Pharmacological properties

Pharmacodynamics

Trental 400 is an angioprotective agent that improves microcirculation in the area of impaired blood circulation. The improvement of the rheological properties of blood occurs due to the effect on the pathologically altered deformability of erythrocytes, inhibition of the process of platelet aggregation and a decrease in the increased viscosity of blood.

Pentoxifylline is a xanthine derivative, its mechanism of action is associated with inhibition of phosphodiesterase and the accumulation of cyclic adenosine monophosphate (cAMP) in the blood cells and vascular smooth muscle cells. It has a weak myotropic vasodilator effect, has a slight effect on the expansion of the coronary vessels and a decrease in the total peripheral vascular resistance.

According to the instructions, Trental 400 improves the symptoms of cerebrovascular accidents.

With intermittent claudication and other occlusive lesions of the peripheral arteries, the properties of the drug are manifested in the elimination of night cramps in the calf muscles, the disappearance of pain at rest, and the lengthening of the walking distance.

Pharmacokinetics

Pentoxifylline after oral administration is absorbed from the gastrointestinal tract (GIT) almost completely.

Bioavailability averages 19 ± 13%.

The main pharmacologically active metabolite is 1- (5-hydroxyhexyl) -3,7-dimethylxanthine, its concentration in blood plasma is 2 times higher than the concentration of pentoxifylline.

The process of biotransformation of the active substance in the body occurs completely.

T 1/2 (half-life) of pentoxifylline - 1.6 hours.

More than 90% is excreted through the kidneys in the form of unconjugated water-soluble metabolites.

In severe renal impairment, the excretion of metabolites slows down.

In case of impaired liver function, there is an increase in the bioavailability of pentoxifylline and an increase in its T 1/2.

Indications for use

  • intermittent claudication and other disorders of the peripheral circulation of atherosclerotic genesis;
  • leg ulcers, gangrene and other trophic disorders associated with diabetic angiopathy;
  • cerebral circulation disorders (consequences of cerebral atherosclerosis, including dizziness, deterioration in concentration, memory impairment), post-stroke and ischemic conditions;
  • circulatory disorders in the choroid and retina;
  • otosclerosis, degenerative changes caused by hearing loss and vascular pathology of the inner ear.

Contraindications

  • acute myocardial infarction;

Instructions for use of Trental 400: method and dosage

Trental 400 tablets are taken orally, with food or immediately after a meal, swallowing whole (without chewing) and drinking plenty of water.

Recommended dosage: an average of 0.4 g (1 tablet) 2-3 times a day, the daily dose should not exceed 1.2 g, or 3 tablets.

In hepatic insufficiency, the dose of Trental 400 is adjusted taking into account individual tolerance.

Side effects

  • from the central nervous system: sleep disturbances, headache, anxiety, convulsions, dizziness; very rarely - aseptic meningitis;
  • from the digestive system: nausea, vomiting, anorexia, intestinal atony, diarrhea, xerostomia, feeling of fullness and pressure in the stomach; very rarely - increased activity of alkaline phosphatase and hepatic transaminases, intrahepatic cholestasis;
  • on the part of the blood coagulation system: bleeding from mucous membranes, vessels of the skin, stomach and / or intestines, hypofibrinogenemia;
  • on the part of the cardiovascular system: cardialgia, tachycardia, arrhythmia, lowering blood pressure, progression of angina pectoris;
  • from the hematopoietic system: thrombocytopenia, pancytopenia, leukopenia;
  • from the senses: scotoma, visual impairment;
  • dermatological reactions: flushing of the skin of the face and upper chest, flushing of the skin of the face, edema, significant fragility of the nails;
  • allergic reactions: pruritus, hyperemia, urticaria, anaphylactic shock, angioedema.

Overdose

Symptoms: nausea, chills, dizziness, lowering blood pressure, tachycardia, arrhythmia, hyperemia of the skin, areflexia, tonic-clonic seizures, loss of consciousness.

Treatment: the patient is placed on his back, his legs should be raised; then symptomatic therapy is prescribed, which ensures the maintenance of normal blood pressure and respiratory function. Since high doses of the drug while taking hypoglycemic agents can cause severe hypoglycemia, patients with diabetes mellitus require a dose adjustment of the drug taken.
