Dioxisept is an antimicrobial drug, quinoxaline, used in the treatment of severe forms of infectious diseases or in cases of ineffectiveness of other antibacterial agents.

Release form and composition

Dioxisept dosage forms:

  • solution for injection and topical application: transparent, light yellow with a greenish tinge (10 ampoules in a package of 5 or 10 ml each);
  • solution for intracavitary administration and external use: transparent, greenish-yellow (in ampoules of 5 or 10 ml, in a cardboard box 10 ampoules with / without an ampoule scarifier or a knife for opening ampoules or 2 blister packs of 5 ampoules).

Each pack also contains instructions for the use of Dioxisept.

Composition of 1 ml solution for injection and local use:

  • active substance: hydroxymethylquinoxalindioxide (dioxidine) - 5 mg;
  • auxiliary component: water for injection - up to 1 ml.

Pharmacological properties

Pharmacodynamics

Dioxidine is a broad-spectrum antibacterial agent, belongs to the group of quinoxaline derivatives. The substance has chemotherapeutic activity in infections caused by Klebsiella, Proteus vulgaris, Pseudomonas aeruginosa and dysentery bacillus, Salmonella, streptococci, staphylococci, pathogenic anaerobes (including pathogens of gas gangrene). It has an effect on bacterial strains that show resistance to other chemotherapy drugs, including antibacterial agents.

When using Dioxisept, drug resistance of bacteria can develop. For dioxidine when administered intravenously, a small therapeutic range is characteristic, therefore, strict adherence to the dosage regimen is required. The substance has a beneficial effect on the course of the wound process. When treating purulent-necrotic and burn wounds with dioxidine, there is a faster cleaning of the wound surface, stimulation of reparative regeneration and marginal epithelialization.

Pharmacokinetics

With local / external use, dioxidine is partially absorbed from the burn or wound surface.

The therapeutic concentration of dioxidine in the blood after intravenous administration is maintained for 4-6 hours. It penetrates into all organs and tissues quickly and well. After a single injection, the time to reach the maximum concentration of the substance in the blood is in the range of 1–2 hours; with repeated injections, dioxidine does not accumulate.

Excretion is carried out by the kidneys.

Indications for use

Dioxisept is prescribed for the treatment of purulent bacterial infections caused by sensitive microflora, in cases of ineffective use of other chemotherapeutic agents or if they are poorly tolerated:

  • outwardly: with superficial and deep wounds of different localization, long-term non-healing wounds and trophic ulcers, infected burns, phlegmons of soft tissues, purulent wounds with osteomyelitis;
  • intracavitary: for the treatment of purulent processes in the chest / abdominal cavity, purulent pleurisy, pleural empyema, lung abscesses, peritonitis, cystitis, wounds with deep purulent cavities (soft tissue abscesses, phlegmons of pelvic tissue, postoperative wounds of biliary and urinary tract and other purulent.);
  • endotracheal: with purulent tracheobronchitis;
  • intravenously: in severe septic conditions (including with burn disease), purulent meningitis, severe purulent-inflammatory processes, accompanied by symptoms of generalization of the infection.

Contraindications

Absolute:

  • adrenal insufficiency, including a burdened history;
  • pregnancy and lactation;
  • age up to 18 years;
  • individual intolerance to the components of the drug and other drugs belonging to the quinoxaline group.

A relative contraindication to the use of Dioxisept, in which care must be taken, is renal failure.

Dioxisept is used in a hospital setting.

Solution for injection and topical use

Dioxisept route of administration: intracavitary, externally, endotracheally, intravenously.

Intracavitary introduction

For the treatment of a purulent cavity, 0.5% Dioxisept solution is used. The drug in a volume of 20–100 ml (depending on the size of the cavity) is injected through a drainage tube, catheter, or with a syringe.

Usually the drug is administered once a day, if indicated, you can divide the daily dose into 2 administrations. The maximum daily dose of a 0.5% solution for intracavitary administration is 140 ml. If indicated and well tolerated, the drug can be used daily for three weeks or longer. After 1–1.5 months, repeated courses are possible, if necessary.

External use

0.1–0.5% drug solutions are used. To obtain 0.1–0.2% solutions, ampoule solutions of Dioxisept are diluted with water for injection or sterile isotonic sodium chloride solution to the desired concentration.

When treating superficial infected purulent wounds, napkins soaked in a 0.5% dioxidine solution are applied to them. After treatment, deep wounds are loosely tamped with tampons soaked in a 0.5% solution. If there is a drainage tube, 20-100 ml of Dioxisept of the same concentration can be injected into the cavity.

For deep purulent wounds with osteomyelitis (wounds of the hands and feet), a 0.5% solution is used in the form of baths. You can also carry out a special treatment of the wound with Dioxisept for 15–20 minutes (by injecting the solution into the wound for a specified period), after which a bandage with a dioxidine solution is applied.

