Amlorus is a slow calcium channel blocker, a drug with antianginal and antihypertensive effects.

Release form and composition

Dosage form - tablets: flat-cylindrical, almost white or white (marbling is possible), with a bevel (10 tab. In blisters, in a cardboard box 3 packs).

Active ingredient: amlodipine (in the form of besylate), in 1 tablet - 2.5, 5 or 10 mg.

Auxiliary components: microcrystalline cellulose, calcium stearate, colloidal silicon dioxide (aerosil), crospovidone, lactose monohydrate, talc.

Indications for use

  • Arterial hypertension;
  • Prinzmetal angina (vasospastic) and stable exertional angina.

Amlorus is used as a monopreparation or as part of a combination therapy (in combination with antihypertensive (arterial hypertension) or antianginal (vasospastic and stable angina pectoris) drugs).

Contraindications

  • Age under 18;
  • Unstable angina (except for vasospastic angina);
  • Cardiogenic shock;
  • Severe arterial hypotension (systolic blood pressure less than 90 mm Hg);
  • Acute myocardial infarction (first 28 days);
  • Left ventricular outflow tract obstruction (including clinically significant aortic stenosis);
  • Glucose-galactose malabsorption, lactase deficiency, lactose intolerance;
  • Lactation period;
  • Hypersensitivity to drug components or other dihydropyridine derivatives.

Carefully:

  • Mitral / aortic stenosis;
  • Chronic heart failure of non-ischemic etiology of III-IV functional class according to NYHA classification;
  • Sick sinus syndrome (tachycardia, severe bradycardia);
  • Hypertrophic obstructive cardiomyopathy;
  • Arterial hypotension;
  • Liver dysfunction;
  • Elderly age;
  • Pregnancy (the drug is prescribed only in cases where the intended benefits outweigh the possible risks).

Method of administration and dosage

Amlorus should be taken orally with a sufficient amount of water, 1 time per day.

The initial dose for arterial hypertension and angina pectoris is 5 mg, if necessary, it is increased to 10 mg.

The maintenance daily dose for arterial hypertension is 5 mg, for Prinzmetal's angina pectoris - 5-10 mg per dose.

Elderly people and patients with hepatic impairment may need to reduce the initial dose for hypertension to 2.5 mg.

A reduction in the initial dose is required for patients with severe liver dysfunction, patients of short stature and people with low body weight. For such patients, Amlorus is prescribed 5 mg as an antianginal agent, and 2.5 mg as an antihypertensive agent.

With the simultaneous appointment of angiotensin-converting enzyme (ACE) inhibitors, beta-blockers or thiazide diuretics, the dose of Amlorus does not need to be adjusted.

Side effects

  • From the side of the cardiovascular system: often (≥1 / 100, <1/10) - a feeling of heat and flushing of the skin of the face, peripheral edema (ankles and feet), heart palpitations; infrequently (≥1 / 1000, <1/100) - excessive decrease in blood pressure, vasculitis, orthostatic hypotension; rarely (≥1 / 10,000, <1/1000) - worsening of the course or development of chronic heart failure; very rarely (<1/10 000, including individual messages) - chest pain, heart rhythm disturbances (including atrial fibrillation, ventricular tachycardia, bradycardia), myocardial infarction;
  • From the side of the central nervous system: often - headache, migraine, drowsiness, dizziness, increased fatigue; infrequently - anxiety, increased sweating, general malaise, unusual dreams, peripheral neuropathy, asthenia, insomnia, nervousness, tremor, hypesthesia, mood lability, fainting, paresthesia, depression; rarely - agitation, convulsions, apathy; very rarely - amnesia, ataxia;
  • From the urinary system: infrequently - nocturia, frequent and / or painful urination, impotence; very rarely - polyuria, dysuria;
  • From the respiratory system: infrequently - epistaxis, rhinitis, shortness of breath; very rarely - cough;
  • On the part of the skin: infrequently - hair loss; rarely - dermatitis; very rarely - xeroderma, skin pigmentation disorders;
  • From the digestive system: often - nausea, abdominal pain; infrequently - thirst, anorexia, flatulence, dryness of the oral mucosa, dyspepsia, constipation or diarrhea, vomiting; rarely - increased appetite, gingival hyperplasia; very rarely - increased activity of hepatic transaminases, gastritis, jaundice (usually cholestatic), hyperbilirubinemia, hepatitis, pancreatitis;
  • From the side of metabolism: very rarely - hyperglycemia;
  • On the part of the hematopoietic organs: very rarely - leukopenia, thrombocytopenic purpura, thrombocytopenia;
  • From the musculoskeletal system: infrequently - back pain, myalgia, arthrosis, muscle cramps, arthralgia; rarely - myasthenia gravis;
  • From the senses: infrequently - taste perversion, visual impairment, conjunctivitis, eye pain, xerophthalmia, diplopia, ringing in the ears, violation of accommodation; very rarely - parosmia;
  • Allergic reactions: infrequently - skin rash, itching; very rarely - urticaria, angioedema, erythema multiforme;
  • Others: infrequently - chills, weight loss or gain, gynecomastia; very rarely - cold sweat.

With arterial hypertension, Amlorus can be combined with ACE inhibitors, alpha- and beta-blockers, thiazide diuretics.

For angina pectoris, Amlorus can be prescribed in combination with other antianginal agents, for example, beta-blockers, short-acting or prolonged-acting nitrates.

Amlodipine has no effect on plasma concentrations of creatinine, uric acid, urea nitrogen, triglycerides, low density lipoproteins, total cholesterol, glucose and potassium ions, therefore it can be used to treat patients with diabetes mellitus, gout and bronchial asthma.

Amlorus can be used to treat patients prone to vasoconstriction (vascular spasm).

During treatment, you should control body weight, limit sodium intake, adhere to a diet prescribed by a doctor, be observed by a dentist (to prevent bleeding, soreness and gingival hyperplasia), and also be careful when driving a car and performing potentially dangerous types of work.

Amlodipine does not cause withdrawal; nevertheless, it is recommended to gradually reduce the dose before withdrawing it.

Drug interactions

  • Calcium preparations: the effect of amlodipine decreases;
  • Inhibitors of microsomal oxidation: the concentration of amlodipine in the blood plasma increases and the risk of side effects increases;
  • Lithium preparations: manifestations of their neurotoxicity (ataxia, diarrhea, nausea, vomiting, tinnitus, tremors) may increase;
  • Inducers of microsomal liver enzymes: decreases the concentration of amlodipine in blood plasma;
  • Antiarrhythmic drugs that cause prolongation of the QT interval (quinidine, amiodarone, procainamide): may increase the severity of their negative inotropic action;
  • ACE inhibitors, beta-blockers, loop and thiazide diuretics, nitrates, verapamil: the antihypertensive and antianginal effect of amlodipine is enhanced;
  • Antipsychotics, alpha 1 -adrenergic blockers, quinidine, amiodarone: the antihypertensive effect may increase;
  • Antiviral drugs (ritonavir): the plasma concentration of amlodipine increases.

When using grapefruit juice during treatment with Amlorus, a decrease in the concentration of amlodipine in the blood plasma is possible, however, with a single dose of 240 mg of juice and 10 mg of amlodipine, significant changes in the pharmacokinetics of the drug are not observed.
