Dosage form Donormil - film-coated tablets: white, rectangular with a dividing line on both sides (30 pcs. In polypropylene tubes, 1 tube in a cardboard box).

Pharmacological properties

Doxylamine is a blocker of H ₁ -histamine receptors belonging to the group of ethanolamines. It is characterized by sedative, hypnotic and m-anticholinergic action. Donormil shortens the time to fall asleep, increases the duration and improves the quality of sleep, while not affecting the sleep phase. Doxylamine works for 6-8 hours.

Pharmacokinetics

On average, the maximum plasma doxylamine content is reached 2 hours after oral administration. The half-life is approximately 10 hours. The absorption of the active ingredient is quite high. The metabolic processes in which doxylamine is involved take place in the liver. The substance well overcomes histohematological barriers (including the BBB). About 60% of doxylamine is excreted in the urine unchanged, partial excretion is carried out through the gastrointestinal tract.

In patients over 65 years of age, as well as in those diagnosed with renal or hepatic insufficiency, an increase in the half-life is possible. With repeated courses of treatment, the stable concentration of doxylamine and its metabolites in the blood plasma is higher and is achieved over a longer period of time.

Indications for use

According to the instructions, Donormil is indicated for patients with transient sleep disorders.

Contraindications

Glucose-galactose malabsorption, congenital galactosemia, lactase deficiency;
Closed-angle glaucoma;
Diseases of the prostate and urethra, accompanied by a violation of the outflow of urine;
Age up to 15 years;
Breastfeeding period;
Hypersensitivity to drug components.

Donormil is prescribed with caution to patients: over 65 years of age - due to a likely increase in T1 / 2 or the danger of falling after taking sleeping pills during night awakenings (dizziness and delayed reactions are possible); with a history of apnea, since while taking the drug there is a risk of sudden respiratory arrest during sleep (doxylamine succinate can aggravate sleep apnea syndrome); with hepatic and renal failure (possibly an increase in T1 / 2).

Instructions for the use of Donormil: method and dosage

Donormil tablets are taken orally, 15-30 minutes before bedtime, with a small amount of liquid.

The recommended dosage is 1/2 or 1 tablet 1 time per day, for 2-5 days.

If therapy is ineffective, the doctor may increase the dose to 2 tablets.

If sleep disturbances persist, a doctor should be consulted.

For patients with hepatic and renal insufficiency and over 65 years of age, a dose adjustment downward is recommended.

Overdose

Symptoms of an overdose of Donormil include daytime sleepiness, seizures (epileptic syndrome), agitation, accommodation disturbances, mydriasis (pupil dilation), involuntary movements (athetosis), dry mouth, tremors (tremors), impaired coordination of movements, redness of the neck skin and face (hyperemia), sinus tachycardia, increased body temperature (hyperthermia), anxiety, confusion, mood deterioration, hallucinations, coma. In some cases, involuntary movements precede seizures, which serves as confirmation of severe intoxication of the body. Even in the absence of convulsive seizures, high-severity doxylamine poisoning can provoke the development of rhabdomyolysis, often accompanied by acute renal failure. At the same time, specialists recommend standard therapy,including constant monitoring of the level of creatine phosphokinase. If there are signs of an overdose of doxylamine, you should immediately consult a doctor.

In case of drug poisoning, symptomatic therapy is indicated, including m-cholinomimetics and other drugs. First aid measures also involve taking activated charcoal (for adults, the dose is 50 g, for children - 1 g / kg of body weight).

Simultaneous use of Donormil with barbiturates, sedative antidepressants (amitriptyline, doxepin, mianserin, mirtazapine, trimipramine), benzodiazepines, pizotifen, morphine derivatives (analgesics, anti-cough drugs), clonidine, antipsychotics, antipsychotics, antipsychotic drugs, thalidomide leads to an increase in the inhibitory effect on the nervous system.

The likelihood of side effects in the form of dry mouth, urinary retention, constipation increases when Donormil is combined with m-anticholinergic drugs (imipramine antidepressants, atropine and atropine antispasmodics, antiparkinsonian drugs, phenothiazine neuroleptics, disopyramide).
