Amoxiclav Kviktab is a broad-spectrum semi-synthetic antibiotic of the penicillin group in combination with β-lactamase inhibitors.
Release form and composition
The drug is produced in the form of dispersible tablets:
- dosages of 500 + 125 mg and 875 + 125 mg: octagonal tablets, oblong, light yellow color with brown splashes, have a pleasant fruity aroma (2 pcs. in blisters, in a carton of 5 or 7 blisters);
- dosages 125 + 31.3 and 250 + 62.5: round tablets, hemispherical depressions on both sides, engraving "SN57" on one side, color from light yellow to yellow, with brown splashes (2 pcs. in blisters, in a pack of cardboard 5, 7 or 10 blisters).
Composition for 1 tablet for dosages of 500 + 125 mg and 875 + 125 mg:
- active ingredients: amoxicillin - 500/875 mg; clavulanic acid - 125 mg (in the form of amoxicillin trihydrate - 574 / 1004.5 mg and potassium clavulanate - 148.87 mg);
- auxiliary components: aspartame, talc, tropical mixture and sweet orange flavors, colloidal anhydrous silicon dioxide, yellow iron oxide (E172), hydrogenated castor oil, microcrystalline silicon cellulose.
Composition for 1 tablet for dosages 125 + 31.3 and 250 + 62.5:
- active ingredients: amoxicillin - 125/250 mg; clavulanic acid - 31.3 / 62.5 mg (in the form of amoxicillin trihydrate - 143.5 / 287 mg and potassium clavulanate - 37.2 / 74.4 mg);
- auxiliary components: silicon dioxide, crospovidone, sweet orange flavor, microcrystalline silicon-containing cellulose, sucralose (trichlorogalactosucrose), sodium stearyl fumarate, yellow iron oxide (E172), mannitol.
Pharmacological properties
Pharmacodynamics
The mechanism of action of Amoxiclav Kviktab is due to the combination of active components in its composition:
- amoxicillin is a broad-spectrum semi-synthetic antibiotic that is active against many both gram-positive and gram-negative microbes; due to its susceptibility to destruction by β-lactamases, the spectrum of activity of amoxicillin is limited by microorganisms that produce this enzyme;
- clavulanic acid is a β-lactamase inhibitor structurally related to penicillins, inactivating a wide range of β-lactamases resistant to cephalosporins and penicillins found in microorganisms; it is quite effective against plasmid β-lactamases, which most often cause bacterial resistance, but ineffective against type I chromosomal β-lactamases. The presence of clavulanic acid in the preparation protects amoxicillin from destruction by β-lactamases, allowing it to expand its antibacterial spectrum.
Amoxicillin in combination with clavulanic acid is active against the following pathogenic microflora:
- gram-positive aerobic microorganisms: Streptococcus pneumoniae, Streptococcus viridans, Streptococcus pyogenes, Enterococcus spp., Streptococcus bovis, Staphylococcus epidermidis (except methicillin-resistant strains), Staphylococcus Staphylococcus aureus-resistant strains (except methicillin-resistant strains), Staphylococcus aureticus-resistant strains;
- gram-negative aerobic microorganisms: Brucella spp., Bordetella pertussis, Campylobacter jejuni, Gardnerella vaginalis, Escherichia coli, Haemophilus ducreyi, Helicobacter pylori, Haemophilus influenzae, Moraxella methium. spp., Salmonella spp., Yersinia enterocolitica, Vibrio cholerae, Eikenella corrodens;
- gram-positive anaerobic microorganisms: Actinomyces israelii, Peptostreptococcus spp., Peptococcus spp., Clostridium spp., Prevotella spp., Fusobacterium spp.;
- gram-negative anaerobic microorganisms: Bacteroides spp.
