Instenon is a combined drug that improves blood circulation and brain metabolism.
Release form and composition
Instenon is available in the following dosage forms:
- Film-coated tablets: round, with a white core and an even shiny surface, white (20 + 50 + 60 mg) or orange (60 + 100 + 60 mg) (10 pcs. In blisters, 3 each (20 + 50 + 60 mg) or 5 (60 + 100 + 60 mg) blisters in a cardboard box);
- Solution for intravenous and intramuscular administration: colorless, transparent (2 ml in ampoules, 5 ampoules in blisters, 1 package in a cardboard box).
The composition of 1 tablet contains active substances:
- Hexobendine dihydrochloride - 20/60 mg;
- Etamivan - 50/100 mg;
- Etophyllin - 60/60 mg.
Auxiliary components: magnesium stearate - 2/3 mg; pregelatinized starch - 7.67 / 17.12 mg; talc - 4/8 mg; lactose monohydrate - 10/12 mg; corn starch - 36.33 / 29.88 mg.
Sheath: povidone K30 - 1.2 / 1.4 mg; glycolic mountain wax - 0.1 / 0.1 mg; macrogol 6000 - 2.4 / 2.8 mg; acacia gum - 4.75 / 5.6 mg; sucrose - 34.33 / 39.3 mg; talc - 26.75 / 28 mg; titanium dioxide - 0.47 / 1.4 mg; additionally for tablets 60 + 100 + 60 mg: dye yellow "Sunset" - 1.4 mg.
The composition of 1 ampoule (2 ml) of solution for intravenous and intramuscular administration includes active substances:
- Hexobendine dihydrochloride - 10 mg;
- Etamivan - 50 mg;
- Etophyllin - 100 mg
Auxiliary components: propylene glycol - 416 mg, disodium edetate - 0.08 mg, water for injection - up to 2 ml.
Pharmacological properties
Instenon improves cerebral and coronary circulation, enhances metabolic processes in the brain and myocardium, and also has a coronary dilating, analeptic and spasmolytic effect.
Pharmacodynamics
The active components of Instenon demonstrate a joint and unidirectional effect on various components of the pathogenesis of hypoxic and ischemic lesions of the central nervous system.
Pharmacodynamic properties of the active ingredients of Instenon:
- hexobendine: provides an increase in the transport and utilization of oxygen and glucose by brain cells as a result of stimulating the flow of glycolysis in anoxic conditions and activation of pentose cycles. Processes are carried out only against the background of hypoxia and ischemia, in violation of the cycle of aerobic oxidative phosphorylation. Due to the stimulation of anaerobic oxidation, an energy substrate is formed, which is used in the synthesis and exchange of mediators and the restoration of synaptic transmission, the depression of which, as well as the destruction of neuronal membranes, is one of the main pathogenetic mechanisms of consciousness disorders and neurological disorders in ischemia and hypoxia of the brain. In addition, hexobendine helps to stabilize the physiological mechanisms of autoregulation of cardiac and cerebral blood flow,due to the influence of the products of anaerobic glycolysis (pyruvate and lactate) on the receptor apparatus of intracranial arterioles and capillaries;
- etamivan: promotes the excitation of the respiratory and vasomotor centers, exhibits a pronounced stimulating effect on the reticular formation of the brain stem and the limbic system, as a result of which the adequate functioning of the neural complexes of the cortex and subcortical-stem formations is preserved;
- etophyllin: promotes an increase in myocardial metabolism and an increase in the cardiac output, thereby increasing the perfusion pressure in the vessels of the marginal zone of ischemia, but without affecting the systemic arterial pressure. The stimulating effect of the substance on the nervous system is also manifested in the stimulation of the subcortical formations, midbrain, stem centers and nuclei (vasomotor, respiratory, centers of autonomic regulation, as well as the nuclei of the cranial nerves, primarily the vagus nerve).
The use of Instenon in closed craniocerebral trauma, ischemic stroke helps to restore the impaired consciousness of the patient, activate the vegetative sphere, and regress neurological disorders.
Pharmacokinetics
Special studies to study the pharmacokinetics of the drug have not yet been conducted.
Indications for use
- Brain diseases of vascular origin and associated with age-related changes;
- Post-stroke conditions;
- Dizziness associated with functional disorders of the brain;
- Consequences of insufficient cerebral blood supply.
Contraindications
- Convulsive syndrome;
- A state of intense excitement;
- Symptoms of increased intracranial pressure (vomiting, prolonged headache, visual impairment);
- Cerebral hemorrhage;
- Epilepsy;
- Hypersensitivity to drug components.
According to the instructions, Instenon should be used with caution after a recent stroke and with stenosis of the carotid artery due to arteriosclerosis.
Nursing and pregnant women can use the drug only under strict indications.
Instructions for use Instenon: method and dosage
Instenon in the form of tablets is taken orally, during or after meals, without chewing and drinking a small amount of liquid.
Depending on the indications, the drug is prescribed 3 times a day, 1-2 tablets (20 + 50 + 60 mg) or 1 tablet (60 + 100 + 60 mg). If necessary, the daily dose can be increased to 5 tablets (60 + 100 + 60 mg), divided into several doses.
The duration of therapy should be at least 6 weeks.
Instenon in the form of a solution for injection is administered intramuscularly or intravenously by drop or slow.
Depending on the severity of the condition, the drug is administered 1-2 times a day, 1 ampoule (intramuscularly or intravenously slowly, or as a drop infusion with 5% glucose solution (250 ml)). After improvement of the condition, therapy can be continued by taking Instenon inside in the form of coated tablets.
With the rapid introduction of the solution in some patients, blood pressure may decrease, and therefore the duration of intravenous administration of 1 ampoule should be at least 3 minutes.
Side effects
In some cases, during the use of high doses of Instenon, blood pressure may decrease, facial flushing and headache may develop.
Overdose
The main symptoms of an Instenon overdose may include facial flushing, prolonged headache, and tachycardia. In this condition, the drug should be discontinued, subsequently resuming therapy at a lower dose.

Influence on the ability to drive vehicles and complex mechanisms
During the period of use of Instenon, care must be taken when performing potentially dangerous types of work that require increased attention and quick psychomotor reactions.
Drug interactions
With the simultaneous use of Instenon with drugs that reduce the activity of platelets (including acetylsalicylic acid), an increase in the antiplatelet effect is possible (due to the influence of hexobendine).
A decrease in the effectiveness of Instenon can be observed when using high doses of caffeine (it is recommended to avoid drinking large quantities of tea and coffee).

In a few reviews of Instenon, patients who have used the drug note that it improves blood circulation in the brain, supports mental performance, relieves dizziness, and has a positive effect on the cardiovascular system. There are very few complaints about side reactions - occasionally they note the appearance of flushing of the face, as well as the diuretic effect of this drug.
