Cycloptics is an antagonist of m-cholinergic receptors, used topically in ophthalmology to dilate the pupil.

Release form and composition

Cycloptic is released in the form of eye drops 1%: colorless or slightly yellowish transparent solution (in polymeric dropper bottles of 5 ml, in a cardboard box 1 dropper bottle).

1 ml drops contain:

  • active ingredient: cyclopentolate hydrochloride - 10 mg;
  • auxiliary components: potassium chloride, benzalkonium chloride, boric acid, disodium edetate dihydrate, sodium carbonate solution 0.2M or hydrochloric acid 0.1M (to adjust the pH), purified water.

Pharmacological properties

Pharmacodynamics

The active ingredient of the Cycloptic drug is cyclopentolate, a blocker of m-cholinergic receptors, which prevents synaptic transmission of excitation by the mediator of cholinergic synapses, acetylcholine. Blocking m-cholinergic receptors in the ciliary muscle and the sphincter of the pupil promotes its expansion due to the prevailing action of the muscle that dilates the pupil and relaxation of the muscle that narrows the pupil. At the same time, cycloplegia (accommodation paralysis) occurs due to a decrease in the tone of the ciliary muscle.

Stable dilatation of the pupil occurs 25–75 minutes after a single instillation of eye drops. Mydriasis persists for 6-12 hours, with increased sensitivity, slight dilation of the pupil can persist much longer. The residual effects of cycloplegia persist for 12 to 24 hours.

Cycloptic shows weak antispasmodic efficiency, inhibits the secretion of the gastric, salivary, sweat, bronchial and pancreas; increases intraocular pressure; reduces the tone of the vagus nerve, which may result in an increase in heart rate (HR) with a slight increase in blood pressure (BP).

Cyclopentolate crosses the blood-brain barrier (BBB). The use of the drug in medium therapeutic doses is capable of exerting a moderate stimulating effect on the central nervous system (CNS), stimulating respiration.

Pharmacokinetics

Cyclopentolate is well absorbed through the mucous membrane of the eye, it binds to plasma proteins moderately. The required concentration in the central nervous system reaches after 30-60 minutes. The half-life (T 1/2) is 2 hours.

Indications for use

  • diagnostics: to determine refraction, for ophthalmoscopy;
  • complex therapy of inflammatory diseases of the anterior part of the eye: keratitis, iridocyclitis, episcleritis / scleritis, uveitis.

Contraindications

Absolute:

  • post-traumatic paresis of the sphincter of the pupil;
  • suspicion of glaucoma;
  • angle-closure glaucoma;
  • children under 3 years old;
  • period of pregnancy and lactation;
  • hypersensitivity to any of the components of the eye drops.

Relative contraindications in which Cycloptic should be used with caution: intestinal obstruction, prostatic hyperplasia, old age.

Instructions for use Cycloptics: method and dosage

Cycloptic eye drops are intended for topical application by instillation into the conjunctival sac of the eye.

Recommended dosage according to indications:

  • diagnostics (for ophthalmoscopy): 1-2 drops in each eye, if necessary, repeat in the same dose after 5-10 minutes;
  • study of refraction (to achieve cycloplegia): 1-2 drops with an interval of 15-20 minutes 2-3 times a day;
  • inflammatory diseases: 1 drop 3 times a day for 5-10 days, in severe conditions it is allowed to use 1 drop every 3-4 hours.

Side effects

  • local adverse reactions: hyperemia, burning sensation, photophobia, edema of the eyeball and conjunctiva, increased intraocular pressure, blepharoconjunctivitis, decreased visual acuity, punctate keratitis;
  • systemic side reactions (occur more often in children): in rare cases - weakness, dry mouth, nausea, headache, dizziness, drowsiness, bladder atony, acute urinary retention, intestinal atony, tachycardia.

Overdose

Symptoms of an overdose of cyclopentolate are mental disorders / behavioral disorder (incoherent speech, seizures, hallucinations, disorientation in space and time, instability of the emotional state), excessive fatigue, ataxia, impaired recognition of objects at close range, coma (up to death).

For the treatment of the condition, intravenous administration of a specific antidote, physostigmine, is necessary. Children's dose - 0.5 mg, in case of ineffectiveness after 5 minutes, the introduction in this dose is repeated; the maximum allowable dose for children is 2 mg. Adults are injected with 2 mg, in case of ineffectiveness, repeat the administration in a dose of 1–2 mg after 20 minutes.

During the use of Cycloptics, for any indication, it is required to monitor intraocular pressure.

In order to reduce the risk of developing systemic adverse reactions after instillation of eye drops, it is recommended for 1-2 minutes to lightly press with fingers at the inner corners of the eyes on the projection area of the lacrimal sacs.

Influence on the ability to drive vehicles and complex mechanisms

Patients during the use of Cycloptics should refrain from driving vehicles and engaging in other potentially hazardous activities that require psychomotor speed and increased concentration.

Application during pregnancy and lactation

According to the instructions, Cycloptic is contraindicated for use during pregnancy and during breastfeeding.

Pediatric use

In pediatric practice, Cycloptic is not used in children under the age of 3 years.
