Papaverine hydrochloride is a drug with antispasmodic and hypotensive effects.

Release form and composition

Dosage forms of papaverine hydrochloride release:

  • solution for injections: transparent, slightly colored (in ampoules of 2, 5 or 10 ml, 5 ampoules in blisters, in a cardboard box 1 or 2 packs or 10 ampoules);
  • rectal suppositories: torpedo-shaped, from white with a creamy / yellowish tint to white (in blister strip packs of 5 pcs., in a cardboard box 2 packs);
  • tablets: white, with a line (10 pcs. in blister and non-blister packs, in a cardboard box 1-3 packs).

Composition of 1 ml injection solution:

  • active substance: papaverine hydrochloride - 20 mg;
  • additional components: methionine - 0.1 mg; edetate disodium - 0.05 mg; water for injection - up to 1 ml.

Composition of 1 suppository:

  • active substance: papaverine hydrochloride - 20 mg;
  • additional components: base for suppositories (emulsifier No. 1, hard fat, cosmetic stearin) - in an amount sufficient to obtain a suppository weighing 1250 mg.

Pharmacological properties

Pharmacodynamics

Papaverine hydrochloride is one of the antispasmodic drugs that have an antihypertensive effect.

Main effects:

  • inhibition of phosphodiesterase;
  • a decrease in the content of calcium in the cell and the accumulation of cyclic adenosine monophosphate;
  • decreased tone and relaxation of smooth muscles of internal organs (genitourinary system / gastrointestinal tract) and blood vessels;
  • a decrease in the excitability of the heart muscle and a slowdown in intracardiac conduction (when used in large doses).

It has a mild effect on the central nervous system (it has a sedative effect in large doses).

Pharmacokinetics

The absorption of a substance is determined by the dosage form. On average, the bioavailability of the substance is 54%. The connection with plasma proteins is 90%.

Penetrates through histohematogenous barriers, being distributed in the body.

Metabolized in the liver. The half-life is 0.5–2 hours (it can be extended up to 24 hours). It is excreted by the kidneys in the form of metabolites. In hemodialysis, it is completely removed from the blood.

Indications for use

According to the instructions, Papaverine hydrochloride is prescribed in the following cases:

  • spasms of smooth muscles of the abdominal organs (pylorospasm, cholecystitis, renal colic, spastic colitis);
  • angina pectoris (simultaneously with other drugs);
  • spasms of blood vessels of the brain and heart;
  • bronchospasm;
  • spasm of peripheral vessels (endarteritis).

The drug in the form of tablets and injectable solution is also used for premedication as an adjunct drug.

Contraindications

Absolute:

  • atrioventricular block;
  • severe liver failure;
  • glaucoma;
  • age up to 6 months / 10 years / 18 years (injection solution / tablets / suppositories);
  • elderly age;
  • individual intolerance to the components of the drug.

Relative (Papaverine hydrochloride is prescribed under medical supervision):

Injection solution

Methods of administration of papaverine hydrochloride - intravenous, intramuscular, subcutaneous.

Recommended dosage regimen:

  • intramuscularly, subcutaneously: 2–4 times a day, 20–40 mg;
  • intravenously slowly: 2-4 times a day, 20 mg (after preliminary dilution in 0.9% sodium chloride solution of 10-20 ml).

The maximum single dose for children 6–12 months old is 1 mg, 1–12 years old - 0.2 mg / kg.

Suppositories

Papaverine hydrochloride is administered rectally.

The recommended dosage regimen: 2-3 times a day, 20-40 mg.

Pills

Papaverine hydrochloride is taken orally.

Recommended dosage regimen:

  • adolescents over 14 years old and adults: 3-4 times a day, 40-60 mg; the maximum single dose is 200 mg, the daily dose is 600 mg;
  • children 10-14 years old: 3-4 times a day, 20 mg.

Side effects

  • hematopoietic system: eosinophilia;
  • digestive system: constipation;
  • cardiovascular system: lowering blood pressure, atrioventricular block, ventricular premature beats;
  • central nervous system: drowsiness;
  • liver: increased activity of hepatic transaminases;
  • allergic reactions: skin rash, pruritus, urticaria.

Overdose
