Amelotex is a non-steroidal anti-inflammatory drug (NSAID).

Release form and composition

  • Solution for intramuscular injection: clear or slightly opalescent liquid, yellow with a greenish tinge (1.5 ml each in colorless glass ampoules, 3 or 5 ampoules in blisters, in a cardboard box 1 or 2 packs of 3 ampoules, 1, 2 or 4 packs of 5 ampoules);
  • Tablets: round, convex on both sides, pale yellow (a slight greenish tint is possible), the presence of marbling in color and slight roughness is allowed, on 15 mg tablets on one side there is a risk (10 pcs. In blisters, in a cardboard box 1 or 2 packs);
  • Rectal suppositories: torpedo-shaped, greenish-yellow in color (1,3,5 pieces in a contour cell package, 1-4 packages in a cardboard box);
  • Gel for external use: yellow (greenish tint is possible), almost transparent or transparent, with a specific odor (30 and 50 g each in aluminum tubes, in a cardboard box 1 tube).

The active substance is meloxicam:

  • 1 ampoule of solution - 15 mg;
  • 1 tablet - 7.5 or 15 mg;
  • 1 suppository - 7.5 or 15 mg;
  • 1 g of gel - 10 mg.

Excipients of the solution: glycofurfural, poloxamer 188, meglumine, glycerol, sodium hydroxide solution 1M, sodium chloride, water for injection.

Additional components of the tablets: crospovidone, microcrystalline cellulose, lactose monohydrate, povidone, colloidal silicon dioxide, sodium citrate, magnesium stearate.

Additional components of suppositories: macrogol glyceryl hydroxystearate, solid fat.

Gel excipients: carbomer, ethanol 95%, methylpyrrolidone, trometamol, purified water, orange blossom oil and lavender oil.

Indications for use

For solution, suppositories and tablets:

  • Ankylosing spondylitis (ankylosing spondylitis);
  • Osteoarthritis;
  • Rheumatoid arthritis.

In the form of a solution, Amelotex is also prescribed for degenerative and inflammatory diseases of the joints, accompanied by pain.

In the form of a gel, the drug is used to treat osteoarthritis, accompanied by pain.

Amelotex is intended for symptomatic therapy, reducing inflammation and reducing pain, it does not affect the progression of the disease.

Contraindications

Absolute for solution, suppositories and tablets:

  • The period after coronary artery bypass grafting;
  • Inflammatory bowel diseases such as ulcerative colitis and Crohn's disease;
  • Progressive kidney disease, including confirmed hyperkalemia;
  • Cerebrovascular or other bleeding;
  • Active gastrointestinal bleeding;
  • Erosive and ulcerative lesions of the mucous membrane of the stomach or duodenum 12;
  • Severe liver failure;
  • Active liver disease;
  • Decompensated heart failure;
  • Severe renal failure in patients not undergoing dialysis (creatinine clearance <30 ml / minute);
  • Complete or incomplete combination of bronchial asthma, recurrent polyposis of the nose and paranasal sinuses, as well as intolerance to other NSAIDs, including acetylsalicylic acid (including a history);
  • Age up to 15 years;
  • Pregnancy and lactation period;
  • Rare hereditary diseases such as glucose-galactose malabsorption, lactose intolerance and lactase deficiency (for tablets only);
  • Hypersensitivity to any component of Amelotex.

Relative for solution, suppositories and tablets:

  • Cardiac ischemia;
  • Congestive heart failure;
  • Cerebrovascular disease;
  • Diabetes;
  • Dyslipidemia / hyperlipidemia;
  • Helicobacter pylori infection;
  • History of indications of the development of ulcerative lesions of the gastrointestinal tract (GIT);
  • Peripheral artery disease;
  • Creatinine clearance less than 60 ml / minute;
  • Severe somatic diseases;
  • Frequent alcohol consumption;
  • Smoking;
  • Elderly age;
  • Long-term use of NSAIDs;
  • Concomitant therapy with oral glucocorticosteroids (for example, prednisolone), antiplatelet agents (including clopidogrel or acetylsalicylic acid), anticoagulants (for example, warfarin), selective serotonin reuptake inhibitors (for example, paroxetine, sertralihnomram), phyloxetine

Absolute for gel:

  • Violation of the integrity of the skin in the places of the intended use of the drug;
  • Complete or incomplete combination of bronchial asthma, recurrent polyposis of the nose and paranasal sinuses, as well as intolerance to other NSAIDs, including acetylsalicylic acid (including a history);
  • Age under 18;
  • Pregnancy and lactation;
  • Hypersensitivity to any component of Amelotex.

Relative for the gel:

  • Peptic ulcer and 12 duodenal ulcer in the acute phase;
  • Active gastrointestinal bleeding;
  • Confirmed hyperkalemia;
  • Chronic heart failure;
  • Progressive kidney disease;
  • Active liver disease or severe liver failure;
  • Coagulation disorders;
  • Inflammatory bowel disease;
  • Elderly age.

Method of administration and dosage

Solution for intramuscular administration

In the form of a solution, Amelotex is injected intramuscularly at 7.5-15 mg once a day.

The highest permissible daily dose is 15 mg.

Patients with severe renal failure on hemodialysis should not exceed a daily dose of 7.5 mg.

Tablets

In the form of tablets, the drug should be taken orally 1 time per day.

Recommended daily doses:

  • Rheumatoid arthritis - 15 mg, if necessary, the dose is reduced to 7.5 mg;
  • Osteoarthritis - 7.5 mg, if necessary, the dose is increased to 15 mg;
  • Ankylosing spondylitis - 15 mg.

