrolled arterial hypertension;
  • blood clotting disorders (including hemophilia);
  • clinically pronounced coronary heart disease, cerebrovascular disease and peripheral arterial disease, the period after CABG (coronary artery bypass grafting);
  • severe heart failure;
  • incomplete or complete combination of the following diseases (including a history): recurrent polyposis of the nose or paranasal sinuses, bronchial asthma and intolerance to acetylsalicylic acid and other NSAIDs;
  • children and adolescents up to 16 years old;
  • During pregnancy and breastfeeding;
  • hypersensitivity to any component of the drug.

Relative (Arcoxia is used with caution):

  • severe somatic diseases;
  • a history of data on the development of ulcerative lesions of the gastrointestinal tract and infection with Helicobacter pylori;
  • arterial hypertension;
  • diabetes;
  • swelling and fluid retention;
  • dyslipidemia / hyperlipidemia;
  • renal failure (with creatinine clearance less than 60 ml / min);
  • smoking;
  • frequent alcohol consumption;
  • elderly age;
  • long-term use of NSAIDs;
  • concomitant therapy with antiplatelet agents, glucocorticosteroids, anticoagulants and selective serotonin reuptake inhibitors.

Method of administration and dosage

Arcoxia tablets are intended for oral administration with or without food. The drug is taken with a little water.

Recommended doses:

  • rheumatoid arthritis and ankylosing spondylitis: 90 mg once a day (the maximum daily dose is 90 mg);
  • acute gouty arthritis: 120 mg once a day (the maximum daily dose is 120 mg);
  • osteoarthritis: 60 mg once a day (the maximum daily dose is 60 mg).

In pain syndrome, the average therapeutic dose is 60 mg once.

When using Arcoxia at a dose of 120 mg, the duration of treatment should be no more than 8 days. It is recommended to use the drug in the minimum effective dose for the shortest possible course.

To eliminate acute pain after dental operations, 90 mg of etoricoxib is prescribed once a day for no more than 8 days (the maximum daily dose is 90 mg).

In liver failure (5-9 points on the Child-Pugh scale), the maximum daily dose should not exceed 60 mg.

Side effects

  • digestive system: often - nausea, dyspepsia, heartburn, epigastric pain, flatulence, diarrhea; infrequently - increased peristalsis, ulcers and dryness of the oral mucosa, gastritis, vomiting, belching, esophagitis, irritable bowel syndrome, bloating, constipation, ulcer of the mucous membrane of the duodenum or stomach; very rarely - hepatitis, ulcers of the gastrointestinal tract (sometimes with perforation or bleeding);
  • respiratory system: infrequently - nosebleeds, shortness of breath, cough; very rarely - bronchospasm;
  • cardiovascular system: often - increased blood pressure, palpitations; infrequently - atrial fibrillation, nonspecific changes in the electrocardiogram, congestive heart failure, myocardial infarction, cerebrovascular accident, hot flashes; very rarely - hypertensive crisis;
  • nervous system and sensory organs: often - weakness, headache and dizziness; infrequently - drowsiness, impaired sensitivity, including hyperesthesia / paresthesia, impaired concentration, anxiety, impaired taste, depression, sleep disturbances, tinnitus, blurred vision, vertigo, conjunctivitis; very rarely - confusion and hallucinations;
  • musculoskeletal system: infrequently - arthralgia, muscle cramps, myalgia;
  • urinary system: infrequently - detection of protein in the urine; very rarely - renal failure, usually reversible after drug withdrawal;
  • metabolism: often - fluid retention in the body, edema; infrequently - an increase in body weight, a change in appetite;
  • dermatological and allergic reactions: often - ecchymosis; infrequently - itchy skin, swelling of the face, rash; very rarely - anaphylactoid or anaphylactic reactions, Stevens-Johnson syndrome, urticaria, Lyell's syndrome;
  • laboratory indicators: often - increased activity of liver enzymes; infrequently - a decrease in hemoglobin and hematocrit, an increase in creatine phosphokinase activity, an increase in nitrogen in the urine and blood, leukopenia, an increase in uric acid, thrombocytopenia, hyperkalemia, an increase in serum creatinine; rarely - an increase in serum sodium concentration;
  • infectious complications: infrequently - infections of the urinary tract and upper respiratory tract, gastroenteritis;
  • other reactions: often - flu-like syndrome; infrequently - chest pain.

special instructions

During treatment with Arcoxia, regular blood pressure monitoring is required, especially in the first two weeks.

It is also necessary to systematically study the indicators of kidney and liver function. If the level of activity of hepatic transaminases exceeds the upper limit of the norm by three times or more, the drug should be canceled.

With prolonged use of etoricoxib, the need for further administration of the drug is periodically assessed, as well as the possibility of dose reduction.

Concomitant use with other NSAIDs is not recommended.

The shell of the tablets contains a small amount of lactose, which must be taken into account when prescribing the drug Arcoxia to patients with lactase deficiency.

During treatment, care must be taken when driving a car or other transport and engaging in other potentially hazardous activities. Patients who develop side effects such as weakness, drowsiness and dizziness should avoid high concentration activities.

Drug interactions

With the simultaneous use of Arkoxia with warfarin or similar drugs at the beginning of therapy and at each change in the dosage regimen of the drug, prothrombin time and INR (international normalized ratio) should be monitored.

When administered together with ACE inhibitors, the hypotensive effect of ACE inhibitors may be weakened. In case of impaired renal function, this combination can lead to further aggravation of functional renal failure.

With the simultaneous use of Arcoxia with cyclosporine and tacrolimus, the risk of developing nephrotoxicity of these drugs increases; with acetylsalicylic acid in low doses - an increase in the frequency of ulcerative lesions of the gastrointestinal tract is possible; with lithium preparations - an increase in the plasma concentration of lithium may be observed; with methotrexate - the toxicity of methotrexate is possible (especially when using Arcoxia at a dose of 120 mg); with oral contraceptives (especially ethinyl estradiol) - the concentration of ethinyl estradiol increases; with digoxin - an overdose of digoxin is possible; with rifampicin - the plasma concentration of etoricoxib decreases; with antacids and ketoconazole - no clinically significant interaction was found.