Pharmacokinetics
The basic pharmacokinetic parameters of the active components of Amoxiclav Kviktab (amoxicillin and clavulanic acid) are similar:
- absorption: after oral administration, the drug is well absorbed from the gastrointestinal tract (GIT), the degree of absorption does not depend on food intake. Bioavailability of amoxicillin - 90%, clavulanic acid - 70%; the maximum plasma concentration (Cmax) is achieved 1 hour after administration and varies depending on the dose: amoxicillin - from 3 to 12 μg / ml; clavulanic acid - about 2 μg / ml. Half-life (T 1/2): amoxicillin - 78 minutes; clavulanic acid - 60-70 min;
- distribution: both active ingredients are well distributed in body fluids and tissues, penetrating into the synovial fluid, sinuses, middle ear, palatine tonsils, saliva, lungs, pleural fluid, bronchial secretions, ovaries, uterus, liver, muscle tissue, gallbladder, prostate gland, peritoneal fluid; high concentrations of the drug are recorded in the urine. With non-inflamed meninges, amoxicillin and clavulanic acid are not able to penetrate the blood-brain barrier (BBB). But they pass through the placental barrier and are excreted in breast milk in trace concentrations. The degree of binding of amoxiclav to plasma proteins is low;
- metabolism: amoxicillin undergoes partial metabolism, clavulanic acid is extensively metabolized;
- excretion: amoxicillin is eliminated by tubular secretion and glomerular filtration by the kidneys, practically unchanged; clavulanic acid - by glomerular filtration, some in the form of metabolites. Part of the substances can be excreted through the lungs and intestines. T 1/2 of Amoksiklava Kviktab in adult patients is about 1 hour.
In patients with severe renal impairment, T 1/2 increases to 7.5 hours for amoxicillin and up to 4.5 hours for clavulanic acid.
Both active substances are removed during hemodialysis and are excreted in small quantities during peritoneal dialysis.
Indications for use
According to the instructions, Amoxiclav Kviktab is recommended for the treatment of infectious and inflammatory diseases caused by microorganisms sensitive to the drug, the following localization:
- upper respiratory tract and ENT organs: acute / chronic sinusitis, acute / chronic otitis media, pharyngitis, pharyngeal abscess, tonsillitis, etc.;
- lower respiratory tract: chronic bronchitis, acute bronchitis with bacterial superinfection, pneumonia, etc.;
- urinary tract: urethritis, cystitis, pyelonephritis, etc.;
- skin and soft tissues (including human and animal bites);
- bone and connective tissue: osteomyelitis;
- biliary tract: cholecystitis, cholangitis;
- oral cavity (odontogenic lesions).
Amoxiclav Kviktab is also used in gynecology and obstetric practice.
Contraindications
Absolute:
- children under 12 years of age;
- body weight up to 40 kg;
- history of cholestatic jaundice and / or other abnormal liver function caused by taking amoxicillin / clavulanic acid;
- anamnestic data on hypersensitivity to any antibiotics from the group of penicillins, cephalosporins, to other β-lactam antibiotics;
- hypersensitivity to any component of the drug.
Amoxiclav Kviktab should be used with caution in case of a history of pseudomembranous colitis, liver failure, severe renal dysfunction and during breastfeeding.
Since many patients with infectious mononucleosis and lymphocytic leukemia experienced the appearance of an erythematous rash during ampicillin therapy, they are not recommended to prescribe antibiotics of the ampicillin group.
Instructions for use of Amoxiclav Kviktab: method and dosage
Tablets Amoksiklav Kviktab taken orally after dissolving in 1 / 2 cups water (not less than 30 mL) and mix thoroughly. You can keep the tablet in your mouth until it is completely dissolved, and then swallow it.
It is recommended to take the drug immediately before meals to reduce the risk of adverse reactions from the gastrointestinal tract.
Recommended dosage for adult patients and children over 12 years old (weighing more than 40 kg):
- mild or moderate infection: 1 tablet Amoxiclav Kviktab 500 + 125 mg every 12 hours;
- severe infection and respiratory tract damage: 1 tablet Amoxiclav Kviktab 875 + 125 mg every 12 hours or 1 tablet Amoxiclav Kviktab 500 + 125 mg every 8 hours.
The duration of the course of treatment is up to 2 weeks.
If therapy began with parenteral administration of amoxicillin and clavulanic acid, in the future it is possible to switch to taking Amoxiclav Kviktab tablets inside.