The highest permissible daily dose is 15 mg.

Patients with severe renal failure on hemodialysis should not exceed a daily dose of 7.5 mg.

Rectal suppositories

Suppositories are injected deep rectally, having previously been freed from the contour packaging, 1 pc. per day.

Recommended dosage regimen:

  • Osteoarthritis: 7.5 mg (if necessary, it is possible to use suppositories at a dose of 15 mg);
  • Rheumatoid arthritis, ankylosing spondylitis: 15 mg (if a positive therapeutic effect is achieved, the dose can be reduced to 7.5 mg).

The maximum daily dose is 15 mg.

Gel for external use

In this dosage form, Amelotex should be applied to clean, dry skin over the lesion and rubbed lightly for 2-3 minutes.

A single dose is 2 g of gel (strip approximately 4 cm long). Frequency rate of application - 2 times a day.

The duration of treatment is determined individually depending on the indications, the severity of the course of the disease, the site of the lesion and the therapeutic effect of the drug, but should not exceed 4 weeks.

Side effects

Possible side effects when using Amelotex in the form of a solution, suppositories and tablets:

  • Allergic reactions: anaphylactoid and anaphylactic reactions, angioedema;
  • From the nervous system: tinnitus, headache, dizziness, drowsiness, emotional lability, confusion, vertigo, disorientation;
  • From the digestive system: constipation or diarrhea, stomatitis, abdominal pain, nausea, vomiting, flatulence, hepatitis, colitis, esophagitis, erosive and ulcerative lesions and perforation of the digestive tract, belching, gastrointestinal bleeding (including latent), gastritis, hyperbilirubinemia, increased activity of hepatic transaminases;
  • From the respiratory system: bronchospasm;
  • From the urinary system: increased serum urea concentration, hypercreatininemia, edema; in rare cases - albuminuria, interstitial nephritis, hematuria, acute renal failure;
  • From the senses: visual impairment (including blurred vision), conjunctivitis;
  • From the side of the cardiovascular system: rush of blood to the skin of the face and upper chest, increased blood pressure, peripheral edema, palpitations;
  • From the side of hematopoiesis: leukopenia, anemia, thrombocytopenia;
  • On the part of the skin: photosensitization, urticaria, bullous rashes, skin rash, erythema multiforme, pruritus, toxic epidermal necrolysis;
  • Local reactions with intramuscular injection: burning and pain at the injection site.

Possible side effects when using Amelotex in the form of a gel:

  • Allergic reactions: peeling, papular-vesicular rashes, photosensitivity, hyperemia;
  • Allergic reactions: systemic anaphylactic reactions, erythema multiforme, itching and burning of the skin, urticaria.

During treatment, the elderly, patients with chronic heart failure and symptoms of circulatory insufficiency, patients with cirrhosis of the liver, and patients with hypovolemia as a result of surgery should be under constant observation and control of indicators of renal function.

With the simultaneous use of different dosage forms of Amelotex (for example, tablets and gel for external use), it should be borne in mind that the total daily dose of meloxicam should not exceed 15 mg.

Solution for intramuscular administration and tablets

Patients with a history of gastric ulcer and duodenal ulcer and in patients receiving anticoagulant therapy have a high risk of developing erosive and ulcerative diseases of the gastrointestinal tract.

In patients with renal insufficiency (provided that creatinine clearance is more than 30 ml / minute), the dose of Amelotex should be adjusted.

With a persistent and significant change in liver function indicators, including an increase in transaminases, the drug should be discontinued and control tests should be performed.

Patients who take diuretics during treatment with Amelotex should consume plenty of fluids.

If allergic reactions develop, the drug is canceled.

Like other NSAIDs, meloxicam can mask the symptoms of an infectious disease.

Amelotex can cause drowsiness, dizziness, and headache. If they develop, you should refrain from driving and performing potentially dangerous types of work.

Rectal suppositories

The use of rectal suppositories requires caution in the presence of diseases of the gastrointestinal tract in history.

Gel for external use

The drug should be applied only to intact skin, avoid contact with open wounds, eyes and mucous membranes.

To reduce the risk of side effects, Amelotex should be used in the minimum effective dose for a minimally short course. If the condition does not improve, you should consult your doctor.

To avoid the risk of developing photosensitivity reactions during treatment, you should avoid ultraviolet irradiation and visiting a solarium.

In the form of a gel, Amelotex does not adversely affect the speed of reactions and the ability to concentrate.

Drug interactions

Solution for intramuscular injection, suppositories and tablets:

  • Other NSAIDs: increased risk of gastrointestinal ulcers and gastrointestinal bleeding;
  • Selective serotonin reuptake inhibitors: increases the likelihood of gastrointestinal bleeding;
  • Lithium preparations: the concentration of lithium in the blood plasma increases;
  • Intrauterine contraceptives, antihypertensive drugs: their effectiveness decreases;
  • Myelotoxic drugs: the manifestations of hematotoxicity of Amelotex are enhanced;
  • Indirect anticoagulants, thrombolytics (streptokinase, fibrinolysin), heparin, ticlopidine: the risk of bleeding increases (it is necessary to periodically monitor blood clotting indicators);
  • Methotrexate: the drug's myelodepressant effect is enhanced;
  • Cholestyramine: the elimination of meloxicam is accelerated;
  • Diuretics: the risk of developing renal dysfunction increases;
  • Cyclosporine: the nephrotoxic effect of meloxicam is enhanced.

In the form of a gel, Amelotex should not be used simultaneously with other NSAIDs and agents intended for external use.
