Esperal is a pharmacological drug for the treatment of alcoholism. Many reviews to Esperal confirm that regular use of the drug contributes to the formation of a persistent negative effect on alcohol.
Antabuse, Torpedo, Lidevin, Disulfiram, Tetlong-250, Teturam are structural analogues of Esperal in terms of the main active component.
Composition and form of release
According to the instructions, Esperal is produced in the form of tablets for oral administration, as well as in the form of a gel and tablets for subcutaneous implantation.
One Esperal tablet contains 0.5 g of disulfiram, the active component of the drug.
Pharmacological action of Esperal
Esperal's principle of action is to inhibit acetaldehydrooxidase, an enzyme involved in the breakdown and neutralization of ethanol in blood plasma. Partial oxidation of ethanol contributes to the accumulation of its intermediate metabolite, acetaldehyde, in the blood, which leads to toxic poisoning of the body. As a result of general intoxication, unpleasant symptoms appear: nausea, tachycardia, vomiting, malaise, general weakness, a feeling of lack of air, drowsiness and lethargy or excessive agitation.
Judging by the reviews, Esperal is used as a remedy for alcoholism at the stage of addiction when the patient can no longer control the amount of alcohol consumed. After ingestion, 70-90% of the drug is rapidly absorbed from the gastrointestinal tract. The maximum therapeutic effect is achieved 11-12 hours after ingestion. Esperal's effect continues for 9-14 days after stopping therapy.
The use of Esperal in the form of implants allows maintaining a constant concentration of disulfiram (inhibitor) in the body.
Before using Esperal, the patient should refrain from taking alcoholic beverages for two to three days.
Some reviews to Esperal say that the body's reaction to alcohol consumption against the background of this remedy can be both lightning-fast and delayed. In the latter case, the patient may develop toxic hepatitis, which affects the liver, kidneys, and pancreas.
Indications for Esperal's use
According to the instructions, Esperal is used to prevent relapses during the treatment of alcohol dependence, as well as as a detoxification agent during the treatment of chronic alcoholism (with nickel poisoning).
Instructions for use
According to the instructions, Esperal in the form of tablets is taken once a day at a dose of 0.5 g (in the morning, during breakfast). 8-10 days after the start of therapy, a teturam-alcohol test is performed (25-30 ml of 40% ethanol after taking Esperal at a dose of 0.5 g). With a weak reaction, the dose of ethanol is increased by 15-20 ml (the maximum allowable dose of alcohol is 110-120 ml). The test is repeated one to two days later in a hospital and three to five days later on an outpatient basis, with adjusting the doses of ethanol and / or medication (if necessary). In the future, Esperal is recommended to be used in a maintenance daily dose (0.150-0.2 g) for 1-3 years.
Implant tablets of the drug are injected into muscle tissue to a depth of four centimeters. Most often, Esperal is implanted in the left iliac region or in the upper part of the buttocks. After a preliminary procedure for anesthesia and thorough disinfection, a small incision (about 6 mm) is made, the subcutaneous tissue is pushed apart with a trocar and two Esperal tablets (0.1 g each) are injected to a depth of about four centimeters. The procedure is repeated four times, injecting tablets parallel to the skin surface (crosswise around the incision site). The total dose of the drug is 0.8 g (eight tablets). After the end of the implantation procedure, a suture and a sterile bandage are applied to the incision. The duration of Esperal is determined by the dosage of the drug and is selected for each patient individually (from six months to five years).
Esperal in the form of a gel is implanted and acts in the same way as in the form of tablets.
Contraindications for use
The drug should not be used in case of hypersensitivity to its components, with severe pathologies of the cardiovascular system (cardiovascular failure, myocarditis), neuropsychiatric diseases, hepatic and / or renal failure, epilepsy, diabetes mellitus, as well as during pregnancy or lactation …
You should not use the drug while taking alcohol or medicines containing alcohol within the previous 24 hours.

Confusion, memory loss, gastritis, optic neuritis are often observed. In rare cases, cerebral thrombosis, hepatitis, asthenia develop.
Esperal's drug interactions

The simultaneous use of the drug and 5-nitroimidazole derivatives - metronidazole, ornidazole, secnidazole, tinidazole - increases the risk of developing delirious disorders, confusion.
