Release form and composition
The dosage form of the drug is a nasal spray: a clear solution, light yellow or colorless (15 ml in plastic bottles with a spray for nasal use, each bottle is packed in an individual carton box together with instructions for the use of Frinosol).
Composition of 1 ml nasal spray:
- active ingredients: phenylephrine hydrochloride - 2.5 mg; cetirizine dihydrochloride - 2.5 mg;
- additional components: sodium hydrogen phosphate (anhydrous), 50% solution of benzalkonium chloride, glycerol, potassium dihydrogen phosphate, disodium edetate, purified water.
Pharmacological properties
Pharmacodynamics
Frinosol is a combined agent for local intranasal use, used for pathogenetic and symptomatic therapy.
Phenylephrine is an α1-adrenergic receptor agonist (sympathomimetic), under its influence there is a narrowing of the blood vessels of the nasal mucosa, tissue hyperemia and edema of the mucous membranes decrease, as well as congestion in the nasal mucosa. Thanks to its action, air permeability of the nasopharynx is restored, which helps to improve the patient's well-being and reduce the risk of developing possible complications that can be caused by stagnation of mucous secretions.
Cetirizine is a histamine H1 receptor blocker. It has anti-allergic properties, reduces capillary permeability, prevents the development of edema of the mucous membrane of the nasal cavity and its paranasal sinuses.
As an auxiliary component, the composition of Frinosol contains a moisturizing substance glycerol, which retains moisture and thereby provides hydration of the nasal mucosa in case of dryness and irritation.
Pharmacokinetics
The action of Frinosol does not depend on the content of its active components in the blood plasma, which is due to the local intranasal method of its application.
Indications for use
The use of Frinosol is indicated for the following diseases / conditions:
- chronic rhinitis;
- acute rhinitis (including with colds);
- vasomotor rhinitis;
- allergic rhinitis (including hay fever);
- chronic and acute sinusitis;
- acute otitis media (as an adjuvant).
In addition, a nasal spray is used in preparation for surgical interventions in the nasal area in order to eliminate edema of the nasal mucosa and paranasal sinuses after surgical interventions in this area.
Contraindications
Absolute:
- atrophic rhinitis;
- angle-closure glaucoma;
- gestation period;
- lactation (breastfeeding);
- the combined use of monoamine oxidase inhibitors, including taking in the previous 2 weeks;
- children and adolescents up to 18 years old (due to the lack of information on the safety and effectiveness of this nasal spray in pediatrics);
- hypersensitivity to phenylephrine, cetirizine or other substances in the combination drug.
Relative (Frinosol nasal spray must be used, observing precautions):
- chronic renal failure (CRF);
- obstruction of the bladder neck (for example, due to prostatic hypertrophy);
- prostate adenoma;
- cardiovascular diseases, including arrhythmias, arterial hypertension, generalized atherosclerosis;
- angina pectoris;
- increased seizure activity;
- epilepsy;
- hyperthyroidism;
- diabetes;
- elderly age.
In addition, Frinosol, like any local vasoconstrictor, is prescribed with caution to patients with severe reactions (tremor, dizziness, insomnia, increased blood pressure or cardiac arrhythmia) to sympathomimetics.
Frinosol, instructions for use: method and dosage
Spray Frinosol is used intranasally.
Recommended daily dose for adult patients: 1-2 injections into each nasal passage 2-3 times a day. The duration of continuous therapy should not exceed 7 days.
How to use the nasal spray:
- The bottle is held vertically, with the spray upwards.
- After thoroughly cleaning the nasal passages, they begin to administer the drug.
- Before injection, tilt the head and, after inserting the tip of the bottle into the nasal passage, press the spray bottle once with a short, sharp movement.
- During the injection, inhale lightly through the nose.
Side effects

Overdose
Since the systemic absorption of Frinosol when applied topically is very low, overdose is almost impossible.
Overdose symptoms registered with excessive intake of the active components of the drug separately:
- phenylephrine: possible increased blood pressure, heart rhythm disturbances, agitation;
- cetirizine: dizziness, headache, confusion, anxiety, increased fatigue, weakness, malaise, diarrhea, itching, sedation, drowsiness, stupor, mydriasis, tremor, tachycardia, urinary retention were observed with a single oral dose of 50 mg.
Treatment: stimulation of vomiting or gastric lavage, administration of activated charcoal, intake of large amounts of liquid; further - supportive and symptomatic therapy. There is no specific antidote. Hemodialysis is ineffective. In case of symptoms of an overdose, you should urgently seek medical help.
special instructions
Continuous use of a nasal spray should not exceed 7 days. With excessive or prolonged use of Frinosol, tachyphylaxis and the rebound phenomenon may occur, due to the re-development of nasal congestion (rhinitis medicamentosa), the development of a systemic vasoconstrictor effect is possible.
When using Frinosol, as with the use of similar vasoconstrictor agents, the dose of the drug recommended in the instructions must not be exceeded. Otherwise, manifestations of the systemic action of the drug may occur, especially in elderly patients.
Patients taking monoamine oxidase inhibitors are not recommended to use phenylephrine for 2 weeks after their cancellation, as this can cause an increase in the severity of the adrenergic effects of sympathomimetic drugs and increase the risk of adverse reactions from the cardiovascular system.
Frinosol is intended exclusively for local intranasal use.

Application during pregnancy and lactation

With systemic absorption of phenylephrine, thyroid hormones mutually increase the associated risk of developing coronary insufficiency, especially in coronary atherosclerosis.
