Bicillin-3 is a drug with antibacterial, bactericidal action.

Release form and composition

The dosage form is a powder for preparing a suspension for intramuscular administration: white with a slightly yellowish tinge or white, prone to clumping, when water is added, forms a stable suspension (in 10 ml vials, in a cardboard box of 1, 10 or 50 vials).

Active substances in 1 bottle of 600 or 1200 thousand units:

  • benzathine benzylpenicillin - 200 or 400 thousand units (units of action);
  • benzylpenicillin sodium salt or benzylpenicillin potassium salt (benzylpenicillin sodium or benzylpenicillin potassium) - 200 or 400 thousand units;
  • novocaine benzylpenicillin salt (procaine benzylpenicillin) - 200 or 400 thousand units.

Pharmacological properties

Pharmacodynamics

Bicillin-3 is one of the combined antibacterial drugs of the long-acting penicillin group, which are destroyed by penicillinase. The mechanism of action is based on a violation of the synthesis of peptidoglycan - a mucopeptide of the cell wall, which causes inhibition of the synthesis of the cell wall of microorganisms, suppression of the reproduction and growth of bacteria.

According to the instructions, Bicillin-3 is active against gram-positive and gram-negative bacteria: Staphylococcus spp. (not producing penicillinase), Streptococcus spp. (including Streptococcus pneumoniae), Neisseria gonorrhoeae, Actinomyces israelii, Neisseria meningitidis, Corynebacterium diphtheriae, Bacillus anthracis as well as Treponema spp., Leishmania, anaerobic spore-forming rods.

Penicillinase-producing Staphylococcus spp. Are resistant to the action of the drug.

Pharmacokinetics

Absorption: after a single application, the active substances remain at an average therapeutic concentration for 6-7 days. The maximum plasma concentration is reached 12-24 hours after administration. Concentration in blood serum on the 14th day after administration of 2400 thousand units is 0.12 μg / ml; after a dose of 1200 thousand units on the 21st day, the serum concentration in the blood is 0.06 μg / ml.

Distribution and metabolism: after intramuscular administration, Bicillin-3 is slowly hydrolyzed with the release of benzylpenicillin. Biotransformation is insignificant. Plasma protein binding is in the range from 40 to 60%. Penetrates into breast milk, passes through the placental barrier. It has a low tissue distribution, while being well distributed in liquids.

Excretion: mainly excreted by the kidneys.

Indications for use

  • infectious diseases caused by pathogens sensitive to the action of penicillin (especially in cases where it is necessary to create a long-term therapeutic concentration in the blood);
  • streptococcal infections, including acute tonsillitis, wound infections, scarlet fever, erysipelas, except for infections caused by group B streptococci;
  • syphilis;
  • leishmaniasis;
  • yaws;
  • rheumatism (prophylactic).

Contraindications

Contraindication to use is individual intolerance to the components of the drug, including procaine (novocaine).

Bitsillin-3 is used with caution in renal failure, diseases of allergic etiology, including bronchial asthma, hay fever (including a history), pseudomembranous colitis.

Instructions for the use of Bitsillin-3: method and dosage

The drug should be injected deep into the upper outer quadrant of the buttock. Intravenous administration is prohibited.

The maximum dose is 1200 thousand units. If two injections are necessary, injections should be made in different buttocks. The drug can be re-administered after 4 days.

Bicillin-3 at a dose of 600 thousand units is administered 1 time in 6 days.

The use of the drug in the treatment of syphilis:

  • primary and secondary: single dose - 1800 thousand units; the full course includes 7 injections: the first - 300 thousand units, the second is injected after 24 hours (the full dose is 1800 thousand units), then the drug is used 2 times a week;
  • secondary recurrent and latent early: the full course includes 14 injections: the first - 300 thousand units, then the drug is used 2 times a week at a full dose of 1800 thousand units.

It is necessary to prepare the suspension immediately before use. 2-3 ml of isotonic sodium chloride solution or sterile water for injection should be injected into the vial with the drug. Within 30 seconds the mixture is shaken (in the direction of the longitudinal axis of the vial) until a homogeneous suspension / slurry is formed. It is not recommended to rub the injection site after injection.

Side effects

  • blood coagulation system: anemia, leukopenia, thrombocytopenia, hypocoagulation;
  • digestive system: glossitis, stomatitis;
  • allergic reactions: urticaria, erythema multiforme, angioedema, exfoliative edema, anaphylactic shock, fever, arthralgia;
  • others: with a long course - mycoses, superinfection.

Overdose

To date, there are no reports of overdose.

In cases of allergic reactions, therapy is immediately canceled.

When anaphylactic shock occurs, anti-shock therapy is prescribed (norepinephrine, glucocorticosteroids, artificial ventilation).

Endolumbar / intravenous administration is not permissible, since this can lead to the development of Wanier's syndrome (manifested in the form of depression, paresthesia, anxiety, visual impairment).

In the treatment of sexually transmitted diseases, in cases of suspicion of syphilis, serological / microscopic examinations must be performed before starting treatment and then for 4 months.

It should be borne in mind that the use of insufficient doses of Bicillin-3 or early termination of therapy can lead to the emergence of resistant strains of pathogens.

Due to the likelihood of developing fungal infections, it is advisable to use antifungal drugs (nystatin, levorin), as well as B vitamins and ascorbic acid.

Application during pregnancy and lactation

There is no information on the use of Bitsillin-3 during pregnancy / lactation.

Drug interactions

With the combined use of certain drugs / substances, the following effects may develop:

  • allopurinol, diuretics, phenylbutazone, nonsteroidal anti-inflammatory drugs: decreased tubular secretion and increased concentration of penicillin;
  • bacteriostatic antibiotics (including tetracyclines, macrolides, lincosamides, chloramphenicol): antagonism of action;
  • bactericidal antibiotics (including rifampicin, cephalosporins, vancomycin, cycloserine, aminoglycosides): synergistic action;
  • oral contraceptives and ethinyl estradiol: decrease in their effectiveness and increase the likelihood of breakthrough bleeding;
  • allopurinol: an increase in the likelihood of the risk of allergic reactions.
