Pentalgin Extra-gel is a non-steroidal anti-inflammatory drug (NSAID) for topical use.
Release form and composition
Dosage form - gel for external use: uniform consistency, from yellowish to reddish brown, transparent or translucent, with a characteristic odor, possibly the presence of air bubbles and opalescence (30, 50 and 100 g in aluminum tubes with polymer bushons, in cardboard pack of 1 tube and instructions for use of Pentalgin Extra-gel).
Composition of 1 g of gel:
- active substance: ketoprofen - 50 mg;
- auxiliary components: peppermint leaf oil (peppermint oil), paprika tincture (peppercorn tincture), camphor, sodium hydroxide, dimethyl sulfoxide (dimexide), ethanol 96%, hypromellose, purified water.
Pharmacological properties
Pharmacodynamics
The active ingredient of Pentalgin Extra-gel is ketoprofen, an NSAID that has anti-inflammatory, decongestant and analgesic properties.
The mechanism of action of the drug is due to its ability to suppress the activity of cyclooxygenase 1 and cyclooxygenase 2 - enzymes that regulate the synthesis of prostaglandins.
When applied externally, Pentalgin Extra-gel has a therapeutic effect on the affected muscles, ligaments, tendons and joints. With articular syndrome, it reduces joint swelling, pain at rest and during movement, as well as morning stiffness.
The gel does not damage the articular cartilage.
Pharmacokinetics
After external application, ketoprofen penetrates through the skin into the inflammation focus, from where it is absorbed extremely slowly. The substance practically does not accumulate in the body.
It is characterized by a very low bioavailability - about 5%.
With external use of ketoprofen at a dose of 50-150 mg, the plasma concentration is 0.08-0.15 μg / ml and is observed after 5-8 hours.
Indications for use
- post-traumatic inflammation of soft tissues and musculoskeletal system: bruises, ligament injuries / ruptures;
- muscle pain, including rheumatic pain;
- inflammatory diseases of the musculoskeletal system (both acute and chronic): lesions of ligaments and tendons of an inflammatory nature, bursitis, lumbago, radiculitis, osteoarthritis, sciatica, osteochondrosis with radicular syndrome, articular syndrome with exacerbation of gout.
Pentalgin Extra-gel reduces pain and inflammation during the period of its use, does not affect the progression of the disease.
Contraindications
- a history of skin allergies to perfumes or sunscreens;
- a history of photosensitivity (increased sensitivity of the skin to the effects of solar radiation);
- damage to the skin at the site of the intended application of the drug (weeping dermatoses, wounds, infected abrasions, eczema, etc.);
- age up to 18 years;
- III trimester of pregnancy;
- lactation period;
- complete or incomplete combination of bronchial asthma, recurrent polyposis of the nasal / paranasal sinuses and NSAID / acetylsalicylic acid intolerance at the present time or in history;
- individual hypersensitivity to NSAIDs / acetylsalicylic acid (a history of rhinitis, urticaria or bronchospasm due to the use of these drugs);
- hypersensitivity to any component of Pentalgin Extra-gel, fenofibrate or tiaprofenic acid.
During the treatment and for 2 weeks after its completion, it is forbidden to expose the treated areas of the body to the sun's rays, including the solarium.
Pentalgin Extra-gel should be used with caution in the presence of erosive and ulcerative lesions of the gastrointestinal tract, hepatic porphyria, severe renal or liver dysfunction, bronchial asthma, chronic heart failure, as well as in elderly patients, pregnant in the first and second trimesters.
Pentalgin Extra-gel, instructions for use: method and dosage
Gel Pentalgin Extra-gel should be applied externally: apply in a small amount (with a strip of 3-5 cm) on the inflamed or painful areas of the body and rub gently. The recommended frequency of application is 2 or 3 times a day.
The maximum duration of a therapeutic course is 14 days.
Side effects
- on the part of the skin: infrequently (from 1/1000 to 1/100) - itching, erythema, eczema; rarely (from 1/10 000 to 1/1000) - urticaria, bullous dermatitis, photosensitivity reactions; very rarely (<1/10 000, including single messages) - contact dermatitis, angioedema;
- from the urinary system: very rarely - worsening of the course of renal failure;
- from the immune system: very rarely - hypersensitivity reactions, anaphylactic reactions;
- from the gastrointestinal tract: very rarely - diarrhea, peptic ulcer, bleeding.
Overdose
When applied externally, the systemic absorption of ketoprofen is very low, so the risk of developing an overdose is minimal.
In case of appearance of undesirable skin reactions, the treated areas of the body should be washed, if necessary, the drug should be canceled.
If the gel is accidentally swallowed in a large volume, systemic side effects inherent in NSAIDs may appear. Such patients are advised to undergo gastric lavage and take activated charcoal, further treatment is symptomatic.

Gel Pentalgin Extra-gel is allowed to be applied only to areas of the skin with an intact skin. It is necessary to avoid getting the drug on open wounds, on mucous membranes and in the eyes. Do not apply the product under occlusive dressings.
The gel can be used for phonophoresis.
A transparent film may form at the application site.
Each time after using the gel, you should wash your hands thoroughly.
With prolonged treatment at the site of application of the gel, an increase in sensitivity and the appearance of skin irritation are possible.
The risk of developing systemic side effects of ketoprofen increases when using the drug in high doses and / or for a long time, when treating large areas of the body, as well as in the presence of skin lesions.
During treatment and for 2 weeks after its completion, it is recommended to avoid exposure to direct sunlight and do not go to the solarium, since photosensitivity may occur.
Pentalgin Extra-gel should be withdrawn immediately if the patient develops any skin reaction, including sunscreens or other cosmetics containing octocrylene (organic sunscreen).
Influence on the ability to drive vehicles and complex mechanisms
When applied externally, ketoprofen does not have any negative effect on the cognitive and psychophysical abilities of a person.
Application during pregnancy and lactation
In the I and II trimesters of pregnancy, Pentalgin Extra-gel can only be used as directed by a doctor who will assess the balance of benefits and risks. In the third trimester of pregnancy, the use of the drug is contraindicated.
If therapy is required by a woman during lactation, breastfeeding is recommended to be discontinued for the period of treatment.
Pediatric use
Pentalgin Extra-gel is not prescribed in children and adolescents (up to 18 years).
With impaired renal function
The gel should be used with caution in patients diagnosed with severe renal impairment.
For violations of liver function

Use in the elderly
In old age, the drug should be used with caution.
Drug interactions
The absorption of ketoprofen through the skin is insignificant, however, with frequent and prolonged use of the gel, symptoms of interaction with other drugs may appear (signs similar to the systemic use of ketoprofen).
Pentalgin Extra-gel can enhance the effects of drugs that can cause photosensitization.
Ketoprofen increases the toxicity of methotrexate.
The simultaneous use of ketoprofen and other NSAIDs, ethanol, corticotropin or glucocorticosteroids increases the likelihood of ulcers in the gastrointestinal tract and the risk of bleeding.
Dimethyl sulfoxide (excipient of Pentalgin Extra-gel) enhances the penetration of other drugs through the skin.
Patients who receive coumarin anticoagulants during the period of therapy should regularly monitor the INR (International Normalized Ratio).
