Texared is a non-steroidal anti-inflammatory drug (NSAID).

Release form and composition

Dosage forms:

  • film-coated tablets, 20 mg: oval, biconvex, dividing risk on one side; the film shell is yellow with a brown tint, the core at the break is bright yellow [10 pcs. in blisters made of a combined material (polyvinyl chloride with aluminum foil), in a cardboard box 1 blister];
  • lyophilisate for preparation of solution for injection: yellow with a greenish tinge lyophilized powder (includes 1 bottle of colorless glass containing 20 mg of lyophilisate, and 1 ampoule with 2 ml of solvent; 1 set in a cardboard box or 1 set in a polyvinyl chloride cassette, 1 cassette in a pack cardboard).

  • active ingredient: tenoxicam - 20 mg;
  • auxiliary components: lactose monohydrate - 90 mg; corn starch - 70 mg; pregelatinized starch - 16 mg; magnesium stearate - 1 mg; talc - 3 mg;

Pharmacological properties

Pharmacodynamics

The active component of Texared, a non-steroidal anti-inflammatory drug (NSAID) tenoxicam, is a thienothiazine derivative of oxicam. It has anti-inflammatory, analgesic and antipyretic activity, and also has an antiplatelet effect, preventing platelet aggregation.

The anti-inflammatory effect of tenoxicam is based on its ability to inhibit the activity of cyclooxygenase isoenzymes (COX) involved in the metabolism of arachidonic acid, thus suppressing the synthesis of prostaglandins. The substance does not affect the activity of lipoxygenases. At the same time, it inhibits some functions of leukocytes, including the release of histamine and phagocytosis, and also reduces the concentration of active radicals in the focus of inflammation.

Pharmacokinetics

Tenoxicam is rapidly and completely absorbed from the gastrointestinal tract (GIT) unchanged. Bioavailability is 100%. The maximum concentration (C max) in plasma is reached 2 hours after oral administration. Taking pills after meals or simultaneously with antacids reduces the rate of absorption, but not its degree.

Tenoxicam binds to blood proteins at a level of 99%. Penetrates well into synovial fluid. The systemic clearance of the substance is low, and the half-life (T 1/2) is long. This allows you to take Texared 1 time per day.

Middle T 1/2 -. About 70 h urine excreted C 2 / 3 of the dose, faeces - 1 / 3. The content of tenoxicam in serum with prolonged use is 10-15 mcg / ml, the cumulation of the substance is not observed.

Indications for use

Texared is recommended for use in rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, articular syndrome with exacerbation of gout, bursitis, tendovaginitis, as well as painful inflammation and degenerative diseases of the musculoskeletal system, such as lumbago, sciatica, epicondylitis.

The drug is effective in relieving pain syndrome of weak and moderate intensity of various etiologies: with arthralgia, myalgia, neuralgia, migraine, toothache and headache, algodismenorrhea, pain as a result of injury and burns.

NSAIDs have only a symptomatic effect, they do not affect the course of Texared's disease. It is unable to prevent development, stop progression and cause remission of the underlying disease.

Contraindications

Absolute:

  • full / partial aspirin triad: a combination of asthma attacks (bronchial asthma), recurrent polypous rhinosinusitis and intolerance to NSAIDs, including acetylsalicylic acid (including history);
  • pathologies of the digestive tract: erosive and ulcerative ulceration of the gastrointestinal tract, gastrointestinal bleeding (including anamnesis), inflammatory diseases of the gastrointestinal tract, severe gastritis;
  • hypocoagulation, hemophilia, other blood diseases;
  • liver failure, liver disease in the active phase;
  • severe renal failure, GFR (glomerular filtration rate) less than 30 ml / min; progressive kidney disease;
  • the recovery period after coronary artery bypass grafting;
  • hearing loss;
  • diagnosed hyperkalemia;
  • pathology of the vestibular apparatus;
  • deficiency of G6PD (glucose-6-phosphate dehydrogenase);
  • pregnancy, lactation period;
  • hypersensitivity to the active and / or auxiliary components of Texared tablets and injections.

The drug is prescribed with caution to patients with diabetes mellitus, edema, chronic heart failure, arterial hypertension, ischemic heart disease, peripheral arterial disease, cerebrovascular disease, dyslipidemia / hyperlipidemia, chronic renal failure (GFR from 30 to 60 ml / min), Helicobacter infection pylori, severe somatic diseases, alcoholism; while taking oral glucocorticosteroids (including prednisolone), anticoagulants (including warfarin), antiplatelet agents (including acetylsalicylic acid and clopidogrel), selective serotonin reuptake inhibitors (including citalopram, paroxetine, sertraline, fluoxetine and NSAIDs in elderly patients) sick.

Texared, instructions for use: method and dosage

  • orally: Texared tablets are taken after meals (preferably at the same time), once a day. Dosage for short-term admission - 1 tablet (20 mg) per day, with long-term treatment - ½ tablet (10 mg) per day. For the treatment of acute attacks of gout - 2 tablets (40 mg) per day for the first 2 days, for the next 5 days - 1 tablet (20 mg) per day. For elderly patients, the maximum dose is 20 mg / day;
  • parenteral: Texared injections are intended for intramuscular or intravenous administration. The dosage for short-term administration is 20 mg / day, for long-term treatment - 10 mg / day. In the treatment of gouty arthritis in the initial period (1-2 days) - 40 mg / day, the next 3-5 days - 20 mg / day. The daily dose is administered at a time.

Side effects

  • Gastrointestinal tract (11.4%): nausea, vomiting, pain / burning sensation in the stomach, constipation, diarrhea, flatulence, abdominal pain, NSAID gastropathy, stomatitis, anorexia; prolonged use of high doses can cause ulceration of the gastrointestinal tract mucosa, perforation of the intestinal walls, bleeding - gastrointestinal, uterine, gingival, hemorrhoidal;
  • liver and bile ducts (1-2%): impaired hepatic function; increased activity of alanine aminotransferase (ALT), aspartate aminotransferase (AST), gamma-glutamyl transferase (GGT) and serum bilirubin concentration;
  • skin and subcutaneous tissue (2.5%): itching, rash, erythema, urticaria; extremely rare - photodermatitis, Stevens-Johnson syndrome, Lyell's syndrome;
  • cardiovascular system: tachycardia, heart failure, increased blood pressure;
  • nervous system (2.6%): dizziness, headache, drowsiness, agitation, depression, tinnitus, hearing loss, eye irritation, visual impairment;
  • urinary system (1–2%): increased blood concentration of urea nitrogen and creatinine;
  • hematopoietic system (1-2%): leukopenia, agranulocytosis; rarely - thrombocytopenia, anemia; extremely rare - granulocytopenia, decreased hemoglobin levels;
  • laboratory data: hyperbilirubinemia, hypercreatininemia, increased activity of hepatic transaminases, prolonged bleeding time.

Against the background of parenteral use of Texared, mental disorders (1.7%) and metabolic disorders (1%) were also observed.

Overdose

In case of an overdose of tenoxicam, symptomatic treatment is performed.