Solutions of 0.1–0.5% can be used to prevent infection after surgery. According to indications (in patients with osteomyelitis) and with good tolerance, therapy can be performed daily for 1.5–2 months.

Endotracheal introduction

For diagnostic, therapeutic and sanitary bronchoscopy, a 0.5% solution of Dioxisept is used in a volume of 10–20 ml. For ultrasonic inhalations (in the treatment of purulent tracheobronchitis), the drug is used in the same dose.

Intravenous administration

Dioxidine 0.5% is injected intravenously by the drop method, the solution is diluted with 0.9% sodium chloride solution or 5% dextrose solution to a concentration of 0.1–0.2%. The daily dose can be administered as a single dose or in divided doses (in 3-4 doses).

Recommended doses of 0.5% Dioxisept (determined by the localization of the infectious process):

  • purulent urinary tract infections: 200-400 mg (40-80 ml);
  • chronic purulent processes in the lungs: 500-600 mg (100-120 ml);
  • purulent meningitis: 600-700 mg (120-140 ml);
  • severe septic conditions: 600-900 mg, administration is carried out in 3-4 doses, the solution is preliminarily diluted to a concentration of 0.1-0.2% in 0.9% sodium chloride solution or 5% dextrose solution.

Intracavitary introduction

Depending on the size, 10–50 ml of 1% Dioxisept solution is injected into the cavity through a drainage tube, catheter or with a syringe. The maximum daily dose is 70 ml. Usually the drug is administered once a day, if indicated, you can divide the daily dose into 2 administrations.

If indicated and well tolerated, the drug can be used daily for 3 weeks or longer. After 1–1.5 months, repeated courses are possible, if necessary.

Elderly patients and patients with impaired renal function, the dose of the drug should be reduced by 2 times.

External use

0.1–1% solutions are used. To obtain the drug of the desired concentration, it is diluted with water for injection or sterile 0.9% sodium chloride solution.

In case of superficial infected purulent wounds, the wound surface is preliminarily cleaned of purulent necrotic masses, after which napkins soaked in a 0.5–1% solution are applied. Dressings, depending on the course of the wound process or the condition of the wound, should be changed daily or every other day.

Deep wounds after treatment should be loosely tamped with tampons soaked in 1% solution. Dressings are done once a day.

The maximum daily dose of 1% Dioxisept is 250 ml, the duration of use is up to three weeks.

For the treatment of deep purulent wounds in osteomyelitis (for wounds of the hands or feet), a 0.5-1% solution is used in the form of baths. It is possible to carry out a special treatment of the wound for 15–20 minutes (by injecting the solution into the wound for a specified time), after which a bandage soaked in 1% solution is applied.

Dioxisept 0.1–0.5% can be used after surgery to prevent infection.

With good tolerance and indications (in patients with osteomyelitis), treatment can be performed daily for 1.5–2 months.

Side effects

With intracavitary and intravenous administration, the following undesirable effects may develop:

  • nervous system: chills, headache, fever;
  • immune system: allergic reactions (in the form of rash, urticaria), photosensitizing effect (in the form of the appearance of pigmented spots on the body under the influence of sunlight);
  • musculoskeletal and connective tissue: muscle cramps;
  • gastrointestinal tract: dyspeptic disorders.

With external use of Dioxisept, the development of peri-wound dermatitis is possible.

Overdose

The main symptoms are acute adrenal insufficiency.

Therapy: immediate withdrawal of the drug and appropriate hormone replacement therapy are indicated.

special instructions

A solution of 1% cannot be administered intravenously, which is associated with the instability of the solution during storage at low temperatures.

In order to prevent side effects, it is recommended to use Dioxisept in combination with calcium preparations and antihistamines.

Before the start of the course of treatment, a test for the tolerance of Dioxisept is carried out, for this, 10 ml of a 0.5 or 1% solution is injected into the cavity. If there are no side reactions (in the form of dizziness, chills, fever) within 3-6 hours, a course application is started.

Dioxisept should be prescribed only for severe forms of infectious diseases or in cases of ineffectiveness of other antibacterial agents, including cephalosporins of II – IV generations, carbapenems, fluoroquinolones.

In chronic renal failure, the dose of Dioxisept should be reduced.

In the event of the appearance of age spots, the duration of a single dose should be increased to 1.5–2 hours. It is also possible to reduce the dose or cancel the drug, the use of antihistamines.

During storage of Dioxisept, the precipitation of crystals of hydroxymethylquinoxylindoxide is permissible. In such cases, the crystals are dissolved by heating the ampoules in a boiling water bath and periodically shaking them until a clear solution is obtained (complete dissolution of the crystals). If after cooling to 36–38 ° С crystals do not fall out again, the drug can be used.

During the period of therapy with Dioxisept, care must be taken when driving.

Application during pregnancy and lactation

Dioxisept is contraindicated during pregnancy / lactation.
