Fortum is an antibiotic from the 3 generation cephalosporin group.
Pharmacological properties of Fortum
Fortum contains an active ingredient - ceftazidime. The mechanism of action of the drug is bactericidal. It consists in disrupting the formation of the cell wall of microorganisms, which causes their death. Fortum is resistant to the damaging action of bacterial enzymes - beta-lactamases.
The spectrum of action of the drug is wide - it has a detrimental effect on gram-negative (Pseudomonas aeruginosa, hemophilic and Escherichia coli, Klebsiella, Proteus, Enterobacteriaceae, Citrobacter, Serrata, Salmonella, Yersinia, Neisseria) and gram-positive (Staphylococcus, Streptococcus), as well as anostridia and etc.).
According to reviews, Fortum is effective against bacteria that are not sensitive to other types of antibiotics. It does not work on methicillin-resistant staphylococci.
The drug is well absorbed from the injection site into the muscle, quickly delivered to the tissues. It practically does not penetrate through the intact blood-brain barrier into the brain, but in the case of inflammation of the meninges (meningitis), therapeutic concentrations of the antibiotic are achieved in the cerebrospinal fluid. Fortum easily crosses the placenta and into breast milk.
The body does not convert Fortum to metabolites. It is excreted unchanged in the urine, about 90% of the administered drug is excreted by the kidneys during the day. With renal failure, the rate of excretion decreases. It is also important to remember that due to the peculiarities of the functioning of the body of newborns, the half-life of the drug slows down 3 to 4 times.
Release form
According to the instructions, Fortum is released in powder for preparation of a solution for injection in vials of 0.25, 0.5, 1 and 2 g.
Indications for use of Fortum
An antibiotic is prescribed for severe infections that are caused by microorganisms that are sensitive to it. It is included in the treatment regimens for sepsis, respiratory tract infections (including those that complicate cystic fibrosis), urinary system, ENT organs, skin and soft tissues, abdominal cavity, biliary tract, digestive tract, osteoarticular system. The advantage of Fortum is that it also helps with nosocomial infections that are caused by microorganisms with multiple resistance to other antibacterial agents.
Contraindications
Fortum is not used for intolerance to ceftazidime, as well as other cephalosporin and penicillin antibiotics.
Instructions for the use of Fortum
According to the instructions, Fortum is injected parenterally into the veins or deep into the muscle. The dose and course are calculated by the attending physician individually, and depends on the severity of the condition, age, kidney condition. Adults are prescribed 1 - 6 g / day in 2 - 3 injections. Standard schemes are 1 g every 8 hours or 2 g every 12 hours. The maximum permissible daily dose for an adult is 6 g, for the elderly it should not exceed 3 g.
In cases of concomitant immunodeficiency and neutropenia, the dose is increased - 2 g every 8 hours 3 times a day or 3 g every 12 hours 2 times a day.
For newborns and infants up to 2 months, the antibiotic calculation is 25 - 60 mg / kg / day in 2 administrations.
For children, starting from the age of 2 months, Fortum is calculated at 30 - 100 mg / kg / day in 2 - 3 injections. With reduced immunity, cystic fibrosis and meningitis, the dose is increased to 150 mg / kg / day.
To prepare an antibiotic solution, you can use 0.9% sodium chloride solution, 5% glucose solution. For intramuscular administration, Fortum is usually diluted with 0.5 or 1% lidocaine.
With the simultaneous use of high doses of Fortum with diuretics and aminoglycoside antibiotics, it is necessary to control renal function.
Side effects of Fortum
According to reviews, Fortum sometimes causes hypersensitivity reactions (in the form of urticaria, dermatitis, Quincke's edema), diarrhea, abdominal pain, vomiting, candidiasis of the mucous membranes, impaired renal function, headache, dizziness, convulsions, changes in biochemical and general blood tests …
