Release form and composition

Dosage form - capsules: size No. 0, hard, gelatinous, with an opaque light yellow body and an opaque dark green lid; content - powder from white to white with a yellow tinge, may contain conglomerates, which, when lightly pressed with a glass rod, turn into a free-flowing powder (10 pcs. in blisters, in a cardboard box 3 blisters).

Composition of 1 capsule:

  • active substance: calcium dobesilate (in the form of monohydrate) - 500 mg;
  • excipients: magnesium stearate, corn starch;
  • capsule body: titanium dioxide E171, iron dye yellow oxide E172;
  • capsule cap: titanium dioxide E171, gelatin, indigo carmine dye E132, iron dye black oxide E 172, iron dye yellow oxide E172.

Pharmacological properties

Pharmacodynamics

Doxy-Hem is an angioprotective agent that:

  • increases the resistance of the capillary walls;
  • reduces increased vascular permeability;
  • increases the elasticity of the erythrocyte membrane;
  • moderately reduces platelet aggregation and blood viscosity;
  • improves microcirculation and drainage function of lymphatic vessels.

The effect of the drug is to some extent associated with an increase in the activity of plasma kinins.

Pharmacokinetics

Calcium dobesylate is rapidly absorbed in the gastrointestinal tract. The maximum concentration in blood plasma reaches 6 hours after oral administration.

Plasma protein binding is about 20-25%.

Practically does not penetrate the blood-brain barrier. In very small amounts (0.0004 mg / ml after taking 1500 mg of the drug) is excreted in breast milk.

The half-life is 5 hours. It is excreted mainly unchanged in approximately equal amounts (50% each) through the intestines and kidneys. In the form of metabolites, 10% of the substance is excreted. The time for complete elimination from the body is 24 hours.

Indications for use

  • vascular lesions accompanied by increased capillary permeability and fragility (nephropathy and diabetic retinopathy) and other microangiopathies associated with various diseases of the cardiovascular system and metabolism;
  • venous insufficiency of varying severity and its complications: pre-varicose condition with pain, symptoms of tissue edema, congestive dermatosis and paresthesias, varicose veins, superficial phlebitis, trophic ulcers.

Contraindications

  • hemorrhages caused by anticoagulants;
  • bleeding from the gastrointestinal tract;
  • period of exacerbation of gastric ulcer and 12 duodenal ulcer;
  • kidney and liver diseases;
  • age up to 13 years;
  • I trimester of pregnancy;
  • hypersensitivity to any component of the drug.

Instructions for the use of Doxy-Hem: method and dosage

Doxy-Hem should be taken with food, swallowing the capsules whole.

During the first 2-3 weeks, 1 capsule is prescribed 3 times a day, then the frequency of administration is reduced to 1 time per day.

In case of retinopathy and microangiopathy, 1 capsule 3 times a day is recommended to be taken for 4–6 months and only then to reduce the frequency of administration to 1 time per day.

Depending on the indications and the therapeutic effect, the course of treatment can last from 3 weeks to several months.

Side effects

Doxy-Chem is generally well tolerated, in rare cases (0.01-0.1%), the following adverse reactions are possible:

  • from the skin and subcutaneous tissues: allergic reactions (itching, rash);
  • from the musculoskeletal and connective tissue: arthralgia;
  • from the gastrointestinal tract: nausea, vomiting, diarrhea;
  • others: fever, chills.

In some cases (0.01-0.1%), reversible agranulocytosis occurs (disappears after drug withdrawal).

Overdose

Overdose cases are unknown.

In very rare cases, calcium dobesylate causes agranulocytosis, its first signs: inflammation of the oral mucosa, pain when swallowing, headache, weakness, chills, fever. If these symptoms appear, you should immediately consult a doctor and conduct a clinical blood test.

The drug can interfere with the results of laboratory tests to determine the level of creatinine.

Influence on the ability to drive vehicles and complex mechanisms

Calcium dobesylate does not negatively affect the speed of mental and physical reactions, as well as the ability to concentrate.

Application during pregnancy and lactation

Adequate and well-controlled studies on the safety of the use of calcium dobesilate in pregnant and lactating women have not been conducted. In this regard, in the first trimester of pregnancy, Doxy-Chem is contraindicated, in the second and third trimesters it can be used only for health reasons and provided that the expected therapeutic effect exceeds the potential risks.

If necessary, treatment during lactation should consider stopping breastfeeding.

Pediatric use

In pediatric practice, the use of the drug is contraindicated for the treatment of children under the age of 13.

With impaired renal function

Doxy-Hem is contraindicated in kidney disease.

For violations of liver function

In liver diseases, the use of the drug is contraindicated.
