Alzepil is a selective brain acetylcholinesterase inhibitor used to treat Alzheimer's disease.

Release form and composition

Dosage form - film-coated tablets: round, convex on both sides, almost white or white, engraving "E381" (5 mg tablets) or "E382" (10 mg tablets) on one side, practically odorless (by 14 tab. In a blister, in a cardboard box 2 or 4 blisters).

1 Alzepil tablet contains:

  • Active ingredient: donepezil hydrochloride - 5 or 10 mg (5.21 and 10.42 mg of donepezil hydrochloride monohydrate, respectively);
  • Auxiliary components: low-substituted hydroxypropyl cellulose (L-HPC B1), titanium dioxide, hypromellose, magnesium stearate, microcrystalline cellulose, macrogol 400, white opadry Y-1-7000.

Indications for use

Alzepil is a drug for the symptomatic treatment of Alzheimer's dementia of any severity.

Contraindications

Absolute:

  • Age under 18;
  • Pregnancy and lactation period;
  • Hypersensitivity to drug components or other piperidine derivatives.

Relative (Alzepil is taken with caution):

  • Bronchial asthma;
  • Chronic obstructive pulmonary disease;
  • Heart rhythm disorders;
  • Peptic ulcer and 12 duodenal ulcer;
  • Surgical interventions using general anesthesia;
  • Simultaneous use of non-steroidal anti-inflammatory drugs, anticholinergics or other cholinesterase inhibitors.

Method of administration and dosage

Alzepil should be taken orally 1 time per day, before bedtime.

The initial dose is 5 mg. The duration of its administration is 4 weeks, after which, if necessary, the dose is increased to 10 mg / day in 1 dose.

Supportive therapy can be carried out as long as the therapeutic effect of the drug persists (it should be regularly evaluated). If Alzepil stops working, it must be canceled.

Patients with impaired liver function (regardless of severity) do not need dose adjustment, since these conditions do not affect donepezil clearance.

Side effects

  • From the digestive system: very often (≥1 / 10) - diarrhea, nausea; often (≥1 / 100, <1/10) - dyspepsia, vomiting, gastrointestinal disorders, anorexia; infrequently (≥1 / 1000, ≤1 / 100) - gastric and duodenal ulcers, bleeding from the gastrointestinal tract;
  • From the nervous system: often - dizziness, increased fatigue, headache, hallucinations, insomnia, muscle cramps, aggressive behavior, agitation, fainting *; infrequently - convulsive seizures *; rarely (≥1 / 10,000, ≤1 / 1000) - extrapyramidal symptoms;
  • From the urinary system: often - urinary incontinence; rarely - liver dysfunction (including hepatitis);
  • From the side of the cardiovascular system: infrequently - bradycardia; rarely - atrioventricular and sinoatrial block;
  • Dermatological reactions: often - itching and rash on the skin;
  • Other: flu-like syndrome, pain of various localization;
  • Laboratory tests: infrequently - a slight increase in the activity of the muscle isoform of creatine phosphokinase in the blood serum.

* Patients with fainting and seizures may have heart block.

special instructions

There is no information on the development of a withdrawal syndrome in the event of a sharp discontinuation of Alzepil, there is a gradual decrease in the therapeutic effect of the drug.

If an operation is required under general anesthesia, it should be borne in mind that donepezil may increase the severity of neuromuscular blockade caused by depolarizing muscle relaxants.

Alzepil can have a vagotonic effect on heart rate (in particular, cause bradycardia). The likelihood of such an effect is important in patients with sick sinus syndrome and other supraventricular conduction disorders (for example, atrioventricular or sinoatrial block).

Cholinesterase inhibitors are presumably capable of causing generalized seizures to a certain extent. However, seizure activity can also be a manifestation of the underlying disease.

The drug is capable of increasing the secretion of hydrochloric acid in the stomach, therefore, special observation during treatment is required for patients who have a risk of developing stomach and duodenal ulcers.

Although this effect has not been seen in clinical studies, the likelihood of urinary retention must be considered.

Alzepil can cause dizziness, fatigue and muscle cramps, especially at the beginning of therapy and when the dose is increased. In addition, the very disease for which the drug is used may be accompanied by a violation of the speed of reactions and the ability to concentrate. For this reason, the question of the possibility of driving a car and performing potentially dangerous types of work should be resolved individually with a doctor.

Drug interactions

Alzepil should be used with caution at the same time as CYP3A4 and CYP2D6 inhibitors (for example, quinidine, ketoconazole, itraconazole, erythromycin, fluoxetine), as they inhibit donepezil metabolism.

Isoenzyme inducers such as ethanol, phenytoin, rifampicin, and carbamazepine can reduce plasma donepezil concentrations. Since the degree of this effect is unknown, such combinations should also be used with caution.

Donepezil has an effect on the effects of drugs that have anticholinergic activity.

With the simultaneous use of cholinergic receptor agonists, quaternary anticholinergic drugs (for example, glycopyrronium bromide), atypical changes in blood pressure and heart rate are possible.

In the case of combined use, donepezil can enhance the effect of suxamethonium bromide and other muscle relaxants, cholinergic receptor agonists, beta-blockers, which affect cardiac conduction.
