Pharmacological properties
Pharmacodynamics
Ethoxidol is one of the inhibitors of free radical processes; has anxiolytic, anticonvulsant, antitoxic, membrane-protective, nootropic effect, increases the body's resistance to stress. Improves blood flow in the ischemic zone, has anti-ischemic properties, exhibits a hypolipidemic effect, limits the zone of ischemic damage, reduces the content of low density lipoproteins and total cholesterol.
Etoxidol increases the body's resistance to the effects of various damaging factors, to oxygen-dependent pathological conditions (ischemia and hypoxia, shock, cerebral circulation disorders, intoxication with antipsychotic drugs and alcohol).
Due to the use of the drug, cerebral metabolism, blood supply to the brain, rheological properties of blood and microcirculation are improved, platelet aggregation decreases, and membrane structures of blood cells (platelets and erythrocytes) are stabilized during hemolysis.
The mechanism of action of Ethoxidol is due to its antioxidant, membrane-protective and antihypoxant effects. It reduces membrane viscosity, inhibits lipid peroxidation, increases membrane fluidity, superoxide oxidase activity and lipid-protein ratio. It modulates the activity of membrane-bound enzymes (adenylate cyclase, calcium-independent phosphodiesterase, acetylcholinesterase), receptor complexes, which leads to an increase in their ability to bind to ligands, improves synoptic transmission, transport of neurotransmitters, and preserves the structural and functional organization of biomembranes.
Pharmacokinetics
After oral administration, ethylmethylhydroxypyridine malate is completely and rapidly absorbed from the gastrointestinal tract, the average rate is 0.569 ± 0.086 hours. After taking 100 mg of Ethoxidol, C max (maximum concentration of the substance) in blood plasma is reached after 0.28 ± 0.08 hours and is 487 ± 72.4 ng / ml, after which the substance is rapidly excreted. T 1/2 (half-life) is 1.46 ± 0.13 hours.
Ethoxidol is rapidly distributed throughout the organs and tissues of the body. It is determined in blood plasma for 7-10 hours.
The drug undergoes intensive metabolism in the liver with the formation of phosphate-3-hydroxypyridine, glucuronoconjugated products.
After i / m administration, Etoxidol is determined in blood plasma for 4 hours. C max with the introduction of 100 mg is in the range of 0.3–1.06 μg / ml, the time to reach it is 0.25 hours.
Ethylmethylhydroxypyridine malate from the bloodstream quickly passes into tissues and organs and is quickly eliminated from the body. The retention time of Ethoxidol is from 1.84 to 2.38 hours.
T 1/2 - 1.15-1.75 hours. The substance is excreted from the body by the kidneys, mainly in the form of glucuronides, unchanged - in a small amount.

Instructions for use of Ethoxidol: method and dosage
Chewable tablets
Ethoxidol is taken orally. The tablets must be chewed and then washed down with water.
Recommended dosage regimen:
- complex therapy of ischemic heart disease, therapy of ischemic stroke: 3 times a day, 1 tablet with a further increase in the dose to an effective one. The maximum single dose is 2 tablets, the daily dose is 8 tablets. The duration of the course is at least 2 months, the doctor may prescribe a second course;
- complex therapy of cognitive disorders: 3-4 times a day, 1 tablet, the duration of use has no restrictions.
Injection solution
Ethoxidol can be administered intramuscularly and intravenously (drip or jet).
The dose is determined individually. Therapy begins with 50-100 mg 1-3 times a day, gradually increasing the dose to an effective one.
Before infusion, the drug is diluted in 0.9% sodium chloride solution.
Intravenous stream Ethoxidol is injected slowly over 5-7 minutes, the drip rate is 40-60 drops / min.
The maximum daily dose is 800 mg.
Recommended dosage regimen:
- acute disorders of cerebral circulation: in the first 2-4 days i.v. (drip) 200-300 mg, then i / m 3 times a day, 100 mg. Duration of use - from 10 to 14 days;
- discirculatory encephalopathy in the decompensation phase: IV (drip or jet) 2-3 times a day, 100 mg for 14 days, then 100 mg / m 2 times a day in the same course;
- course prevention of discirculatory encephalopathy: intramuscularly 2 times a day, 100 mg. Duration of use - from 10 to 14 days;
- mild cognitive impairment in elderly patients and with anxiety disorders: i / m, 100-300 mg per day. Duration of use - from 14 to 30 days;
- alcohol withdrawal syndrome: i / m 2-3 times a day, 100-200 mg, or i / v drip 1-2 times a day for a course of 5 to 7 days;
- acute intoxication with antipsychotic drugs: intravenous injection of 50–300 mg per day for a course of 7 to 14 days.
Side effects
Chewable tablets
Ethoxidol is generally well tolerated. In rare cases, the following disorders may occur: allergic reactions, dyspepsia, nausea and xerostomia, diarrhea (as a rule, they quickly pass on their own or after drug withdrawal).
During a long course, sleep disturbances (in the form of drowsiness or sleep disturbance) and flatulence may occur.
Injection solution
Possible disorders: metallic taste / dry mouth, unpleasant odor, feeling of spreading warmth throughout the body, feeling short of breath, discomfort in the chest, sore throat (as a rule, these side reactions are of a short-term nature and are associated with an excessively high injection rate) …
During a long course, sleep disturbances (in the form of drowsiness or sleep disturbance), nausea and flatulence may occur.
Overdose
Chewable tablets
The development of an overdose is unlikely, due to the low toxicity of Ethoxidol.
The main symptoms: sleep disturbances (in the form of drowsiness, insomnia).
Conducting symptomatic therapy is shown.
Injection solution
The main symptoms are: sleep disturbances (in the form of insomnia, sometimes drowsiness); with the on / in the introduction - a short (no more than 1.5-2 hours) and a slight increase in blood pressure.
Overdose symptoms usually disappear on their own within 24 hours and do not require therapy. In severe cases of insomnia, 10 mg nitrazepam, 10 mg oxazepam, or 5 mg diazepam are prescribed. With an excessive increase in blood pressure, drugs with an antihypertensive effect under the control of blood pressure are indicated.
