Composition and form of release of Enam

The drug is available in the form of tablets at a dosage of 2.5 mg, 5 mg and 10 mg. The active substance of the drug is enalapril maleate.

According to the instructions, Enam is an antihypertensive drug. The active ingredient is rapidly absorbed into the body, forming a pharmacologically active substance - enalaprilat. The use of Enam inhibits the angiotensin-converting enzyme in the body, preventing the conversion of angiotensin I to angiotensin II, which helps to reduce the total peripheral vascular resistance, reduces the level of aldosterone and increases the level of bradykinin, activates depressive systems and inhibits pressor systems.

Reception of Enam according to indications leads to a decrease in diastolic and systolic pressure, without developing reflex tachycardia.

According to reviews, Enam reduces left ventricular hypertrophy and reduces myocardial oxygen demand. In conditions of hypoxia, it improves the condition of the heart muscle. The drug prevents the development of diabetic nephropathy, increases renal blood flow, which leads to a decrease in cardiac mortality. Enam has no effect on carbohydrate and lipid metabolism.

Indications for Enam's use

According to the instructions, Enam is prescribed for the treatment of patients suffering from arterial hypertension in various forms, including essential arterial and renovascular hypertension.

In addition, the indication for Enam is chronic heart failure (as part of a combination therapy).

The drug is used to prevent coronary ischemia and severe heart failure in patients with reduced left ventricular function.

Method of application of Enam and dosage regimen

According to the instructions, Enam is intended for oral administration. The drug is taken regardless of food intake, as a rule, once a day at approximately the same time. The dosage of the drug depends on the age of the patient and the indication for Enam.

With essential hypertension in adults, as a rule, the dosage is 2.5-5 mg per day. The maximum dosage of the drug is 40 mg per day.

Before taking enalapril, it is recommended that patients taking diuretics stop taking them at least three days in advance. Enam therapy is started with a minimum dosage, gradually increasing it to the optimal one.

With heart failure in adults, the initial dosage is 2.5 mg per day, with a gradual increase to 20 mg.

With reduced renal function, the dosage of enalapril needs constant correction. The maximum daily dosage for people with reduced creatinine clearance from 30 to 80 ml / min is 10 mg, and with creatinine clearance from 10 to 30 ml / min - 5 mg. For patients on hemodialysis, the maximum daily dosage is 2.5 mg.

The intervals between dosage increases should be at least two weeks.

For children over six years of age with arterial hypertension, the initial dosage of Enam is 2.5 mg per day. The maximum daily dosage for children is calculated on the basis of 0.58 mg per kg of body weight.

Contraindications

The instructions for Enam contain the following contraindications:

  • hypersensitivity to enalapril and other components of the drug;
  • aortic and mitral stenosis;
  • the period of pregnancy and breastfeeding;
