Pharmacological properties

Pharmacodynamics

Guttalax Express is a laxative. Its active substance, bisacodyl, belongs to the group of diphenylmethane derivatives and has a local laxative effect with a slight antiresorptive effect. The mechanism of action is due to direct stimulation of nerve endings in the mucous membrane of the large intestine. Bisacodyl is hydrolyzed in the large intestine, contributing to the stimulation of its peristalsis, the accumulation of water and electrolytes in the lumen of the large intestine, and softening of the stool. This activates the process of emptying the intestines, reducing the time for evacuation of its contents.

The activity of bisacodyl at the level of the colon stimulates the natural evacuation process in the lower gastrointestinal tract (GIT). The drug has no effect on the absorption and digestion of high-calorie foods or essential nutrients in the small intestine.

The laxative effect develops within 10-30 minutes, in some cases a little longer.

Pharmacokinetics

After the administration of the suppository Guttalax Express, the absorption of bisacodyl is insignificant.

It is metabolized by enzymes (esterases) of the mucous membrane of the large intestine with the formation of an active metabolite - bis- (p-hydroxyphenyl) -pyridyl-2-methane. The maximum concentration (C _max) of the active metabolite in plasma is achieved in 0.5–3 hours. The laxative effect does not depend on the concentration level of the active metabolite in plasma.

Biotransformation of bisacodyl to inactive glucuronide occurs almost completely in the intestinal wall and liver.

The substance does not pass into breast milk.

The half-life of bisacodyl is approximately 16.5 hours. Most of it is excreted in the form of an active metabolite: almost completely (up to 90% of the total amount) through the intestine; up to 3.1% is excreted in the urine. Unchanged, some bisacodyl is excreted through the intestines.

Chronic abuse of laxatives contributes to damage to the renal tubules, the development of metabolic alkalosis, muscle weakness due to hypokalemia.
