Release form and composition
The drug is produced in the form of a cream for external use, white or white with a yellowish tinge (5 or 10 g each in an aluminum tube with a bushon or in a laminate tube made of a combined material with a bushon, with a first opening control; in a cardboard box 1 tube and instructions for use Vartocide).
1 g of cream contains:
- active substance: imichimod - 50 mg;
- additional components: propyl parahydroxybenzoate, phenoxyethanol, oleic acid, methyl parahydroxybenzoate, diethylene glycol monoethyl ether, trolamine, carbomer (carbomer copolymer), polysorbate 20, carbomer (carbomer interpolymer), purified water.

Pharmacodynamics
Imichimod, the active ingredient of Vartocid, is an immunomodulatory agent, an immune response modifier that does not exhibit direct antiviral activity when applied externally. The effect of the drug is associated with the induction of interferon alpha, tumor necrosis factor (TNF) and cytokines from other families. When applying the cream to problem areas, there is a decrease in the number of angiogenesis processes as a result of increased production of interleukin-18 (IL-18) and interferon-gamma.
Pharmacokinetics
After applying a single dose of the drug through the skin, less than 0.9% of imiquimod labeled with a radioactive isotope is absorbed. An insignificant amount of the active substance that has reached the systemic circulation is excreted without delay from the body with urine and with feces in approximately a ratio of 3 ÷ 1.
After repeated or single external application, the serum concentration of imiquimod in the blood did not reach a level that could be quantified (more than 5 ng / ml).
Indications for use
Vartocid cream is recommended for use in adults for the treatment of external genital warts located on the external genitalia and / or in the perianal zone.
Contraindications
- age up to 18 years;
- pregnancy and lactation;
- hypersensitivity to any component of the product.
Vartocid, instructions for use: method and dosage
Vartocid cream is used externally.
It is advisable to apply the drug before bedtime, leaving it on the skin overnight (at least 6-10 hours). After this period, the cream must be washed off with warm water using soap with a neutral pH level. Apply the cream only to previously cleansed affected areas of the skin with a thin layer and then rub gently until completely absorbed. When applying the product, you can use the nose of the tube as an applicator; it is not recommended to apply bandages to the treated skin area.
A single (daily) dose of the cream is 10 mg per 1 cm² (a strip of the drug 0.5 cm long and 2 mm in diameter). The maximum area of the treated surface is 25 cm², which is proportional to the maximum daily dose of the product - 250 mg. It is necessary to apply Vartocid once a day, every other day, until the genital warts are completely removed, but the duration of the course of therapy should not exceed 16 weeks.
If the patient has forgotten to treat the affected surface according to the schedule, the missed dose should be applied as soon as possible, and then the cream should be used as usual.
You should not use Vartocid more than 1 time per day.
Side effects
- genitourinary system: infrequently - pain in the vagina, pain in the penis, painful intercourse, vaginitis, vulvitis, atrophic vaginitis, herpes simplex, bacterial / fungal infection;
- local skin reactions: very often - itching and pain at the site of application of the cream; often - edema, excoriation / desquamation, erosion, infection, erythema; infrequently - urticaria, pruritus, folliculitis, dermatitis, eczema, erythematous rash; rarely, local hypopigmentation / hyperpigmentation, scabbing and blistering, induration, ulceration;
- hematopoietic system: very rarely - a decrease in hemoglobin levels, leukopenia, thrombocytopenia, absolute neutropenia;
- musculoskeletal system: infrequently - back pain, arthralgia;
- nervous system: often - headache, dizziness; infrequently - irritability, ringing in the ears, drowsiness / insomnia, paresthesia, depression, migraine;
- respiratory system: infrequently - rhinitis, pharyngitis;
- digestive system: often - nausea; infrequently - vomiting, diarrhea, abdominal pain, pain in the anus, anorexia, rectal lesions;
- others: often - fatigue, flu-like symptoms, fever, myalgia, infection; infrequently - increased sweating, lymphadenopathy.
In case of development of local undesirable phenomena, it is necessary to remove the cream by washing it off with soap and water. You can resume the use of Vartocid after the skin reaction has been eliminated.

Overdose
Overdose of Vartocid is unlikely due to its low absorption.
In case of accidental oral administration of imiquimod at a dose of 200 mg, which is equivalent to 4 g of cream, symptoms such as headache, nausea, vomiting, fever, myalgia may appear. The most severe adverse reaction after oral administration of several doses of 200 mg is a decrease in blood pressure. In this condition, it is necessary to immediately consult a doctor and carry out symptomatic treatment.

It is necessary to avoid contact of the cream with the eyes, if this happens, it is required to rinse the eyes with plenty of water.
In the treatment of genital warts localized under the foreskin, men who have not undergone circumcision should wash the affected area every day, having previously pulled the foreskin. If early signs of phimosis are detected, it is recommended to urgently cancel treatment with Vartocid.
Before intercourse, it is required to wash off the cream from the skin.
The areas of the skin to which Vartocid is applied must not be exposed to ultraviolet radiation (UV radiation), both natural (solar) and artificial, until the treatment is complete.
The cream applied to the affected area should be left on for no more than 10 hours.
Do not allow the use of excessive doses of the drug, this can lead to an increased risk of severe local reactions.
Vartocid can provoke an aggravation of the course of inflammatory skin diseases.
It is not recommended to use an occlusive dressing during the treatment period.
The cream should be applied only to problem areas, avoiding contact with healthy skin and mucous membranes. In the case of applying Vartocid to the latter, they must be washed immediately with warm water and soap.
Do not carry out drug therapy until the skin is completely healed after other types of medication / surgical treatment. Also, do not treat skin areas with ulcers / erosions and open wounds before scarring.
In the treatment of genital warts located in the urethra, cervix, vagina, mucous membrane of the anus and rectum, the efficacy and safety of Vartocid has not been studied. It is not recommended to use the cream with these localizations.
When the drug is applied to the skin, it is necessary to avoid any sexual contact - sexual, anal or oral sex.
Since the cream can reduce the tightness of the vaginal diaphragms and condoms, during the treatment period, as well as for 1 month after its completion, in order to prevent the development of pregnancy, it is recommended to use other methods of contraception.
Due to the fact that imiquimod does not show a direct antiviral and cytotoxic effect, after the course, the development of new warts of the perianal and genital areas can be observed. In some cases, even during the period of therapy, new genital warts may appear in other areas of the skin. In this situation, further treatment tactics should be determined by the doctor.
Do not re-use Vartocid in immunocompromised patients.

Vartocid is contraindicated for use during pregnancy and breastfeeding.
Pediatric use

Drug interactions
The study of the interaction of imiquimod with other drugs / agents, including immunosuppressants, has not been performed.
The minimal absorption of Vartocid through the skin limits its interaction with drugs that have a systemic effect.
