Gel for external use Detragel
Gel for external use Detragel

Detragel is a combined topical preparation with venotonic, anticoagulant, antiplatelet and anti-inflammatory effects.

Release form and composition

The drug is produced in the form of a gel for external use: a jelly-like mass of light yellow color, with a transparent structure and a characteristic odor (40 g each in aluminum tubes, in a cardboard box 1 tube and instructions for using Detragel).

1 g of gel contains:

  • active ingredients: sodium heparin - 100 IU (International Unit); escin - 10 mg; essential phospholipids - 10 mg;
  • auxiliary components: isopropanol, ethyl parahydroxybenzoate, glycerol 85%, methyl parahydroxybenzoate, trolamine, propyl parahydroxybenzoate, cologne, carbomer-940, lavender oil, rosemary oil, water.

Pharmacological properties

Pharmacodynamics

Detragel is a combined preparation for topical use, the effectiveness of which ensures the action of the active components that make up its composition and possess pharmacodynamic and pharmacokinetic synergism. The use of the gel causes a decrease in vein permeability and an improvement in microcirculation.

The mechanism of action is due to the following properties of the active ingredients in Detragel:

  • sodium heparin: being a direct anticoagulant, it inactivates biogenic amines and promotes blocking of lysosomal enzymes in tissue. Thanks to this, it has an anti-inflammatory effect, prevents thrombus formation, accelerates the dissolution of microthrombi in the subcutaneous capillaries and activates the fibrinolytic system. Improves microcirculation; against the background of inhibition of hyaluronidase activity, the regeneration of connective tissue is accelerated;
  • essential phospholipids: affect the metabolism of fats, causing a decrease in blood viscosity, a decrease in platelet aggregation processes;
  • escin: a plant component, it is a triterpene glycoside (saponin) derived from the fruit or seeds of horse chestnut. Venotonic agent with anti-inflammatory, angioprotective, and anti-edematous action. Interferes with the activation of lysosomal enzymes that break down proteoglycan. Eliminates venous congestion, increases the tone of the walls of venous vessels, reduces fragility and capillary permeability. Reduces fluid effusion in the tissue, accelerates the resorption of existing edema, and reduces exudation. Helps to slow down inflammatory processes and tissue repair, improves microcirculation.

Pharmacokinetics

After applying the gel to the skin, sodium heparin rapidly penetrates through the epidermis. It accumulates in the upper layers of the skin, and is absorbed into the systemic circulation in a small amount (less than 0.2%). The maximum concentration (Cmax) of sodium heparin in the blood is reached 8 hours after application.

Biotransformation of sodium heparin occurs in the liver and in the reticuloendothelial system. The half-life (T 1/2) is 12 hours. It is excreted mainly through the kidneys. Does not cross the placental barrier and does not penetrate into breast milk.

Essential phospholipids have hydrophilic and lipophilic properties, which promote the rapid penetration of active substances through the sebaceous and sweat glands.

Indications for use

  • varicose veins accompanied by the following symptoms: spider veins and / or asterisks (telangiectasia), varicose veins, pain syndrome, edema, feeling of heaviness and fatigue in the legs, night cramps of the calf muscles;
  • superficial phlebitis;
  • chronic venous disease (to treat symptoms);
  • thrombophlebitis;
  • hematomas with injuries, including sports sprains and bruises;
  • postoperative hematomas in areas without compromising the integrity of the skin.

Contraindications

Absolute:

  • hemorrhagic diathesis, including thrombocytopenic purpura;
  • hemophilia;
  • violation at the site of application of the integrity of the skin of various origins: ulcerative necrotic lesions, open wounds, skin infections, burns, eczema;
  • damage to the mucous membranes of the body;
  • age up to 18 years;
  • individual intolerance to the components in Detragel.

With caution, only as directed by a doctor, Detragel should be used during pregnancy and breastfeeding.

Detragel, instructions for use: method and dosage

The gel is applied externally, by applying a thin layer to the problem area of the skin of the body and even distribution with massaging light movements. You can not rub the gel with thrombophlebitis.

If necessary, Detragel can be applied under a bandage or elastic stockings.

Recommended frequency of procedures: 2-3 times a day. The duration of the course is no more than 15 days.

If the desired therapeutic effect is absent after 15 days of treatment or if the condition worsens, you should consult a doctor.

Side effects

  • on the part of the skin: very rarely - itchy skin, rash, urticaria, contact dermatitis;
  • on the part of the immune system: the frequency has not been established - against the background of prolonged therapy, the development of anaphylactic reactions;
  • from the respiratory system, chest and mediastinal organs: very rarely - bronchospasm.

Overdose

Overdose symptoms have not been established.

Treatment: in case of accidental ingestion of Detragel, you should immediately consult a doctor. To neutralize the action of heparin, it is recommended to use a solution of protamine sulfate.

special instructions

The recommended dosage regimen should not be violated.

If symptoms of side effects develop, you should stop the procedure and consult a doctor.

It is necessary to avoid accidental contact of the gel in the eyes; after each procedure, it is recommended to thoroughly wash your hands with soap.

In case of deep vein thrombosis, the use of the gel is not recommended.

Influence on the ability to drive vehicles and complex mechanisms

The negative impact of Detragel on the patient's ability to drive vehicles and work with complex mechanisms has not been established.

Application during pregnancy and lactation

There are no data on undesirable effects when using Detragel for the treatment of pregnant women in relation to the fetus or mother. Despite this, use during gestation and breastfeeding is indicated only as directed by a doctor in special cases, when the expected therapeutic effect for the mother outweighs the potential threat to the fetus.

Pediatric use

Drug interactions

The use of Detragel on large areas of the skin for a long time against the background of concomitant therapy with indirect anticoagulants (including warfarin, acenocoumarol) must be accompanied by control of prothrombin time and blood coagulation time.

For the treatment of one area of the skin, you cannot practice the simultaneous application of several drugs for external use.
