Selamerex is a drug for the treatment of hyperphosphatemia.

Release form and composition

The drug is produced in the form of tablets coated with a film coating: biconvex, oval, a core on a cross section and a film shell - from white to white with a yellow tint, SVL engraving on one side (30, 60, 90 or 180 pcs. In polymer bottle with a screwed polymer cap with child protection and first opening control, or without them; in a cardboard box 1 bottle and instructions for the use of Selamerex).

1 tablet contains:

  • active substance: sevelamer carbonate - 800 mg (in terms of dry matter);
  • additional components: colloidal silicon dioxide, zinc stearate, lactose monohydrate;
  • film shell: polyvinyl alcohol and macrogol copolymer, talc.

Pharmacological properties

Pharmacodynamics

Sevelamer is a drug used for the treatment of hyperphosphatemia and is a polymer of allylamine hydrochloride. By binding phosphates in the gastrointestinal tract (GIT), it reduces their serum levels in the blood. Selamerex does not contain ionized calcium (Ca 2+), as a result of which it does not contribute to the development of hypercalcemia, which is observed when using phosphate-excreting agents that contain calcium ions. The effect on the content of phosphorus in the blood is preserved while taking the drug for one year.

Pharmacokinetics

The active substance is not absorbed in the gastrointestinal tract. With a long course of therapy, the likelihood of systemic absorption and cumulation of sevelamer cannot be completely excluded.

Indications for use

Selamerex is used for hyperphosphatemia in the following cases:

  • against the background of chronic renal failure (CRF) in patients receiving treatment with hemodialysis or peritoneal dialysis, as part of a combination therapy including Ca 2+ preparations, colecalciferol or its analogs;
  • in patients on hemodialysis.

In addition, the drug is recommended to be used as a component of complex therapy aimed at preventing bone damage associated with kidney disease, and including calcium supplements, 1,25-hydroxyvitamin D 3 or one of its analogues.

Contraindications

Absolute:

  • intestinal obstruction;
  • hypophosphatemia;
  • age up to 18 years;
  • hypersensitivity to any component of Selamerex tablets.

Relative (taking the drug requires caution):

  • violation of gastrointestinal motility, including constipation;
  • diseases of the gastrointestinal tract, of an inflammatory nature;
  • surgical operations on the gastrointestinal tract (in history);
  • difficulty swallowing;
  • dysphagia;
  • pregnancy and lactation;
  • combined use with antiepileptic and antiarrhythmic drugs (specialist advice is required).

Selamerex, instructions for use: method and dosage

Selamerex tablets are taken orally with meals. They are swallowed whole, without crushing or chewing. During the period of therapy, it is required to adhere to the established diet.

If patients have not received phosphate binders, then the dose of the drug should be set individually, depending on the level of phosphoric acid salts in serum. In this case, at a phosphate concentration of 1.76-2.42 mmol / l (5.5-7.5 mg / ml), an initial dose of Selamerex 800 mg 3 times a day is recommended, at a concentration of> 2.42 mmol / l (> 7.5 mg / ml) - 1600 mg with the same frequency of administration.

If patients have taken phosphate-binding agents (Ca 2+ preparations), then sevelamer is used in equivalent doses in terms of 1 mg of these preparations, including Ca 2+ (by base).

The dose can vary from 800 to 4000 mg, while the maximum amount of sevelamer taken per day should not exceed 7000 mg. The dose is set taking into account the serum phosphate content in the blood; it is recommended to reduce its concentration to 1.76 mmol / l (5.5 mg / dl) and below.

Phosphate levels are measured every 2-3 weeks until stabilization is achieved.

Side effects

Possible side reactions when taking sevelamer:

  • cardiovascular system: very often - increase / decrease in blood pressure;
  • digestive system: very often - abdominal pain, nausea, vomiting, dyspepsia, constipation, diarrhea; often - flatulence;
  • nervous system: very often - headache;
  • skin: very often - itching; often a rash;
  • others: very often - pain of various localization; often - pharyngitis.

Intestinal obstruction has been noted extremely rarely in clinical practice. The most common adverse reactions were observed in patients in the terminal stage of chronic renal failure.

Overdose

Overdose cases of sevelamer have not been recorded.

During treatment with Selamerex, a deficiency of vitamins A, D, E, K and B 9 (folic acid) in the blood may occur, as a result of which the latter can be prescribed additionally.

If 1 dose is missed, the next dose must be taken at the usual time. In this case, doubling the dose in order to make up for the missed dose should not be.

Pediatric use

Persons under 18 years of age are contraindicated to use Selamerex, since there are no data confirming the safety and efficacy of its administration in patients of this age category.

With impaired renal function

Patients with impaired renal function use Selamerex according to indications, according to the recommended dosing regimen.

Drug interactions

  • mycophenolate mofetil, cyclosporine, tacrolimus: sevelamer weakens the effect of these substances;
  • ciprofloxacin: its bioavailability decreases by 50%;
  • drugs for which a decrease in bioavailability may have a certain clinical significance: it is recommended to use these drugs 1 hour before or 3 hours after taking sevelamer;
  • levothyroxine: in rare cases, there may be a deficiency of thyroid hormones.
