Sinestrol is a hormonal drug used to treat estrogen deficiency.
Release form and composition
The drug is produced in the form of an oily solution for intramuscular (i / m) administration / solution for injection (oily): a clear oily liquid of pale or golden yellow color, with possible precipitation of crystals or crystallization of the entire contents of the ampoule (1 ml each in an ampoule of neutral glass, in a blister strip 5 ampoules, in a cardboard box 10 ampoules or 2 blister packs, an ampoule scarifier / knife and instructions for the use of Sinestrol; if the ampoules have notches, rings or break points, the scarifier and the knife are not inserted).
1 ml of the preparation contains:
- active substance: hexestrol (sinestrol) - 1 or 20 mg;
- additional components: peach oil or olive oil.
Pharmacological properties
Pharmacodynamics
Sinestrol is a synthetic estrogen, which is close to steroid estrogenic hormones in its medicinal and biological properties, but differs from them in chemical structure. The drug is used for hypoestrogenic conditions. Its therapeutic effect is identical to the effect of follicular hormone preparations on the uterus and mammary glands, on the cardiovascular and nervous systems, as well as on the general condition of the woman's body. Sinestrol helps to increase the blood supply to the genitals, activates the processes of endometrial proliferation. May lead to engorgement and enlargement of the mammary glands as a result of overgrowth of glandular tissue.
The active substance is capable of alleviating and arresting vegetative-vascular disorders that appear in women as a result of depletion of ovarian function (menopause, condition after castration), as well as prevent the occurrence of osteoporosis.
Pharmacokinetics
The agent has a high absorption, characterized by the phenomenon of the first passage through the liver. It is excreted by the kidneys, while the rate of excretion can vary and depends on age, physiological state (for example, different phases of the menstrual cycle) and other factors.
Indications for use
- hypoplasia of the genitals;
- conditions caused by insufficient ovarian function (oligomenorrhea, dysmenorrhea, primary and secondary amenorrhea);
- infertility; post-term pregnancy, weakness of labor;
- climacteric and post-castration disorders;
- breast cancer, prostate cancer.
Contraindications
- various forms of hyperestrogenism;
- mastopathy;
- endometriosis;
- uterine bleeding of unknown etiology;
- tendency to uterine bleeding;
- liver and / or kidney disease;
- endometritis, salpingo-oophoritis and other diseases of the female genital organs, of an inflammatory nature;
- hyperfollicular stage of the climacteric period;
- diseases caused by increased blood clotting ability;
- malignant and benign tumors of the genital and other organs in women under the age of 60;
- pregnancy;
- hypersensitivity to any component of the hormonal agent.
Sinestrol, instructions for use: method and dosage
Sinestrol oil solution in ampoules is injected in / m.
Before use, the ampoule with the drug should be heated to 30–40 ° С.
The recommended dosage regimen of Sinestrol, depending on the indications:
- hypogenitalism, primary amenorrhea and underdevelopment of the uterus: 1–2 mg daily, for 4–6 weeks or more; when a positive result is achieved, then progesterone or pregnin is prescribed for 6-8 days;
- secondary amenorrhea: within 15-20 days, 1-2 mg, then use progesterone or pregnin in the prescribed dose for 6-8 days;
- infertility due to underdevelopment of the uterus: in the first 7-8 days after menstruation, 1 mg;
- oligomenorrhea: daily or every other day during the first half of the intermenstrual period, 1 mg;
- climacteric disorders: 0.5-1 mg each, the course is set individually;
- breast cancer in women over 60 years of age (solution 20 mg / ml): daily, the initial daily dose is 20 mg (1 ml), then the dose is gradually increased to 100 mg (5 ml) per day; the optimal dose is determined with careful observation, the course is long;
- adenoma of the prostate gland (solution 20 mg / ml): daily 40 mg (2 ml) for 30 days; spend with an interval of 2-3 months another 2-3 repeated courses for 20 days;
- prostate cancer (solution 20 mg / ml): daily 60-80 mg (3-4 ml) for 2 months, then in a daily dose of 10-20 mg (0.5-1 ml); the total dose and duration of therapy depend on the changes occurring in the prostate, the development or absence of metastases, the degree of feminization and the general condition.
The maximum single dose of sinestrol for adults (solution 1 mg / ml) is 2 mg, the maximum daily dose is 3 mg.
In the treatment of malignant neoplasms (solution 20 mg / ml): the maximum single dose is 60 mg (3 ml), the maximum daily dose is 100 mg (5 ml).
Side effects
During the period of Sinestrol's use, the following undesirable effects may occur: rash, eyelid edema, dizziness, nausea, vomiting, erythropenia.
In the case of a long course of therapy in men, feminization may develop (pigmentation and swelling of the nipples, a decrease in the size of the testicles, a decrease in sexual function), in women - peripheral edema (impaired sodium metabolism), ovarian sclerosis, metrorrhagia.
Overdose
If Sinestrol is administered in large doses, such adverse reactions as peripheral edema, endometrial hyperplasia and uterine bleeding, increased blood clotting, toxic liver damage are possible; severe feminization in men (see the section "Side effects").
special instructions
If crystals fall out or the entire contents of the ampoule crystallize, it is recommended to heat it up with periodic shaking in a boiling water bath. If the drug becomes transparent after dissolving the crystals, and when cooled to 36–38 ° C, they do not fall out again, then the solution is suitable for use.
Sinestrol ampoules should be used only as directed and under the supervision of the attending physician after carrying out all the necessary laboratory and instrumental studies, and also taking into account all the above contraindications.
Influence on the ability to drive vehicles and complex mechanisms
When driving vehicles and operating other complex mechanisms during the period of using the hormonal agent, care must be taken due to the possible development of dizziness.
Application during pregnancy and lactation
Sinestrol is contraindicated for pregnant women.
With impaired renal function
In renal failure, drug therapy is contraindicated.
For violations of liver function
Sinestrol is contraindicated in patients with hepatic impairment.
Drug interactions
- clomiphene citrate and other antiestrogenic agents prescribed for the treatment of infertility: the effect of estrogens is weakened as a result of blocking estrogen receptors by synestrol;
- preparations of thyroid hormones, folic acid: enhance the effect of sinestrol;
- progesterone, pregnin and other hormonal agents containing gestagens: reduce the side reactions of sinestrol, therefore it is recommended to use it in combination with gestagens;
- antiarrhythmic and antihypertensive drugs, indirect anticoagulants, lipid-lowering drugs, diuretics: sinestrol enhances the effect of these drugs.

Ovestin, Ovanelia, Estrokad, Estrovagin are analogues of Sinestrol.

Most often, in the few reviews about Sinestrol, the patients discussed the use of the drug with weak labor to stimulate delivery. They noted the positive effect of the drug after 3–7 days of therapy, which was expressed in an increase in the elasticity of the cervix and led to the opening of the uterine pharynx. What contributed to the onset of the labor process and prevented fetal overburden. Also good, according to many, Sinestrol has established itself in the treatment of primary amenorrhea, oligomenorrhea, dysmenorrhea, uterine hypoplasia.
