Valtsikon is an antiviral agent.

Release form and composition

Valtsikon is produced in the form of film-coated tablets: round, convex on both sides, almost white or white (5 or 14 pieces in a contoured cell package, in a cardboard box 2 or 3 packages, respectively).

Active ingredient: valacyclovir (in the form of hydrochloride), 1 tablet contains 500 mg.

Additional components:

  • excipients: hyprolose (hydroxypropyl cellulose), magnesium stearate, crospovidone, microcrystalline cellulose;
  • film shell: hypromellose, titanium dioxide, talc and hyprolose (hydroxypropyl cellulose) or dry mixture for film coating (hypromellose, talc, hydroxypropyl cellulose, titanium dioxide).

Indications for use

For adults:

  • treatment of infections of the skin and mucous membranes caused by the herpes simplex virus (Herpes simplex types 1 and 2), including herpes labialis, newly diagnosed and recurrent genital herpes;
  • treatment of herpes zoster (Herpes zoster) (relief of pain syndrome and reduction of its duration, including acute and postherpetic neuralgia);
  • prevention of transmission of the genital herpes virus to a healthy partner (the drug is used as suppressive therapy in combination with safe sex);
  • prevention (suppression) of recurrent infections of the skin and mucous membranes caused by the Herpes simplex virus (types 1 and 2), including genital herpes.

For adults and children over 12 years old:

  • prevention of acute graft rejection in patients after kidney transplantation;
  • prevention of cytomegalovirus, opportunistic and other herpesvirus infections after organ transplantation.

Contraindications

  • human immunodeficiency virus (HIV) in case of CD4 + lymphocyte count less than 100 in 1 μl;
  • children under 12 years of age - in the prevention of cytomegalovirus infection after transplantation, up to 18 years - in the presence of other indications;
  • hypersensitivity to the components of the drug or acyclovir.

Carefully:

  • pregnancy and lactation;
  • elderly age;
  • simultaneous use of nephrotoxic drugs;
  • hypohydration;
  • renal / hepatic impairment;
  • clinically expressed forms of HIV infection.

Method of administration and dosage

Valtsikon should be taken orally.

Recommended doses:

  • herpes zoster: 1000 mg 3 times a day for 7 days;
  • infections caused by Herpes simplex: episode therapy - 500 mg 2 times a day for a course of 5 days, in severe cases in case of onset of the disease, the drug should be taken as early as possible, the duration of treatment, if necessary, is increased to 10 days. The duration of therapy for relapses is 3 or 5 days, it is recommended to start taking the drug in the prodromal period or immediately after the first signs of the disease appear. With labial herpes, as an alternative treatment, it is possible to take Valtsikon at a dose of 2000 mg 2 times a day (with an interval of 6-12 hours), the drug should be started when early symptoms of the disease appear (itching, burning, tingling), the course is 1 day;
  • prevention of relapse of infections caused by the herpes simplex virus: 500 mg once a day, in the presence of concomitant immunodeficiency - 500 mg 2 times a day;
  • prevention of the transmission of genital herpes to a healthy partner in immunocompetent patients with relapses no more than 9 times a year: 500 mg once a day daily for 1 year, if necessary - more;
  • prevention of infections after transplantation, including in children over 12 years old: 2000 mg 4 times a day, the drug should be started as soon as possible after transplantation. The duration of treatment is 90 days, but in high-risk patients, therapy can be extended.

Patients with impaired renal function

Recommended dosages for the treatment of herpes zoster and herpes simplex virus infections and for the prevention of recurrence of Herpes simplex infection and transmission of genital herpes to a healthy partner:

  • shingles (Herpes zoster): CC 15-30 ml / minute - 1000 mg 2 times a day, CC less than 15 - 1000 mg once a day;
  • treatment of infections caused by the Herpes simplex virus (according to the scheme 500 mg 2 times a day): CC less than 15 - 500 mg 1 time a day;
  • treatment of labial herpes (according to the scheme 2000 mg 2 times a day for one day): CC 31-49 - 1000 mg 2 times a day, CC 15-30 - 500 mg 2 times a day, CC less than 15 - 500 mg once a day;
  • prevention (suppression) of recurrent infections caused by the herpes simplex virus: CC less than 15 in patients with normal immunity - 250 mg once a day, in patients with reduced immunity - 500 mg once a day;
  • reducing the risk of transmission of genital herpes to a partner: CC less than 15 - 250 mg 1 time per day.

