Release form and composition

The drug is produced in the form of a hemostatic sponge: a twisted porous plate with an almost white embossed surface, a yellow coating is applied on one side with active substances (1 pc. In foil containers, in contour packs of laminated aluminum foil 1 container complete with 1 drying agent agent: sponge 2.5 × 3 × 0.5 cm, in a cardboard box 1 or 3 packages, a sponge 4.8 × 4.8 × 0.5 cm, in a cardboard box 2 packages, sponge 9.5 × 4.8 x 0.5 cm, in a carton box of 1, 5 or 10. Pharmacological properties

Pharmacodynamics

Tachocomb is a hemostatic drug for local use. The mechanism of action of the drug is due to the properties of active components - fibrinogen and thrombin, which are located in the form of a dry coating on the surface of the collagen sponge.

After contact with blood, lymph or electrolyte solutions, the components of the sponge coating dissolve and partially penetrate into the tissues of the wound surface. The reaction arising in the process of diffusion of fibrinogen and thrombin initiates the last phase of physiological blood coagulation.

The transformation of fibrinogen into fibrin monomer and its subsequent polymerization with the formation of a fibrin clot (thrombus) promotes tight fixation of the collagen of the sponge on the surface of the wound. With the help of blood coagulation factor XIII, fibrin polymers are crosslinked and a mechanically strong solid network structure is formed. Its good adhesive properties ensure reliable wound closure.

Pharmacokinetics

After entering the body, the active components undergo progressive biodegradation. The metabolism of the fibrin clot occurs by fibrinolysis and phagocytosis, similar to endogenous fibrin. Under the influence of the resorptive granulation tissue of the collagen of the sponge, it breaks down.

Indications for use

The use of the Tachocomb sponge is indicated during surgical operations in adults as an aid to ensure tissue connection and improve hemostasis.

In vascular surgery, a collagen sponge is used to strengthen sutures when standard vascular connection methods are not effective enough.

In addition, for surgical interventions on the lungs, Tachocomb is used to ensure complete tightness.

Contraindications

Absolute:

  • children / adolescents under 18 years of age;
  • hypersensitivity to the active and auxiliary components of the hemostatic sponge.

With caution, only in cases of extreme necessity, it is recommended to resort to the use of Tachocomb during pregnancy and breastfeeding.

Tachocomb, instructions for use: method and dosage

Tachocomb is not intended for intravascular (intravascular) use.

The sponge is applied topically, by applying directly to surgical wound surfaces.

The drug can only be used under sterile conditions and from undamaged packaging, it cannot be re-sterilized.

The outer aluminum foil packaging bag can be opened in the non-sterile operating area, and the sterile inner blister can only be opened in the sterile area.

To use Tachocomb, the wound surface must be prepared in advance by thoroughly cleaning it from blood, disinfectants (solution of ethanol, iodine or heavy metals) and other liquids.

After opening the inner blister, the sponge is removed, moistened in 0.9% sodium chloride solution, immediately applied to the prepared area with the yellow side covered with active substances, and lightly pressed with moistened gloves or a moistened pad for 3-5 minutes.

To avoid adhesion of the sponge to blood-stained gloves or instruments, they should be pre-moistened with 0.9% sodium chloride solution.

Then the pad or glove is carefully removed, for a more reliable and tight hold of the sponge in the right place, you can use a pair of clips at one of its ends.

In case of severe bleeding, the Tachocomb sponge can be applied to the wound surface without preliminary moistening and slightly fixed for 3-5 minutes.

The size of the sponge used depends on the size of the wound surface, if necessary, it can be cut to obtain the required size, and unused fragments disposed of.

The sponge should overlap the wound edges by 1–2 cm.

If the area of the wound surface exceeds the size of one sponge, then the required number of pieces should be used, applying to the wound so that their edges overlap each other.

Side effects

Like any protein product, the Tachocomb sponge, when used in patients with hypersensitivity, can provoke the development of allergic reactions such as urticaria (including in generalized forms), lowering blood pressure, airway obstruction, anaphylactic shock. With the development of such symptoms, the use of a hemostatic sponge should be discontinued immediately.

Other side effects from systems and organs when using Tachocomb collagen sponge:

  • from the immune system: infrequently - hypersensitivity reactions; extremely rare - anaphylactic shock;
  • from the side of the vessels: extremely rarely - thromboembolism against the background of intravascular use of a sponge;
  • general disorders and disorders at the site of application: often - hyperthermia; rarely - the formation of antibodies to fibrin or hemostatic products.

Overdose

Overdose symptoms have not been established.

Intravascular use of Tachocomb is inadmissible. Accidental ingestion of the sponge inside the vessel can lead to life-threatening thromboembolic complications.

If patients develop symptoms of allergic reactions, airway obstruction, low blood pressure or anaphylactic shock, the drug should be discontinued immediately and standard emergency therapy should be started.

When using Tachocomb, the possibility of introducing viral agents cannot be excluded, despite the standard measures for preventing infections in the manufacture of drugs from human blood or plasma. In this connection, it is recommended to record the name and batch number of the drug so that, if necessary, an appropriate analysis can be made. There is a risk of transmission of previously identified or previously unknown viruses and other pathogens, even with careful selection of donors, screening of material from donors for specific markers of infections and special procedures for inactivating or eliminating viruses during the technological process.

Prevention measures are considered effective for enveloped viruses (hepatitis B and C virus, human immunodeficiency virus) and non-enveloped viruses (including hepatitis A virus).

The effectiveness of preventive measures is limited in relation to the non-enveloped parvovirus B19, infection with which is dangerous in immunodeficiency, hemolytic anemia and other conditions characterized by an increase in erythropoiesis, as well as during pregnancy, since the fetus may become infected.

Application during pregnancy and lactation

It is recommended to use Tachocomb during pregnancy or breastfeeding only in cases where the expected effect of therapy for the mother outweighs the potential threat to the fetus or child.

Pediatric use

The use of Tachocomb in children under the age of 18 is contraindicated due to the lack of sufficient data confirming the safety and efficacy of the drug in this category of patients.

Drug interactions

When Tachocomb comes into contact with solutions of ethanol, iodine or heavy metals (including antiseptic solutions), the properties of its components may be disturbed. As a consequence, careful removal of such products from the areas of the intended application of the sponge is required.
