Ganfort is a combined antiglaucoma drug.

Release form and composition

Dosage form - eye drops: colorless or light yellow liquid with a transparent structure (3 ml each in a polyethylene dropper bottle, 1 or 3 bottles in a cardboard box).

1 ml drops contain:

  • active ingredients: bimatoprost - 0.3 mg, timolol maleate - 6.8 mg (which is equivalent to 5 mg of timolol);
  • auxiliary components: hydrochloric acid, benzalkonium chloride, sodium chloride, citric acid monohydrate, sodium hydroxide, sodium hydrogen phosphate heptahydrate, water.

Indications for use

The use of Ganforth is indicated for open-angle glaucoma and intraocular hypertension to reduce intraocular pressure (IOP) in the absence of a sufficient effect from the local use of beta-blockers and prostaglandin analogues.

Contraindications

  • syndrome of increased airway reactivity (including a period of exacerbation of bronchial asthma or an indication in the history of episodes);
  • chronic obstructive pulmonary disease (COPD) in severe form;
  • sinus bradycardia;
  • atrioventricular block II and III degree;
  • heart failure in a clinically pronounced form;
  • cardiogenic shock;
  • age up to 18 years;
  • During pregnancy and breastfeeding;
  • hypersensitivity to the components of the drug.

Care should be taken to prescribe Ganfort for neovascular, inflammatory, angle-closure glaucoma, inflammatory eye changes, congenital glaucoma, narrow-angle glaucoma, impaired liver and / or kidney function, the presence of risk factors for macular edema (including aphakia, pseudophakia, lens rupture), diabetes mellitus (unstable course), impaired glucose tolerance.

Method of administration and dosage

The drug is instilled into the conjunctival sac of the affected eye.

Recommended dosage: 1 drop, 1 time per day, in the morning.

In elderly patients, dose adjustment is not required.

If you accidentally skip the introduction of the next dose, it is skipped and instillation is carried out the next day at the prescribed time.

The maximum daily dose is 1 drop.

Side effects

Adverse events reported during the Ganforth clinical trials:

  • on the part of the organ of vision: very often - the growth of eyelashes, conjunctival hyperemia; often - itching, dryness of the mucous membrane of the eyes, burning pain or burning sensation in the eyes, pain in the eyes, superficial keratitis, corneal erosion, foreign body sensation, redness of the eyelids, discharge from the eyes, photophobia, itching of the eyelid skin, blurred vision; infrequently - conjunctival edema, swelling of the eyelids, iridocyclitis, blepharitis, irritation of the mucous membrane of the eyes, soreness of the eyelids, epiphora, trichiasis, asthenopia, decreased visual acuity; frequency unknown - cystic macular edema;
  • from the central nervous system: infrequently - headache;
  • dermatological reactions: often - pigmentation of the eyelid skin; infrequently - hirsutism;
  • from the respiratory system: infrequently - rhinitis.

Since the drug consists of two substances, it is possible to develop side effects that have occurred with monotherapy with each of them.

Side effects of drops that have been identified with the use of timolol:

  • on the part of the organ of vision: diplopia, decreased sensitivity of the cornea, ptosis, after surgery - retinal detachment, keratitis, change in refraction (in some cases against the background of the abolition of miotics);
  • mental disorders: nightmares, insomnia, decreased libido;
  • from the cardiovascular system: chest pain, cardiac arrhythmias, bradycardia, heart block, loss of consciousness, palpitations, heart failure, congestive heart failure, lowering blood pressure (BP), cerebrovascular accident, Raynaud's syndrome, intermittent claudication, cold extremities, cardiac arrest;
  • from the central nervous system: paresthesia, myasthenia gravis, cerebral ischemia;
  • from the organ of hearing and vestibular apparatus: tinnitus;
  • from the gastrointestinal tract: nausea, dryness of the oral mucosa, diarrhea, dyspepsia;
  • from the respiratory system: shortness of breath, cough, bronchospasm (more often with a history of bronchospasm);
  • dermatological reactions: exacerbation of psoriasis, psoriasis-like rashes, alopecia;
  • from the urinary system: Peyronie's disease;
  • from the musculoskeletal system: systemic lupus erythematosus;
  • others: fatigue, chest pain, swelling.

Side effects of drops that may occur while using bimatoprost:

  • on the part of the cardiovascular system: increased blood pressure;
  • on the part of the organ of vision: cataract, increased pigmentation of the iris, allergic conjunctivitis, uveitis, darkening of eyelashes, eyelid retraction, blepharospasm, retinal hemorrhage;
  • laboratory indicators: violation of the activity of liver enzymes;
  • general disorders and local reactions: peripheral edema, asthenia.

special instructions

Since the drug can enter the systemic circulation, during the period of its use, regular monitoring of the patient's condition is required.

Ganfort should be prescribed after the symptoms of heart failure have been compensated; in severe heart failure, it is necessary to control the heart rate.

The presence of timolol (beta-blocker) in the composition can cause a worsening of Prinzmetal's angina pectoris, mask signs of hyperthyroidism, severe vascular disorders, arterial hypotension, hypoglycemia, and may reduce the effect of epinephrine (adrenaline) used to stop anaphylactic reactions.

The negative effect of bimatoprost on liver function has not been established in patients with mild liver dysfunction or initially increased activity of aspartate aminotransferase (ACT), alanine aminotransferase (ALT) and / or total bilirubin.

Before starting therapy, it is necessary to inform patients about the possible effect of the drug on the growth of eyelashes, increased pigmentation of the skin of the eyelids and iris of the eyes. Some changes (including pigmentation of the iris) can become permanent, and since instillations can be carried out in only one eye, long-term use of the drug is accompanied by the development of differences between the eyes.

Patients with soft contact lenses are advised to remove the lenses before each procedure and put them back on after instillation after 15 minutes.

Due to the risk of developing acute keratitis and / or toxic corneal ulcers, careful monitoring of patients with changes in the cornea and dry eye syndrome is required.

When using the drug, aseptic rules must be observed, this will reduce microbial contamination of the contents of the vial and the risk of inflammatory lesions of the eyes. The period of use of the drops should be strictly observed, for this it is recommended to write down the date of opening the bottle on the packaging.

Since instillations of the drug can cause a temporary deterioration in vision, care should be taken not to start driving vehicles and mechanisms until vision is fully restored.