Side effects
Side effects due to therapy with Amoxiclav Kviktab on the part of systems and organs (mostly weak and transient):
- digestive system: loss of appetite, nausea / vomiting, diarrhea; rarely - an increase in the activity of liver enzymes: alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP) and impaired hepatic function; in isolated cases - hepatitis, cholestatic jaundice, pseudomembranous colitis;
- central nervous system (CNS): dizziness, headache; extremely rarely - convulsions (more often with impaired renal function in combination with taking high doses of the drug);
- hematopoietic system: rarely - reversible leukopenia (including neutropenia), thrombocytopenia; extremely rarely - hemolytic anemia, a reversible increase in prothrombin time (subject to joint use with anticoagulants);
- urinary system: extremely rare - crystalluria, interstitial nephritis;
- hypersensitivity reactions: itching, erythematous rash, urticaria; extremely rarely - angioedema, erythema multiforme exudative, anaphylactic shock, allergic vasculitis; in isolated cases - Stevens-Johnson syndrome, exfoliative dermatitis, toxic epidermal necrolysis (Lyell's syndrome), acute generalized exanthematous pustulosis;
- other reactions: rarely - the development of superinfection (including candidiasis).
Overdose
Data on life-threatening side effects or death due to an overdose of Amoxiclav Kviktab were not recorded.
Overdose symptoms are gastrointestinal disorders: abdominal pain, diarrhea / vomiting; insomnia, agitation, dizziness are also likely; in some episodes - seizures.
Treatment is prescribed symptomatic, having previously taken measures (with recent pills, no more than 4 hours) to reduce the absorption of the drug - gastric lavage and intake of activated carbon. Amoxicillin and clavulanic acid are removed by hemodialysis. The patient needs medical supervision.
With a decrease in blood pressure, the patient should take the Trendelenburg position - lie on his back, raise the pelvis in relation to the head at an angle of 45 °.
special instructions
In the course of course therapy, monitoring of the function of the liver, kidneys and hematopoietic organs is required.
In case of severe impairment of renal function, adequate dose adjustment of the drug or an increase in the intervals between doses is necessary.
During therapy with Amoxiclav Kviktab, a false-positive reaction is possible due to the use of Benedict's reagent or Felling's solution to determine the level of glucose in the urine, therefore it is recommended to carry out enzymatic reactions with glucosidase.
Influence on the ability to drive vehicles and complex mechanisms
Data on the negative effect of Amoxiclav Kviktab, taken in recommended doses, on the speed of psychophysical reactions and the ability to concentrate attention have not been reported.
Application during pregnancy and lactation
Amoxiclav Kviktab is allowed to be used during pregnancy and lactation (breastfeeding) in the presence of clear indications, when the intended benefit to the mother significantly outweighs the potential risks to the fetus / child.
Pediatric use
In pediatric practice, it is contraindicated to prescribe the drug for the treatment of children under the age of 12 years and / or with a body weight of up to 40 kg.
With impaired renal function
Dosage of Amoxiclav Kviktab is adjusted towards lowering the dose or increasing the interval between doses in accordance with the severity of renal dysfunction:
- moderate renal failure with creatinine clearance (CC) from 10 to 30 ml / min: 1 tablet (500 + 125 mg) every 12 hours;
- severe renal failure with CC less than 10 ml / min: 1 tablet (500 + 125 mg) every 24 hours.
With anuria, the interval between doses should be increased to 48 hours or more.
Patients with severely impaired renal function are advised to be careful when taking the drug.
For violations of liver function
Patients with hepatic impairment are advised to be careful when taking Amoxiclav Kviktab.
Drug interactions
- antacids, glucosamine, laxatives, aminoglycosides: slow down the absorption of Amoxiclav Kviktab;
- ascorbic acid: increases the absorption of amoxicillin and clavulanic acid;
- diuretics, allopurinol, phenylbutazone, non-steroidal anti-inflammatory drugs (NSAIDs), other drugs, blockers of tubular secretion: increase the concentration of amoxicillin, but do not affect the level of clavulanic acid, since it is excreted mainly by glomerular filtration;
- methotrexate: Amoxiclav Kviktab increases its toxicity;
- allopurinol: the drug increases the incidence of exanthema;
- disulfiram: it is required to avoid joint use with Amoxiclav Kviktab;
- anticoagulant drugs: since Amoxiclav Kviktab can, in some cases, lengthen the prothrombin time, caution should be exercised when using them simultaneously;
- rifampicin: is an amoxicillin antagonist with mutual weakening of antibacterial efficacy;
- bacteriostatic antibiotics (macrolides, tetracyclines), sulfonamides: Amoxiclav Kviktab should be used several hours before taking them;
- probenecid: inhibits the excretion of amoxicillin, increasing its serum concentration;
- oral contraceptives: Amoxiclav Kviktab reduces their effectiveness.
