Algavak M is a medical immunobiological preparation (MIBP), a vaccine for the prevention of hepatitis A.

Release form and composition

The vaccine is produced in the form of a suspension for intramuscular (i / m) administration, slightly opalescent, which, when settled, is divided into two layers: the upper is a colorless transparent liquid, the lower is a white precipitate that easily breaks up when shaken, without the presence of impurities and the formation of flakes [in ampoules of 0.5 ml (1 child dose) and 1 ml (1 adult dose), in a pack of cardboard 10 ampoules with a cardboard insert "snake", with or without an ampoule scarifier and instructions for use of Algavak M].

Composition of 1 inoculation dose of suspension:

  • active substance: inactivated antigen of hepatitis A virus - at least 160 (dose for children 0.5 ml) or at least 320 (dose for adults 1 ml) ELISA units;
  • auxiliary components: formaldehyde, aluminum hydroxide, phosphate-saline buffer solution 0.01 M (water for injection, sodium chloride, sodium hydrogen phosphate dodecahydrate).

Pharmacological properties

Algavac M is a vaccine representing hepatitis A virions (strain LBA-86), grown on a transplantable cell culture approved for production 4647, purified, concentrated, inactivated and adsorbed on an aluminum hydroxide gel.

A full course of immunization (administration of two doses of Algavac M vaccine with an interval of 6–12 months) activates additional production of antibodies in high titers, which ensures the development of long-term specific immunity to the hepatitis A virus in 90–100% of vaccinated people.

Indications for use

Algavac M is used for active prevention of viral hepatitis A in adults and children from 3 years of age. Vaccination can be given to different populations, but it is especially recommended for people who are or will be at risk of infection:

  • persons who have contact in the foci of hepatitis A;
  • persons traveling (travelers, military personnel, etc.) or living in regions with a high incidence of hepatitis A;
  • persons from special risk groups: patients with an increased risk of liver disease, chronic liver disease, hemophilia or multiple blood transfusions, people with promiscuous sex life, homosexuals, drug addicts;
  • persons at risk of occupational infection: medical workers, nursing staff (especially in pediatric, infectious and gastroenterological departments), employees of preschool institutions, staff of catering and food industries, workers in water supply and sewage services.

Contraindications

  • malignant blood diseases and neoplasms;
  • immunodeficiency states;
  • a pronounced reaction with the previous administration of the drug (an increase in body temperature above 40 ° C, hyperemia and edema at the site of inoculation more than 8 cm in diameter);
  • children under 3 years old;
  • hypersensitivity to any component of Algavac M.

Temporary contraindications to the Algavac M vaccination are acute infectious and non-infectious diseases or exacerbation of chronic diseases. The introduction of the vaccine is possible 2–4 weeks after complete recovery or remission. For mild acute intestinal diseases and acute respiratory viral infections, the drug can be administered after normalization of body temperature.

Vaccination during pregnancy and lactation is possible if the expected benefits are higher than the potential risks.

Algavak M, instructions for use: method and dosage

Algavac M is injected intramuscularly into the deltoid muscle of the shoulder.

Intravenous administration of the drug is prohibited! It is not recommended to inject the vaccine subcutaneously, intradermally or intramuscularly into the gluteal region, as this leads to the formation of a lower immune response.

One vaccination dose is 0.5 ml for children and adolescents up to 17 years old, 1 ml - for adults over 18 years old. Long-term protection against hepatitis A is provided by a course of two vaccinations, given 6–12 months apart. In exceptional cases, revaccination is allowed at a later date after the first dose.

The opening of the ampoules and the introduction of the Algavac M vaccine is carried out by a medical worker, strictly observing the rules of antiseptics and asepsis. Before entering the syringe, the drug must be thoroughly mixed by vigorous shaking of the ampoule until a homogeneous, slightly turbid suspension of white color is formed and carefully examined. Do not use the drug that has been frozen, as well as if the integrity of the ampoule or labeling is violated, the appearance of the suspension changes (change in transparency and color, the presence of flakes or any foreign inclusions).

The introduction of the drug is recorded in the established registration forms indicating the date of immunization, the dose of the vaccine, its batch number and manufacturer, as well as the reaction to vaccination.

Side effects

Algavac M is generally well tolerated (it is weakly reactogenic).

Possible adverse reactions reported in clinical and post-marketing studies:

  • very often (> 10%): soreness and flushing of the skin at the injection site;
  • often (from> 1% to <10%): fever more than 37 ° C, dizziness, headache, mild infiltration at the injection site;
  • very rare (<0.01%): muscle twitching at the vaccination site.

All emerging adverse events were short-term (no more than 24 hours) and did not require special treatment.

Overdose

Overdose cases of Algavac M are unknown.

To identify contraindications to vaccination on the day of vaccination, the doctor conducts a survey and examination of the vaccinated person with mandatory thermometry.

Algavac M can be used for revaccination of persons who received a previous immunization with another inactivated vaccine for the prevention of hepatitis A.

Vaccine administration during the incubation period of hepatitis A disease may not be effective enough.

Weak immune response to drug administration can be observed in HIV-infected people and people receiving treatment with immunosuppressants or immunostimulants.

Influence on the ability to drive vehicles and complex mechanisms

Algavac M does not adversely affect the ability to perform activities that require quick reactions and increased attention, including driving a car and working with moving machinery.

Application during pregnancy and lactation

Studies on the effect of Algavac M on the fetus / infant when administered during pregnancy / lactation have not been conducted.

The decision on vaccination is made by the attending physician, carefully comparing the ratio of the expected benefits to the woman and the potential risks to the fetus / child.

Pediatric use

Vaccination against hepatitis A is contraindicated before the age of 3 years.

Drug interactions

Suspension of Algavac M should not be mixed in the same syringe with other drugs / vaccines.

If necessary, Algavac M can be administered on the same day or with an interval of 1 month with other inactivated vaccines provided for by the National Calendar of Preventive Vaccinations and the Calendar of Preventive Vaccinations for Epidemic Indications, with the exception of rabies vaccine.
