Fexofast is a drug with antiallergic effect.

Release form and composition

The dosage form of Fexofast is film-coated tablets: 120 mg - white, round, biconvex; 180 mg - orange, oblong (in a cardboard box 1-3 or 10 blisters of 10 tablets).

Composition of 1 tablet:

  • active substance: fexofenadine - 120 or 180 mg (fexofenadine hydrochloride - 126 or 189 mg);
  • auxiliary components (120/180 mg): microcrystalline cellulose - 3.9 / 43.5 mg; starch - 2/60 mg; lactose - 2/159 mg; croscarmellose sodium - 13.75 / 36.5 mg; povidone - 0.75 / 3 mg; colloidal silicon dioxide - 0.6 / 1 mg; magnesium stearate - 1/3 mg;
  • shell (120/180 mg): hydroxypropyl methylcellulose - 3.69 / 10.36 mg; propylene glycol - 0.69 / 2.144 mg; purified talc - 0.933 / 2.5 mg; titanium dioxide - 0.937 / 2.812 mg; dye sunset yellow varnish - 0 / 0.936 mg.

Pharmacodynamics

Fexofenadine hydrochloride is a non-sedative histamine H 1 receptor blocker and is a pharmacologically active metabolite of terfenadine.

The antihistamine effect of Fexofast develops 60 minutes after ingestion, it reaches its maximum after 6 hours, its duration is 24 hours.

The emergence of tolerance with repeated use is not noted.

It has no sedative, cholino- and adrenolytic effects.

It does not lead to a change in the function of calcium / potassium channels, the QT interval when used in therapeutic doses.

Pharmacokinetics

After oral administration, fexofenadine hydrochloride is rapidly absorbed from the gastrointestinal tract, reaching Cmax (maximum concentration) in 1-3 hours. The average Cmax after taking 120 and 180 mg is approximately 427 and 494 ng / ml, respectively.

Plasma protein binding ranges from 60 to 70% (mainly with albumin and alpha1-glycoprotein). It does not penetrate the blood-brain barrier.

T1 / 2 (half-life) after repeated administration of fexofenadine - 14.4 hours. In moderate and severe renal failure (creatinine clearance 41–80 and 11–40 ml / min), T1 / 2 increases by 59 and 72%, respectively; in patients on hemodialysis - by 31%.

Fexofenadine hydrochloride undergoes partial extrahepatic metabolism (5% of the dose). It is excreted mainly in the bile (80%), 11% of the substance is excreted unchanged by the kidneys.

Indications for use

  • seasonal allergic rhinitis;
  • chronic urticaria.

Contraindications

  • pregnancy and the period of breastfeeding;
  • age up to 12 years;
  • individual intolerance to the components of the drug.

A relative contraindication is chronic renal failure (Fexofast is used under medical supervision).

Instructions for the use of Fexofast: method and dosage

Fexofast is taken orally.

Frequency rate of application - 1 time per day.

Recommended single dose:

  • seasonal allergic rhinitis: 120 mg;
  • chronic urticaria: 180 mg.

In chronic renal failure, Fexofast is prescribed in a daily dose of 60 mg once.

Side effects

Possible side reactions: dizziness, headache, nausea, drowsiness: rarely (less than 1 case out of 1000 appointments) - insomnia, fatigue, sleep disturbance, nervousness; in some cases - urticaria, skin rash, pruritus, other hypersensitivity reactions (including angioedema, shortness of breath).

Overdose

The main symptoms are dizziness, dry mouth and drowsiness.

Therapy: standard measures aimed at removing the unabsorbed drug from the gastrointestinal tract; symptomatic / supportive treatment. Hemodialysis is not effective.

Drug interactions

Possible interactions:

  • erythromycin, ketoconazole: the plasma concentration of fexofenadine increases significantly;
  • aluminum or magnesium-containing antacids (taken 15 minutes before taking Fexofast): the bioavailability of fexofenadine decreases, the recommended interval is at least 120 minutes.
