Herperax is an antiviral drug that is active against cytomegalovirus, Herpes simplex viruses (herpes simplex) types 1 and 2, Epstein Barr virus and Varicella zoster virus (Varicella zoster).

Release form and composition

  • tablets: round, flat, with beveled edges, pinkish-brown in color, with a dividing line on one side (10 pcs. in aluminum or PVC blisters, 3 blisters in a cardboard box);
  • ointment for external use 5%: white, homogeneous, without any inclusions (5 g each in aluminum tubes, 1 tube in a cardboard box).

Composition of tablets:

  • active substance: acyclovir - 200 mg in 1 tablet;
  • additional components: purified talc, sodium lauryl sulfate, sodium carboxymethyl starch, povidone (polyvinylpyrrolidone), colloidal silicon dioxide, corn starch, magnesium stearate, microcrystalline cellulose, iron oxide red (dye).

The composition of the ointment:

  • active substance: acyclovir - 50 mg in 1 g;
  • additional components: macrogol 400, macrogol 1540, macrogol 4000.

Indications for use

Pills

  • treatment of infections of the mucous membranes and skin caused by herpesvirus types I and II (primary and secondary), including genital herpes;
  • prevention of exacerbations of recurrent infections caused by herpesvirus types I and II in patients with normal immune status;
  • prevention of primary and recurrent infections caused by herpesvirus I and II types in patients with immunodeficiency;
  • treatment of primary and recurrent infections caused by varicella zoster virus (chickenpox), including shingles caused by the herpes zoster virus (Herpes zoster);
  • complex therapy of severe immunodeficiency in patients with HIV infection (including the stage of AIDS, early clinical signs and a detailed clinical picture) and in patients who have undergone bone marrow transplantation.

Ointment for external use

  • skin infections caused by herpesvirus types I and II;
  • genital herpes;
  • chicken pox;
  • shingles.

Contraindications

  • lactation period - for tablets;
  • children under 3 years old;
  • hypersensitivity to any component of the drug or to ganciclovir.

Carefully:

  • renal failure;
  • dehydration;
  • pregnancy;
  • neurological disorders, including in the anamnesis - for tablets;
  • old age - for pills;

Method of administration and dosage

Pills

In tablet form, Herperax is indicated for oral administration: the tablets should be taken during or immediately after a meal with plenty of water.

The doctor determines the required dose individually, depending on the severity of the disease.

For infections of the mucous membranes and skin caused by herpesvirus types I and II, adults and children from 3 years of age are prescribed 200 mg 5 times a day at 4-hour intervals (night break - 8 hours) for 5 days. In severe cases of the disease, the duration of treatment is increased at the discretion of the attending physician.

With severe immunodeficiency, including with concomitant HIV infection, and after bone marrow implantation, Herperax is prescribed 400 mg 5 times a day as part of complex therapy.

For the prevention and treatment of recurrent infections caused by herpesvirus, patients with normal immune status are prescribed 200 mg 4 times a day at 6-hour intervals.

For the prevention of infections caused by herpesvirus, adults and children from 3 years of age with immunodeficiencies are prescribed 200 mg 4 times a day (at intervals of 6 hours). Depending on the severity of the infection, the dose may be increased, but not more than up to 400 mg 5 times a day.

For infections caused by Varicella zoster, the dose for adults is 800 mg 5 times a day (with 4-hour intervals during the day and 8-hour intervals at night) for a course of 7-10 days, for children from 3 years old with a body weight of up to 40 kg - 20 mg / kg 4 times a day for 5 days. For children weighing more than 40 kg, the drug is prescribed in a dose for adults.

For infections caused by Herpes zoster, adults are prescribed 800 mg 4 times a day (at 6-hour intervals) for 5 days.

Recommended doses for patients with impaired renal function, depending on creatinine clearance (CC):

  • treatment and prevention of infections caused by Herpes simplex: with CC less than 10 ml / min - 200 mg 2 times a day at intervals of 12 hours;
  • Treatment of infections caused by Varicella zoster: with CC less than 10 ml / min - 800 mg 2 times a day at intervals of 12 hours, CC 10-25 ml / min - 800 mg 3 times a day at intervals of 8 hours.

Ointment for external use

In the form of an ointment, Herperax should be applied externally: apply a thin layer (with clean hands or with a cotton swab) to the affected and adjacent skin areas 5 times a day at intervals of 4 hours.

Treatment should be continued until the blisters are completely healed or a crust forms on them.

The average duration of treatment is 5 days, the maximum is 10 days.

Side effects

Herperax is well tolerated. Side effects are rare.

When taking the drug in tablet form, the following undesirable reactions may occur:

  • gastrointestinal tract: diarrhea, nausea, abdominal pain, vomiting;
  • nervous system: weakness, confusion, visual impairment, agitation, tremor, headache, myalgia, drowsiness, dizziness, paresthesia, hallucinations, increased fatigue;
  • laboratory parameters: a slight transient increase in the activity of liver enzymes, leukopenia, hyperbilirubinemia, erythropenia, a slight increase in the levels of urea and creatinine;
  • allergic reactions: rash, itching, urticaria, anaphylactic reactions, Lyell's syndrome, exudative erythema multiforme, including Stevens-Johnson syndrome;
  • others: lymphadenopathy, fever, alopecia, peripheral edema.

When treating with Herperax ointment, the following are possible: burning and inflammation (when it gets on the mucous membranes), dryness, hyperemia, peeling of the skin, allergic dermatitis.

special instructions

When treating genital herpes, the patient should be informed that acyclovir does not prevent sexual transmission of the virus, therefore, until the end of the course of therapy, it is recommended to refrain from sexual intercourse (or use condoms), even if there are no clinical signs of the disease.

Pills

With prolonged or repeated treatment with the drug in patients with reduced immunity, it is possible that viral strains that are insensitive to the action of acyclovir may appear.

In order to avoid the development of complications, Herperax should be used strictly according to the doctor's prescription, in compliance with all his recommendations.

Adequate fluid intake must be ensured during treatment.

Renal function (plasma creatinine and blood urea) should be monitored periodically.

Ointment for external use

The drug is not recommended to be applied to the mucous membranes of the eyes and mouth, since severe local inflammation may develop.

To achieve the maximum therapeutic effect, treatment should be started as early as possible after the first signs of infection appear: tingling, itching, burning, redness, feeling of tension.

Drug interactions

With oral administration of Herperax simultaneously with probenecid, the average half-life increases and the clearance of acyclovir decreases.

Used in combination, nephrotoxic drugs increase the risk of renal impairment.

With external use of Herperax, no interaction with other drugs has been identified.
