Gynoflor E is a combined drug for regulating the balance of vaginal microflora.

Release form and composition

Dosage form of the drug - vaginal tablets: oval, biconvex, from light beige to white, inclusions are allowed (6 pcs. In blisters; 1 or 2 packs in a cardboard box).

Composition for 1 tablet:

  • active ingredients: acidophilic lactobacilli (in the form of lyophilisate) ≥ 100 million viable bacteria - 50,000 μg; estriol - 30 mcg;
  • auxiliary ingredients: lactose monohydrate, sodium carboxymethyl starch, magnesium stearate, microcrystalline cellulose, sodium dihydrogen phosphate.

The composition of Gynoflor E contains as active ingredients: a short-acting female sex hormone of the estrogen class - estriol; live bacteria that dominate the vagina of a healthy woman, possessing antagonistic activity against a wide range of pathogenic and opportunistic microflora, supporting and normalizing the vaginal biocenosis - lactobacillus acidophilus.

Estriol has a protective trophic effect on the vaginal epithelium and does not affect the woman's body systemically. The mature proliferating epithelial layer of the vagina accumulates glycogen, which is necessary for the functioning of lactobacilli, which produce lactic acid to maintain the acidic environment of the vagina (pH 3.8-4.5), as well as create unfavorable conditions for the development and colonization of pathogenic microflora. Together with lactic acid, acidophilic lactobacilli produce bacteriocins and hydrogen peroxide, which also act depressingly on the development of pathogenic and conditionally pathogenic microflora.

Modification of the normal physiological environment of the vagina can occur due to the use of antibacterial agents of local or systemic action for the treatment of vaginal infections or systemic diseases, as well as if personal hygiene is not followed. In such cases, a significant decrease in the number of natural lactobacilli is likely. With intravaginal application, Gynoflor E normalizes the state of the vaginal epithelium and restores its natural microflora. Contained as a filler in vaginal tablets, lactose can be fermented by lactobacilli into lactic acid.

In case of hormonal disorders, the epithelial layer of the vagina can become thinner and damaged, therefore Gynoflor E is also recommended for use in postmenopausal women to relieve its local symptoms: itching, burning, dryness in the vagina, pain during intercourse. The drug helps to normalize the state of the epithelium of the vagina and urethra, reduces the frequency of degenerative inflammatory processes.

In vitro studies have shown that acidophilic lactobacilli reduce the pH to normal within a few hours. Estriol, when treated with a course of 6–12 days, provides a proliferative trophic effect on the epithelial layer of the vagina.

Pharmacokinetics

When the Gynoflor E tablet is introduced into the vagina, lyophilized acidophilic lactobacilli and estriol are released.

The study of the absorption of estriol from Gynoflor E was carried out with the use of the drug by women in the postmenopausal period. The plasma concentration of estriol after repeated use corresponds to the level of endogenous unbound estriol. After 12 days of treatment with vaginal tablets Gynoflor E, provided that they are used once a day, the maximum concentration (Cmax) in the blood plasma of unbound estriol does not differ from the initial one, which is an indicator of the absence of systemic absorption.

The use of the drug does not affect the plasma concentration of the sex hormones present in the woman's body - estrone and estradiol, since estriol is the end product of their metabolism.

Indications for use

According to the instructions, Gynoflor E is recommended for use to restore normal vaginal microflora after the use of antibiotics and other antimicrobial drugs for local / systemic treatment of vaginal infections (bacterial vaginosis, non-specific vulvovaginitis), urogenital infections and sexually transmitted diseases. In the postmenopausal period, the drug is used in combination with hormone replacement therapy (HRT) for estrogen-dependent atrophic vaginitis.

Contraindications

  • vaginal bleeding of unknown etiology;
  • malignant estrogen-dependent tumors, including neoplasms of the mammary gland, ovaries, uterus or vagina (in history, diagnosed, as well as if they are suspected);
  • endometriosis (suspected / diagnosed);
  • untreated endometrial hyperplasia;
  • increased individual sensitivity to any component of the drug.

Gynoflor E is contraindicated for use before the onset of sexual activity.

Instructions for the use of Gynoflor E: method and dosage

The tablets are intended for intravaginal (deep) administration. The procedure is recommended to be carried out in the evening, before going to bed, preferably in the supine position, with the legs slightly bent at the knees.

Dosing regimen according to indications:

  • restoration of the normal microflora of the vagina: inject 1-2 tablets daily, the course is 6-12 days;
  • treatment of estrogen-dependent atrophic vaginitis in the postmenopausal period: inject 1 tablet daily, course 6-12 days; maintenance dose - 1 tablet 1-2 times a week.

Side effects

When using Gynoflora E in the vagina, a burning sensation is possible, in rare cases - redness of the vulva and vagina and vulvovaginal itching.

The composition of the Gynoflor E preparation contains components that cannot dissolve without a trace, therefore, sometimes you can find the remnants of a vaginal tablet on your underwear. This fact does not affect the effectiveness of the drug.

In exceptional cases, with excessive vaginal dryness, the tablet may not dissolve. To prevent such a situation, it can be moistened with a small amount of plain water before introduction - this will facilitate faster dissolution. During therapy, patients are advised to use sanitary pads.

Drug interactions

The simultaneous use of Gynoflora E with antibacterial drugs is not recommended, since acidophilic lactobacilli are sensitive to most antibacterial drugs for local or systemic therapy. Their simultaneous use can significantly reduce the effectiveness of treatment.

It is also not recommended to use Gynoflor E simultaneously with spermicidal agents.
