Release form and composition
Synvisc is a synovial fluid prosthesis: a sterile pyrogen-free solution with a viscoelastic structure (2 ml in a glass syringe, 1 syringe in polymer blisters, 1 or 3 blisters in a cardboard box).
1 ml of liquid contains:
- active substance: Gilan GF 20 (including Gilan A and Gilan B) - 8 mg;
- auxiliary components: sodium dihydrogen phosphate, sodium chloride [pH (acidity) about 7.2], sodium hydrogen phosphate, water for injection.
Pharmacological properties
Synvisc is a biological analogue of hyaluronan, which determines the viscosity and elastic properties of the synovial fluid. Hilans - derivatives of hyaluronan (sodium salt of hyaluronic acid), are repeating disaccharide units of N-acetylglycosamine and sodium glucuronate. Hylan B is presented as a hydrated gel, with an average molecular weight of Hylan A of approximately 6 million daltons. Hilans are not toxic, they are excreted from the body in a natural way, similar to hyaluronan.
The drug has higher viscoelastic properties than synovial fluid. Its use is effective at any stage of joint pathology; a higher result is achieved with active and regular load on the affected joint.
The clinical effect of the drug is to restore the viscosity of the synovial fluid, its rheological properties. This, in turn, contributes to the physiological restoration of the tissues of the joint affected by osteoarthritis, the protection of cartilaginous tissue cells from damage. The purpose of using Synvisc is to reduce discomfort and pain during joint movement. Its effect extends only to the affected joint and does not have a general effect. The therapeutic effect lasts on average for 26 weeks, although there are reports of both shorter and longer periods. For example, data from prospective clinical studies of patients with osteoarthritis of the knee joint showed the effectiveness of therapy up to 52 weeks after one course of 3 injections.
Indications for use
According to the instructions, Synvisc is indicated for temporary replenishment and replacement of synovial fluid in the treatment of pain syndrome caused by osteoarthritis of the hip, knee, ankle or shoulder joint.
Contraindications
- infected joint;
- venous or lymphatic stasis on the side of the affected joint;
- signs of severe joint inflammation;
- infectious skin lesions in the immediate vicinity of the injection site;
- established hypersensitivity to the components of the drug.
Care should be taken when prescribing Synvisc to patients with hypersensitivity to chicken protein (the drug contains a small amount of chicken protein).
There is no information on the possibility of introducing a synovial fluid prosthesis to pregnant and breastfeeding women, as well as to children under the age of 18 years.
Instructions for the use of Synvisc: method and dosage
The synovial fluid prosthesis is intended for intra-articular administration only.
The contents of the syringe are sterile and pyrogen-free and should not be re-sterilized.
If a violation of the integrity of the package is found, the drug cannot be used.
After opening, the contents of the syringe are intended for a single injection, the temperature of the drug must reach room temperature.
The synovial fluid, or intra-articular exudate, should be removed before each injection.
The synovial fluid prosthesis must be inserted strictly into the articular space. The use of fluoroscopy has been shown to determine the direction of injection, especially in the treatment of the hip and shoulder joints. In this case, the amount of ionic or non-ionic contrast medium should not exceed 1 ml for every 2 ml of Synvisc.
Depending on the joint, the size (18–22) and length of the needle for inserting the prosthesis are selected.
The procedure must be carried out in strict accordance with the rules of asepsis.
To minimize the loss of the drug, the cap must be turned before removal; during the injection, the needle must be in close contact with the syringe.
To avoid breaking the tip of the syringe, removing the cap and fixing the needle should be done without significant effort.
Synvisc should not be administered intravascularly, extraarticularly, including synovial tissues or the joint capsule.
The dosage regimen is set individually, depending on the clinical indications.
Recommended dosage:
- osteoarthritis of the knee joint: 1 injection into the knee joint 1 time in 7 days. The course of treatment is 3 injections, it ensures the achievement of the maximum therapeutic effect. If necessary, the course can be repeated, but not earlier than after 28 days, the maximum dose of the drug is 6 injections within 180 days;
- osteoarthritis of the shoulder, hip, ankle joints: 1 injection. If there is no adequate improvement in symptoms after 30–90 days, it is recommended to re-insert the synovial fluid prosthesis. Clinical experience confirms the greatest effectiveness from the second injection.
Side effects
- local reactions: short-term pain, swelling in the injection area, the appearance of intra-articular exudation;
- systemic reactions: sometimes - nausea, chills, general weakness, headache, dizziness, rash, itching, urticaria, skin redness, difficulty breathing, paresthesia, muscle cramps, peripheral edema, including facial edema.
Overdose
Overdose symptoms have not been established.

With a significant accumulation of exudate, the development of a stronger and more prolonged pain syndrome after injection is possible. In this case, a puncture of the joint is necessary to remove exudate and analyze it for infection or microcrystalline arthropathy. The development of such reactions does not affect the effectiveness of treatment, they usually disappear within a few days.
Do not administer Synvisc with significant intra-articular effusion.
After the injection, the patient should not be exposed to physical activity. A gradual recovery to normal activity is shown and should continue for several days.
Application during pregnancy and lactation
The efficacy and safety of using Synvisc during gestation and during breastfeeding have not been established, therefore, the drug is not prescribed to pregnant and breastfeeding women.
Pediatric use
The efficacy and safety of using Synvisc in patients under 18 years of age have not been established.
Drug interactions
When preparing the skin for injection, do not use disinfectants containing quaternary ammonium salts. Their presence can cause precipitation of hyaluronan.