Patients on hemodialysis should use the drug immediately after the end of the hemodialysis session in the same dose as for patients with CC less than 15 ml / minute.

There is no experience of using Valtsikon in children with a CC value of less than 50 ml / minute / 1.73 m 2.

Recommended doses for the prevention of cytomegalovirus infection after transplantation:

  • CC ≥ 75 - 2000 mg 4 times a day;
  • CC 50-74 ml / minute - 1500 mg 4 times a day;
  • CC 25–49 ml / minute - 1500 mg 3 times a day;
  • CC 10-24 ml / minute - 1500 mg 2 times a day;
  • CC <10 ml / minute or dialysis * - 1500 mg once a day.

* Patients who are on hemodialysis should take the drug after the end of the session.

Side effects

  • dermatological and allergic reactions: skin rashes, alopecia, photosensitivity, urticaria, pruritus, erythema multiforme, anaphylaxis, angioedema;
  • on the part of the hematopoietic system: anemia, thrombocytopenia, thrombotic thrombocytopenic purpura, leukopenia (mainly occurs in patients with reduced immunity);
  • from the respiratory system: dyspnea;
  • from the urinary system: pain in the projection of the kidneys, renal colic, acute renal failure;
  • from the digestive system: a feeling of abdominal discomfort, nausea, vomiting, diarrhea, reversible violations of liver function tests (increased activity of alkaline phosphatase, alanine aminotransferase and aspartate aminotransferase);
  • from the central nervous system: confusion or depression of consciousness, agitation, dizziness, dysarthria, mental decline, hallucinations, headache, mania, tremor, ataxia, psychotic symptoms, convulsions, encephalopathy, coma;
  • on the part of the cardiovascular system: increased blood pressure, tachycardia;
  • laboratory parameters: hypercreatininemia, decreased hemoglobin;
  • others: visual impairment, fatigue, nasopharyngitis, respiratory tract infections, dysmenorrhea.

In patients with severely impaired immunity, especially in adults with advanced stage of HIV infection, who receive Valcicon in a large dose (8000 mg / day) for a long time, there have been cases of thrombocytopenia, microangiopathic hemolytic anemia and renal failure.

In case of an overdose, the following complications may develop: acute renal failure, nausea, vomiting, and neurological symptoms, including hallucinations, agitation, confusion, depression of consciousness and even coma.

special instructions

Suppressive drug therapy reduces the risk of transmission of genital herpes, but does not eliminate the risk of infection and does not lead to a complete cure. Treatment with Valcycone is recommended to be combined with safe sex.

The use of the drug in high doses for a long period in conditions accompanied by severe immunodeficiency (for example, with a clinically pronounced course of HIV, with bone marrow and kidney transplantation), is fraught with the development of hemolytic uremic syndrome and thrombocytopenic purpura, which can be fatal.

During treatment, it is necessary to regularly determine the level of CC, especially during a period when renal function is changing rapidly, for example, after a transplant operation and during the graft engraftment period. The dose must be adjusted in accordance with the creatinine clearance.

Patients at risk of developing dehydration, especially the elderly, should ensure adequate fluid intake during therapy.

To avoid overdose, the dosage regimen prescribed by the doctor should be strictly observed. Many cases of overdose are associated with exceeding the dose of Valcycone when treating the elderly and patients with impaired renal function.

There are no data on the negative effect of the drug on the ability to drive a car and work with complex mechanisms. However, given the potential for central nervous system side effects, caution is advised when engaging in potentially hazardous activities.

Drug interactions

  • acyclovir: its concentration in blood plasma increases;
  • drugs that compete with acyclovir for the elimination pathway: a plasma increase in the level of one or both drugs or their metabolites is possible;
  • nephrotoxic drugs: the likelihood of developing renal dysfunction increases (kidney function must be monitored);
  • tubular secretion blockers, cimetidine: the effectiveness of valacyclovir decreases (but dose adjustment is not required in patients with normal CC levels).

With the simultaneous use of thiazide diuretics, digoxin, aluminum- or magnesium-containing antacids, the pharmacokinetics of valacyclovir does not change.
